Job Description Title: Multiple Sclerosis Clinical Nurse Specialist Accountable to: Clinical Manager Main Purpose: Client group includes all visitors and patients with pending and confirmed diagnosis of Multiple Sclerosis. To collaborate in the development of Revive MS Support services for people with a Multiple Sclerosis, ensuring services are in line with strategic guidelines/directives. To facilitate the assessment, planning, and delivery of care for our clients, in order to obtain the best possible outcome. To provide an information resource, counselling, education and practical support for clients and carers as well as Revive staff. To provide an expert link between the voluntary sector, Health Boards, and community services. To act as a clinical support to other members of the multidisciplinary team. General Duties: Clinical/Professional 1. To provide a direct link for clients and their carers between Revive, health professionals, community and voluntary services to ensure continuity of care. 2. To provide expert advice for clients, families, health care professionals and multiagency teams in all aspects of managing Multiple Sclerosis. 3. To arrange the assessment of new clients and oversee their Guided Journey. 4. To provide nursing input to the Outreach Clinics as required. 5. To demonstrate specialist knowledge and skills through nursing practice. 6. To provide expert communication skills for any given situation. 7. To maintain a high profile in the clinical area and act as a role model for other health care professionals. 8. To maintain own expert levels of clinical competence within speciality. 9. To be accountable to the Clinical Manager and provide information as required. 10. To represent Revive MS Support and take part in recognised local and national interest groups. 11. To be an educational resource to other clinicians within the team. 12. To have the ability to give presentations and or talks to external organisations as well as within Revive MS Support. 13. To provide counselling, advice and health promotion as necessary. 14. To participate in multiprofessional goal planning meetings for clients and carers. 15. To work within the agreed policies of Revive MS Support, and other relevant legislation. Education 1. To provide information, using a variety of media, about Multiple Sclerosis, its implications, consequences and practical/symptom management to clients and their families and to assess their level of understanding. 2. To produce multimedia information relevant to the client group. 3. To develop and support current education programmes for client, carers and health professionals. 4. To maintain links with other MS Nurses and Therapy specialists in the field in order to share or learn from examples of good practice. 5. To be responsible for your own continuing professional development. 6. To participate in an annual appraisal and agree a Personal Development plan. Research, Audit and Quality 1. To maintain client records and data bases as necessary. 2. To critically evaluate relevant research findings in order to develop practice. 3. To liaise with, and provide training opportunities as required, with Student Nurses and other health professionals. 4. To manage and monitor own workload, maintaining work boundaries. Management 1. To manage own client caseload. 2. To advise clients, families and Revive staff on health and safety issues and risk management. 3. To support the implementation and monitoring of nursing/clinical practice guidelines including standards, policies and protocols in line with national directives. 4. To attend relevant Clinical Group meetings and disseminate information/ follow through actions to other team members. Limits of Authority: The post-holder has no authority to: Discipline or dismiss staff without prior consultation with the Senior Management Team. Change agreed policies and procedures. All clinical professionally regulated staff must abide by the codes of conduct issued by their respective regulatory bodies and ensure that they maintain updated registration as required by the role. Safeguarding All staff providing services to clients are reminded of their responsibility to adhere to Safeguarding policies. Confidentiality All employees of Revive MS Support are reminded of the need to treat all information, particularly clinical and management information, as confidential. This job description will be reviewed yearly as part of the annual individual performance review, to ensure that it reflects the responsibilities of the post. No changes will be made without full consultation with the post holder.
