JOB DESCRIPTION: HOME HEALTH AIDE
POSITION SCOPE:
To ensure quality and safe delivery of the home health aide service (personal care).
To assist in the provision of home care services that reflect the philosophy and standards
of Hillebrand Home Health and/or the C.O.A.
To assist clients in achieving optimal function with activities of daily living and
instrumental activities of daily living.
POSITION QUALIFICATIONS:
High school graduate or completed a G.E.D. or has minimum of 2 years life work
experience.
Must pass the home health aide proficiency test.
Is able to understand the home health aide care plan, execute instructions, and document
services delivered.
Good verbal and written communication skills with clients, families, and emergency
service systems personnel.
Demonstrates evidence of sympathetic attitude toward care of the sick.
PERFORMANCE RESPONSIBILITIES AND STANDARDS:
1. Ensures quality and safe delivery of home health care services.
a. Participates in development and implementation of client care plans.
b. Participates in client case conferences and supervisory visits.
c. The provided services reflect the care plan.
d. Information regarding care plans is submitted to the R.N. in a timely manner.
e. Documentation reflects delivery of quality home health care services.
2. Implements current home health aide services.
a. Client plans of care are discussed with the R.N. case manager as needed.
b. Clinical records are documented per H.H.H. policy and procedure.
c. Client assignments and reports are received from the R.N. case manager or D.O.N.
d. Adheres to infection control and safety policy and procedures.
e. Identify client needs and communicates them to the R.N. case manager or D.O.N.
f. Clients physical and environmental appearance should reflect quality home health
aide services.
g. Observations of client condition are accurately reported to the R.N. case manager
or D.O.N.
h. Assignments are organized and completed on time.
i. Clients requests are responded to promptly.

j. Personal Hygiene and Care: Bathing (bed, tub, shower or sponge), oral and denture
care, hair care, shaving, perineal care, skin care, hand and foot care, dressing and
grooming.
k. Mobility: turning and positioning, assisted transfers and ambulating- with or
without device, passive ROM exercises under the direction of the RN and/or
or Physical therapist.
l. Elimination: measure intake and output, assist in use and cleaning of bedpan,
bedside commode, toileting activity. Incontinence care, catheter care-limited
to cleansing\positioning of external parts of drainage systems and emptying
drainage systems.
m. Nutrition: meals planning\preparation, special diet preparation, clean eating\food
preparation areas, encourage\facilitate adequate nutritional and fluid intake,
record weight-nutritional intake.
n. Homemaking incidental to personal care services: bed making (occupied\
unoccupied), cleaning bathroom, laundry (bed linen, towels, underwear,
sleepwear, or other clothing), dishwashing, trash removal, incidental errands.
o. Identify and reports safety hazards, eliminate safety hazards with agency
approval, knowledge of emergency protocol.
p. Knowledge of basic body function and changes to report, reality orientation,
communication skills, accompany to appts.
Tasks that home health aides may NOT do.
a. administer over the counter , topical ointments, or prescription medications.
b. perform tasks that require sterile technique.
c. administer irrigation fluids to intravenous lines, foley catheters or ostomies.
d. administer food or fluids through feeding tubes.
e. administer enemas or suppositories.
Promotes effective written\verbal communication.
a. A positive work environment is promoted for other staff members.
b. Participation in staff meetings is required.
c. Consistent and concise lines of authority and responsibility are maintained.
Uses equipment and supplies effectively and efficiently.
a. Accurate utilization of equipment is demonstrated.
Complies with H.H.H. policies and procedures.
