Botox PBS criteria for neurologic disorders
Dr. Kiers is a Consultant Neurologist, with subspecialty training in Botulinum toxin injections and clinical
neurophysiology (nerve conduction studies). She is the Director of Clinical Neurophysiology at the Royal
Melbourne Hospital, and has accreditation at Epworth Hospital. She is a member of both the
Australasian and New Zealand Associations of Neurologists, and has her American Board of
Electrodiagnostic Medicine. She has extensive experience and has completed comprehensive training in
Australia and the United States of America.
A clinical consultation is required to determine if a patient’s clinical problem is likely to respond to
treatment with Botox. Following consultation, and if eligible, patients are given detailed information about
the procedure and any potential side effects. For particular conditions, muscles to be injected will be
identified using electromyography (EMG) which records muscle activity.

Neurologic disorders being treated
Below are the conditions which Lyn treats at her Brighton Spinal Group clinics on a Wednesday
afternoon. Where applicable, the relevant conditions for Pharmaceutical Benefits Scheme (PBS)
coverage to apply have been noted. Please be aware that for rebates to apply, a valid referral is required
at all times.
Hemifacial spasm
Conditions for PBS funded treatment:
 Patient must be 12 years or older
Cervical dystonia
Focal spasticity in adults
Conditions for PBS funded treatment:
 Maximum of 4 treatments per patient on any one day (two treatments per limb)
Severe Primary Axillary Hyperhidrosis of the Axillae
Conditions for PBS funded treatment:
 Patient must be 12 years or older
 Must have failed treatment with clinical strength deodorant (DRICLOR)
Blepharospasm (unilateral/bilateral)
Conditions for PBS funded treatment:
 Patient must be 12 years or older

Chronic migraine
Conditions for PBS funded treatment:
 Patient must be 18 years or older
 An average of 15 or more headache days/month, with at least 8 days of migraine for at least six
months. A headache diary must be completed for at least 1mth prior to treatment
 Must have experienced inadequate response to at least 3 preventative migraine medications
 Only applicable more than twice if the patient achieves and maintains at least a 50% reduction in
the number of headache days per month from baseline after 2 treatment cycles (each of 12
weeks duration)
For chronic migraine patients to confirm eligibility, a questionnaire and headache diary (for at least one
month) will need to be completed prior to their appointment. After the first treatment with Botox, a
headache diary will also need to be maintained to ensure ongoing eligibility for PBS funding. A form to

Migraine patients will need to continue regular review with their primary treating GP or neurologist for
migraine treatment in addition to the Botox injections.
