1. JOB SUMMARY
The post holder will work as part of the dedicated pharmacy team to provide and develop specialist
clinical pharmacy services to the Acute & Emergency and Post-Acute Medicine Care Groups. The post
holder will participate in the following:














To lead, develop, manage and monitor clinical pharmacy services across the Acute & Emergency
and Post-Acute Medicine Care Groups.
To act as a role model and to provide specialist and general advice to the care groups.
To participate regularly in multidisciplinary ward rounds delivering and promoting high quality, costeffective, patient-focused pharmaceutical care
To ensure a high level of clinical pharmacy input to areas within medicine.
To participate in relevant clinics and other patient programmes, and assist the further development
of the pharmacist’s role in this area
To promote safe drug use and identify and manage risk within the specialty
To develop, implement and audit guidelines, policies and procedures
To be responsible for financial reporting, drug usage and expenditure forecasting, and drug budget
management
To provide education and training to clinical pharmacy staff, healthcare professionals, patients and
carers within and outside the Trust
To work across the interface to ensure continuity of pharmaceutical care at admission and discharge
To participate in practice and clinical research and audit
To work with the Principal Pharmacist Clinical Services to extend the boundaries of professional
practice within the Care Groups
To deputise for the PRUH Principal Pharmacist, Clinical Services, when necessary
To be accredited for discharge prescription transcribing in order to reduce the time patients wait for
discharge medication (Drug Listing for TTO)
2. MAIN DUTIES & RESPONSIBILITIES
Service / Policy Development








To lead, develop, manage and monitor clinical pharmacy services across the medicine care
groups in line with local and national policies, guidelines and frameworks
To propose, implement and audit policies, guidelines and procedures in line with national and
local consensus and priorities.
To comment on and participate in policy and service development for the wider clinical services
in line with departmental and Trust policy
To participate in audit and practice research to investigate and where appropriate improve the
quality of service to patients
To participate in the preparation of relevant business cases to support pharmacy/medicine
development bids.
To collaborate in the development of Integrated Care Pathways and Patient Group Directions
To contribute and adhere to clinical pharmacy service standards
To report, analyse, identify and manage medication risk and demonstrate a commitment to the
Trust’s Clinical Governance agenda





To act as a specialist practitioner and point of expert, evidence-based advice for healthcare
staff within and outside the Trust
To promote and deliver pharmaceutical care for patients within and outside of the care groups
To provide a regular clinical pharmacy service to a designated ward within medicine
To participate regularly in multidisciplinary ward rounds, including post-take, to deliver and

promote high quality, cost-effective, patient-focused pharmaceutical care
To practice as a prescriber in area of own expertise, as appropriate, and provide direct high
quality of care to outpatients and inpatients within scope of practice
To participate in patient education schemes, including counselling at discharge
To ensure appropriate and timely supply of medication to patients, including participation in the
POD scheme
To liaise with relevant healthcare professionals within and outside the Trust to ensure continuity
of pharmaceutical care
To participate in late duty, weekend, bank holiday and other out of hours services, such as
post-take ward rounds, as necessary for the service
Financial / Resource Management






To monitor drug expenditure and provide appropriate information and analysis to the care
groups
To identify areas of potential cost-savings and to develop and implement systems and
policies to realise savings and contain prescribing costs
To use horizon scanning techniques to identify future high cost medications
To educate other healthcare professionals on the costs associated with drug therapy and
promote the use of evidence-based, cost-effective prescribing
To attend care group finance meetings and Drug Expenditure Committee meetings when
required
To work across the interface to ensure effective financial management with the care group
Staff Management & Training











To act as a specialist practitioner and point of expert, evidence-based advice at a local and
national level
To participate in the induction, education and training of all healthcare professionals at
undergraduate and postgraduate level
To act as a tutor for pharmacists undertaking clinical diplomas or other relevant courses
To participate in the recruitment and selection of clinical pharmacy staff as needed
To regularly train, supervise and assess junior and pre-registration pharmacy staff providing
services to wards within the care groups
To plan activities for, supervise and train pre-registration pharmacists attached to the team in
conjunction with the Principal Pharmacist Clinical Services and Lead Education and Training
Pharmacist
To supervise and support the day to day activities of technical staff providing ward based
pharmacy services
To participate in the in-service clinical pharmacy training of all pharmacy staff
To undertake CPD to ensure that knowledge is continually updated
To delegate tasks to junior staff as and when required
To deputise for the Principal Pharmacist Clinical Services when necessary
Research & Development




To regularly initiate or contribute to relevant practice research or audit projects and to present
and publish the results locally, nationally or internationally, as appropriate
In conjunction with the Principal Pharmacist Clinical Services, to develop, implement, monitor
and evaluate evidence-based protocols and guidelines
To maintain and further develop specialist information on the use of drugs in clinical conditions
relevant to the care groups
To proactively support the development of ward and clinical pharmacy services to the Division
in order to advance the safe, clinically effective and cost effective use of drugs to medical
patients

To liaise with clinical trial nursing and pharmacy staff with regards to monitoring and continuing
supply of trial medicines to patients locally and on national level
3. GENERAL RESPONSIBILITIES

To observe the rules, policies, procedures and standards of King’s College Hospital NHS
Foundation Trust together with all relevant statutory and professional obligations.
 To observe and maintain strict confidentiality of personal information relating to patients and
staff.
 To be responsible, with management support, for their own personal development and to
actively contribute to the development of colleagues.

local policies and procedures which will be made clear during your induction and subsequent
refresher training. These standards must be strictly complied with at all times.
