Medical Home CHARGE TO THE NEGC MEDICAL HOME WORKGROUP: • improve care and outcomes for children diagnosed with genetic conditions by identifying and testing improvements in the way specialty care, primary care, and families work together Greg Prazar, MD, Chair Highlights of the past year • Adding new members – Members from every NE state, pending MA • Webinar series on care coordination – May 26th at noon: “Balancing Strengths & Needs: Family Centered Conversations” • Monthly telephone calls – 2 guest presentations • Committed to learning from other NEGC work groups Lessons Learned/Future Work • Funding significantly helps to propel any project. • Family involvement in our workgroup is essential. • Future Medical Home projects will need to address behavioral and mental health concerns affecting children with complex and genetic conditions. continued • Our workgroup would also like to sponsor a workshop led by Family Voices representatives, since we believe families have a great deal to teach medical and non-medical professionals about optimal care for children with genetic and metabolic conditions. • Understanding the work of other NEGC workgroups is critical in providing our workgroup with ideas for future projects.
