Request for Proposal for MHA HEN 2.0 – Pressure Ulcer Prevention Mini-Grants Proposal Deadline: Oct. 30, 2015 Overview The Minnesota Hospital Associations’ (MHA) Partnership for Patients (PfP) Hospital Engagement Network (HEN) is looking to engage organizations to strengthen and expand their pressure ulcer prevention programs. To assist members, MHA is offering up to 5 grant awards with funding up to $10,000 for hospitals to work on 6 month projects within their organization addressing this issue. Proposal content will be scored as follows: Describe the pressure ulcer prevention topic(s) that your team will address and why you feel this is important, considering the questions below. (5 points) o Does the project target at least one of the pressure ulcer focus areas for 2015-2016: pressure ulcer related to respiratory devices or preventing coccyx/sacral pressure ulcers? o What is the project’s potential to increase pressure ulcer detection, prevention, and management across the organization? Project plan: Tell us about your project. Identify the tasks, milestones and supporting activities you plan to accomplish. (10 points) Project Innovation: Is this project innovative? If so, how? (e.g., is it something brand new and/or different? Are you adapting an idea that you saw or tried elsewhere?) (10 points) Inclusion and education of staff/providers, patients and families: Describe your plans for including and educating staff/providers, patients and families in this project. (15 points) Measurement: Describe how you plan to measure the progress towards your project goals, including metrics other than the already established roadmap and outcome measures. (10 points) Applicants are encouraged to review the SAFE SKIN 3.0 Roadmap and Pressure Ulcer Toolkit for additional guidance and resources. Project Expectations Project period begins Dec. 1, 2015 and ends May 31, 2016. Awardees will be expected to: Prepare monthly progress reports, including a brief project narrative and monthly data. Participate in round robin learning calls with other hospitals working on this topic. 1|Page Prepare and submit a final project report outlining project activities and outcomes by June 30, 2016. Share project results during webinars/presentations. Eligibility Criteria Only hospitals currently participating in the MHA HEN are eligible to apply Selection Criteria Proposals will be reviewed based on demonstrated ability to meet the above criteria and expectations. Selected grant recipients will be notified by Nov. 15, 2015. Funding is limited and will be provided to the top scoring proposals. Total number of projects funded will depend on amount of funding requested. *Applicants should note this RFP is issued based on the anticipated availability of funds. Until funds are made available and a grant agreement is executed, MHA has no legal liability for any costs expended by the applicant.
