The health status of the population in Tanzania has improved
over recent years. Life expectancy has increased and underfive mortality has decreased markedly. However, maternal
mortality rates persist at a high-level and the proportion of
deliveries with skilled attendance has increased only slightly.
There are marked regional discrepancies in health status and
the gap between rural and urban areas is widening. Access to
health care services of adequate quality is still not equally
available for everyone.

Against this backdrop the Government of the United Republic
of Tanzania is implementing a coherent policy and reform
system which includes the health sector. Here, strategies
focus in particular on improving access to primary health care
services and the prevention and treatment of communicable
diseases.
Since 1998, Germany contributes to the health sector reform
process in Tanzania together with all relevant development
partners in a sector-wide approach (SWAp). German
Development Cooperation comprises financial contributions,
infrastructure development and equipment as well as advisory
services and human capacity development at different levels
of intervention. The Tanzanian German Programme to Support
Health (TGPSH), which started in 1998, aims to improve the
general health status of all women and men living in Tanzania
with a focus on particularly vulnerable groups in the Lindi,
Mbeya, Mtwara and Tanga regions.
Building upon TGPSH and closely interlinked with the
programme, the project “Enhancement of mother and
newborn health in Tanzania”, is about to start in April 2015

and will end in March 2017. Executing agency and lead
counterpart is the Ministry of Health and Social Welfare
(MoHSW). Implementing agencies comprise health institutions
in the Lindi and Mtwara regions including two Regional Health
Management and 13 Council Health Management Teams, 4
regional and 10 district hospitals, 35 health centres and some
160 dispensaries. The project has its focus on poor and
disadvantaged women and girls in Lindi and Mtwara and their
access to quality obstetric care services. Human rights, gender
equity and HIV/AIDS are cross-cutting subjects.
The project’s objective reads:
“Especially poor and disadvantaged women and girls are
increasingly using the improved services in obstetric care in
the Lindi and Mtwara regions”.
The project aims to enhance the level of obstetric care quality
in all districts of Lindi and Mtwara in order to increase
utilisation and thereby reduce the number of newborn and
maternal death cases. It supports MoHSW as well as staff of
regional and district health institutions and selected health
services in all districts of Lindi and Mtwara in a multi-level
approach. Emphasis is on capacity development at personnel
and institutional level in two intervention areas:
(I) Capacity improvement in obstetric care of mothers and
newborns
(II) Availability of operational medical equipment and
devices
In intervention area (II) GIZ assists health institutions and
services to establish and institutionalise a service and
maintenance management system for (emergency) obstetric
care medical equipment in all districts of Lindi and Mtwara.
Where it is not yet available, health services will be vested with
the necessary basic obstetric care equipment.
Results will be measured against the

improved availability of medical devices for
(emergency) obstetric care in the 4 regional, 10 district
hospitals and 35 health centres;

number of health services that provide professional
service and maintenance staff.
Implementation of intervention area (II) shall be contracted out
to a consulting firm who will work closely with the project
manager and with GIZ project staff in the two regions and in
Dar es Salaam.
GIZ places particular importance on a participatory and
iterative approach with the Tanzanian partners, a high quality
process and results documentation which also includes
transparent information about implementation costs. It is
important that after the initial assessment the Consultancy

Firm works jointly with maintenance officers designated by
hospitals, health centres, selected dispensaries and existing
maintenance workshops to further strengthen the system’s
capacity for high quality obstetric care at regional and district
level, to anchor implementation and assure sustainability.
These officers shall act as multipliers and drivers of a service
and maintenance culture in their respective institutions beyond
the assignment. The selection process for these officers will be
initiated by GIZ ahead of the assignment.
The implementation will focus on regional and district
hospitals, health centers, a selection of dispensaries and the
few existing maintenance workshops in the two regions. The
tasks of the Consulting Firm are:

Assessment of the state of service and maintenance
in (emergency) obstetric care in the regional and
district hospitals, health centres and selected
dispensaries in Lindi and Mtwara including existing
maintenance workshops.
o Framework (policies, rules, regulations, standards
and their state of implementation, roles and
responsibilities and their enforcement at national,
regional and district level
o Personnel situation (number of service and
maintenance workforce, qualification, professional
knowledge, training opportunities)
o Infrastructure (building standards; available obstetric
care equipment; power supply including wiring;
water supply and sewage; hospital waste disposal)
o Former/existing initiatives (financial and technical
support provided by other donors; lessons learned
o Intervention (recommendation for meaningful and
sustainable short-, mid- and long-term measures)
o Presentation of assessment results to GIZ and
partners in a 1.5 day workshop.

Concept development for and implementation of a
regional Service and Maintenance Management

the hospitals at regional and district level, health
centres and maintenance workshops.

cooperation with the designated maintenance
officers for implementation at national, regional and

o Establishing a maintenance culture
o Strengthening the existing workshops

Training of service and maintenance personnel
o assessing and analysing existing training
opportunities;
o evaluating possible partnerships;
o recommending future comprehensive training
courses and short term courses on preventive
maintenance with emphasis on training trainers;
o developping options for institutionalisation at
national, regional and district level.

Equipment of necessary basic obstetric care in
regional and district hospitals, health centres and
selected dispensaries
o Needs assessment together with the designated
maintenance officers;
o budgeting;
o tendering;
o procurement;
o installation;
o provision of spares;
o assignment of responsibilities and
o recommendations for institutional anchoring
GIZ reserves the right to do the procurement (dependant on
the results of the assessment).
In addition to the efforts of the consulting firm, the project in
cooperation with TGPSH will support needs oriented
budgeting at regional and local institutional level to secure the
future provision of sufficient funding for service and
maintenance, provision and training of staff as well as for new
equipment in obstetric care.
Personnel: Specification of

The project “Enhancement of mother and newborn health in
Tanzania” is looking for an international consulting firm.
However, implementation should draw on locally available
expertise and be in close collaboration with national
(Tanzanian) partners.
The consultancy firm must have proven experience in

hospital service and maintenance management;

facility management;

quality management;

organisational development;

(emergency) obstetric care;

training of (technical) personnel;

assessment, budgeting, tendering, procurement and
installation (for and of medical care equipment)
The company is expected to cover the service package
described above in all aspects, to provide the experts specified
below and to provide the technical and conceptual
backstopping through
 1 senior international technical advisor with
longstanding experience in policy advice, management
and implementation for up to 6 Expert Months, EM,
(situation assessment, reporting, conceptualisation;
overall coordination, backstopping). The expert has
more than 5 years practical experience at policy,
management and implementation level in Europe or
the US and in resource poor settings and can
demonstrate a relevant professional network
(memberships, publications etc.).
 1 senior regional or national technical advisor based in
Tanzania throughout the assignment with longstanding
experience in the areas mentioned above as technical
advisor, team leader and national coordinator (24
expert months, EM)
 a pool of international, national and regional short-term
(STE) and long-term experts (LTE). The latter will be
based in the Lindi and Mtwara regions. The pool
ensures state of the art technical advice and support to
the equipment supply and maintenance training
