NC Pre-Kindergarten Program
Dear Parent / Guardian,
Thank you for expressing an interest in the Stanly County Pre-K Program for 2016-2017. Enclosed you
will find the North Carolina Pre-Kindergarten Application for 2016-2017. The information obtained will
be used to help determine your child’s eligibility for the North Carolina Pre-K program for next school
year. Please complete the application and return it to Stanly County Partnership for Children as soon as
possible, but no later than June 1, 2016. If you need help completing the forms you may call with your
questions or schedule an appointment to come in for assistance.
Please return the following documents:




Completed NC Pre-K Application (please make sure to sign & date)
Income Verification (ex: pay stubs, 2015 W2’s)
Copy of child’s birth certificate
Copy of proof of residency to include physical address with no P.O. Boxes. The following
documents can be accepted: (signed lease agreement, rental receipt, utility bill (gas, water,
electric) current listing on income tax form, home ownership proof, personal property tax listing,
banking statements and payroll addresses to domicile; NO phone or cable)
We will copy the birth certificate and proof of residency at the Partnership for Children to help you if
needed.
Screening of children will be scheduled at a later time.
