Pennsylvania Pharmacists Educational Foundation 508 North Third Street, Harrisburg, PA 17101-1199 Phone: 717-234-6151 Fax: 717-236-1618 Pharmacists…Advancing Patient Care 2018 Grant Announcement Grant Program Description Annually, the Pennsylvania Pharmacists Association Educational Foundation awards up to four grants with a maximum amount of $1,000 per awarded grant. Grants may be used for projects which are targeted towards advancing patient care by pharmacists and innovative activity around pharmacy practice. Objectives The purpose of the grants is to encourage exploration of innovative and best pharmacy practices especially those which focus on patient-centered services or which offer some patient benefit. Submitted projects must have a measurable result or outcome. For 2018: The PPA Educational Foundation encourages the submission of grants that investigate, demonstrate, document, measure or otherwise explore the value of pharmacist involvement on the healthcare team and other innovative models of collaboration. Of particular interest to the Foundation, are the following aspects related to the business model of embedding a pharmacist in the PCMH/ACO: productivity, ROI, payment, and/or quality outcomes/measures. This focus does not preclude applications of another topic or project. Grant requirements - - - Applicants and co-applicants must be current members of PPA at the time of application and continue through as members to grant completion. Applicants’ and co-applicants’ primary practice site must be physically located in Pennsylvania. Applicants may be pharmacy students, pharmacy residents, pharmacy technicians, or pharmacists. If a pharmacy student or pharmacy technician is submitting this request, a faculty or pharmacist member must be a co-applicant. Submit signed and completed application with complete proposal to PPA by the stated deadline. Any pharmacy or pharmacist directly involved in the project shall also be a PPA member or in the case of the pharmacy, have at least one member from that location. Within one year of the announcement of the grant award, the grant applicant must submit a final report to PPA indicating the specific outcomes. The submitted report is to follow the article guidelines for inclusion in the Pennsylvania Pharmacist, and Page 1 of 4 - may also be provided in the form of a poster or platform presentation at a PPA Conference. While not required, the report/article may be optionally submitted for the peer review process and publication in the Pennsylvania Pharmacist. Applications should include information about how any data or outcomes will be utilized or shared to potentially change or affect practice. Appropriate Use of Grant Funds Grant funds may only be used for the direct costs associated with the development and implementation of the project as described in the application. No indirect funds will be awarded. Funds will not be awarded for capital/equipment expenditures, the purchase of medications and/or medical devices or related medication/testing supplies, and/or regular administration expenses such as salaries and/or travel or convention/conference expenses for the recipient. Travel costs for patients involved in the study/project may be considered if reasonable. Monies may be used for reasonable printing costs relative to materials used for the project. Funding through and from other sources is permitted. Projects and related reports must recognize the PPA Educational Foundation as a grantor. Application Procedure The application process requires the submission of the application form and a detailed proposal with appropriate attachments. We ask all documents be combined upon submission into one pdf, if possible. Instructional Webinar The Foundation has available on its website a short recorded webinar (about 15 minutes) on the Foundation, the grant program, its goals and objectives, and the application and decision process. Watching the webinar is not required but may be helpful in providing any applicant with a greater chance of success by a full understanding of the process and expectations. Participation or non-participation will not be considered in the grant evaluation process and therefore is not a requirement or guarantee of success. Application and Proposal The Grant Application is available on the PPA website under the Foundation section, www.papharmacists.com/foundation. The application shall be fully completed and submitted according to the guidelines described on the application. In addition to the application form, a typed description outlining the detailed proposal must be included. Your description must address each of the following areas limiting your total response to 4 pages. The four page limit does not include the attachment(s) outlined in the final bullet point below or the cover application. General project description (please include how you believe this will advance the practice of pharmacy) Project objectives and evaluation strategy Methods to be utilized Page 2 of 4 Timetable for the project Budget – Include total project budget outlining specific funds being requested for this grant. Projected utilization of any data or outcomes. In other words – the point or purpose of the project or study. Attachments: Supporting documents (not included in the 4 page limit) – These would include samples of any assessment tool(s), survey(s), or patient materials you plan to utilize If these are not fully developed include basic structure and plan. Review Procedures Grant submissions will be initially reviewed by the Grant Review Committee of the Board of Directors of the PPA Educational Foundation and will take into account the following criteria. - PPA Membership of applicant - Completeness of application and proposal - Significance of the project - Clarity of project objectives - Project design - Adequacy of resources available to complete the project - Evaluation strategy and intended outcomes - Reasonableness of budget - Probability of completing the project - Utilization of resulting data or outcomes or purpose of study/project A copy of the form that the Reviewers utilize is available on the PPA website, under Foundation. Final awardees will be confirmed by the Foundation’s Board of Directors. All members of the PPA Foundation Board, including the Grant Review Committee must disclose any potential relationships or conflicts of interest with any potential awardees and if deemed relative, must recuse themselves from the grant evaluation and approval process. Members of the PPA Educational Foundation Board of Directors are not eligible to be applicants, co-applicants, or co-investigators on any grant submissions. Submissions Applications are to be submitted by electronically and must be received by November 1, 2017. Electronic submissions shall be submitted to [email protected]. The Foundation does NOT accept faxed or mailed submissions. Applications should be submitted in one pdf document. All submissions will receive a confirmation receipt. It is the responsibility of the applicant to verify such receipt. Applicants that do not receive a confirmation of receipt within 72 hours of submission have the obligation to follow-up to assure there was not an email issue and that the application was, in fact, received. Award Presentation, Timetable, and Payment Page 3 of 4 Successful awardees will be notified no later than December 23, 2017. One half of the grant will be presented immediately upon the Foundation’s receipt of the Grant Acceptance/Agreement form completed and signed by the awardee, following this initial announcement of the grant award. Awards will be formally announced at the PPA Mid-Year Conference, January 25-28, 2018, in Lancaster, PA. While not required, the Foundation encourages successful recipients to be present for this recognition and to hear the verbal presentations of the 2017 awardees. Applicants that are not selected will also receive a notice on or around the same time. This notice will include a brief explanation regarding why the grant was not selected. The remaining amount of the grant will be paid upon delivery of the final report suitable for publication and the oral presentation, if at all feasible, along with an accounting or documentation of the expenses incurred. The Foundation sincerely hopes that the grant awardee will make the oral presentation on the project at the PPA Mid-Year Conference in January of 2019.
