Dog Training Program Contract

_______(Initial) Payments. Payments may be made in the form of check, cash, or credit card. In order
to guarantee your reservation, payment for all training programs is due in its entirety at the time of
booking the training reservation and start date. Payments hold a place in the schedule and facility of
Woof Central Inc. to provide these services in lieu of taking other dogs. This payment is refundable if
cancelled (15) or more days prior to your dog's scheduled arrival. If your reservation is
cancelled within (8-14) days of arrival, your deposit will convert to a credit on your Woof Central
account for you to use within a 12-month timeframe. If your reservation is cancelled within (0 to
7) days of your scheduled arrival date your deposit is forfeited. No shows will be responsible
for the full amount of the scheduled reservation. Overdue amounts, including bounced payments or
chargebacks, will be assessed a late payment charge at a monthly rate of 1.5% or the maximum
provided by law, whichever is less. Woof Central Inc. shall have the right to recover expenses including
collection costs and reasonable attorney's fees incurred in collecting overdue amounts.
_______(Initial) Reasonable Care. Woof Central Inc. shall exercise reasonable care to maintain a
clean, safe and secure environment for the dog while in our care. The dog shall be fed properly and
regularly, housed in its own quarters, and may leave our premises as part of our training process.
_______(Initial) Training. Both the dog and the owner will receive the training indicated in this
Agreement. The dog will be trained to perform as shown by demonstration at the conclusion of the
training. Included in the residency/day training program shall be follow up training sessions with the
owner following the dog’s residency/day training. The owner understands and agrees that they have
responsibility to cooperate fully in completing the owner training sessions, and that Woof Central Inc.
cannot and does not assume or accept responsibility for the behavior or performance of the dog after it
has been returned to the owner.
_______(Initial) Expectations. Owner understands and agrees that genetics and environmental history
(e.g. past experiences, lack of training, nutrition, health, etc.) will affect the dog’s behavior. Owner
agrees to maintain the dog’s training after the training program is complete and understands that
because the training of a dog is a dynamic process, not fully in the control of any party, there are no
guarantees of any specific result, and there will be no refunds for this training.
_______(Initial) Liability. After the dog has been returned to the owner, Woof Central Inc. cannot and
does not except any responsibility for the illness, injury or death of the dog or of any person or animal,
or damage to any property, caused by the dog or resulting from its activity.
_______(Initial) Representations. Owner represents that owner is the legal owner of the dog; that the
dog is free and clear of any mortgage or other encumbrances; that the dog has not been exposed to
Distemper, Rabies or Parvo within the last thirty days; that the dog has been inoculated as required in
the Woof Central Client Application and Contract and that the dog is currently and properly licensed.

_______(Initial) Media Release. I consent that I may be photographed, videotaped, or recorded in or
around my participation at a Woof Central Inc. service or event. I acknowledge and agree that Woof
Central Inc. may use such photography, videography, or audio recordings (“Media”), including my
name, likeness, or image, and/or the name, likeness, or image of my dog, for internal education and
training purposes, in or around the promotion, marketing, or sale of any product or service of Woof
Central Inc. or in or around any other media regarding Woof Central Inc. without any additional
compensation or payment to me. If I do not want to consent to this recording or use, I must notify Woof
Central Inc. in writing before my participation in their service or event.
_____ (Initial) Arbitration. I agree to submit any disputes that may arise between myself and Woof
Central Inc., and/or the agents of Woof Central Inc., to binding arbitration in Hennepin County,
Minnesota, according to the rules of the American Arbitration Association. It is further agreed that a
judgment upon any award rendered by an arbitrator may be entered in any court having competent
jurisdiction. This clause shall not preclude parties from seeking provisional remedies in aid of arbitration
from a court of appropriate jurisdiction. In any arbitration arising out of or related to this Agreement, the
arbitrator(s) shall award to the prevailing party, if any, the costs and attorneys’ fees reasonably incurred
by the prevailing party in connection with the arbitration.
_______(Initial) I have read and agree to all the conditions above.
Email return of this contract and/or drop off of owner’s dog, shall constitute full acceptance of the terms
and conditions above.
