PRIMARY PURPOSE OF THE JOB:
To ensure quality and safe delivery of health care services in home and/or facility in accordance with the nurse
practice act, Title 22 and the federal register, patient plan of treatment, and agency policies and procedures under the
supervision of a registered nurse / Director of Patient Care Services/Associate Director of Patient Care.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
Licensed Vocational nursing personnel shall perform, but not be limited to, the following:
(1) Prepare documentation and clinical notes, completed by the end of your shift and turned into office within
48 hours.
(2) Re-evaluate the patient’s nursing needs q shift, updating the physician orders, medication profile, MAR &
POT as required.
(3) Prompt reporting to immediate supervisor of any and all incidents. Written report on appropriate form in
agency within 24 hrs of event.
(4) Inform the physician, assigned nursing personnel, A.D.O.P.C., and scheduling coordinator of
changes in the patient’s condition and needs.
(5) Initiate appropriate preventive and rehabilitative nursing procedures.
(6) Counsel and instruct the patient, family, and other nursing personnel in meeting nursing and related
needs.
(7) Assume responsibility for professional growth by taking advantage in participating in-service training
programs.
(8) Implement infection control and safety measures at all times. Report infection for the client and the care
giver as well as unsafe condition or hazards if applicable. (Universal Precautions at all times).
(9) Adhere to the policies, procedures and standards of the facility to which assigned.
(10) Follow all regulation guidelines, ie; mandatory reporting, Title 22, and Medicare CoP’s.
(11) Provides direct patient care, evaluates outcomes, consult with other practitioners as required and adjusts
nursing care processes as appropriate to ensure optimal patient care under direction from the RN.
(12) Provide planned educational programs and information to individual patients and family by modifying
teaching strategies or content. Integrates education during the delivery of patient care.
(13) Advocate for patient rights and identify potential conflict. Seek counsel for resolution of
conflict.
(14) Demonstrate competence to provide patient specific care which includes factors such
as physical condition, diagnosis and it’s meaning to the patients.
(15) Administer medications and treatments as prescribed by the physician/RN.
(16) Use supplies and equipment effectively and efficiently.
(17) Accept responsibility of assignment to perform specialized procedure such as IV therapy, only when
qualified with specialty training, proven competency, clinical background, and expertise in the specialty
area.
(18) Provide appropriate lifting and moving techniques of the client.
(19) Gives report to Case Manager and consult when needed with the RN for guidance and direction. May
consult with ADOPC if needed.
(20) Demonstrate knowledge and awareness of legal issues in all aspects of patient care.
MARGINAL DUTIES AND RESPONSIBILITIES:
Perform special projects or other related work as required or requested.
ESSENTIAL PHYSICAL FUNCTIONS:
Occasional walks to other areas within or outside the facility or home. Occasional climbing of stairs or ladder;
occasional crouch, kneel or balance positions required to provide nursing care. Frequent bending, stooping,
squatting, reaching, occasional lifts and carries up to fifty (50) pounds. Pushing, pulling frequently increased to ten
(10) pounds and occasionally increasing to greater than one hundred (100) pounds. Sharp hearing acuity required,
precise visual acuity with correction. Color differentiation and manual dexterity required. Medium to high mental
demand capabilities.
QUALIFICATIONS:
Must have one (1) year of prior professional nursing experience. Demonstrates the ability to read and write English
and perform basic math functions, including addition, subtraction, multiplication, and division at a level typically
acquired through graduation from high school or equivalent work experience.. Must demonstrate effective and
diplomatic oral and written communication skills. Must posses: a current licensed vocational nursing license from
the State of California board of licensed vocational nurses and psychiatric technicians; current CPR certification;
valid California drivers license; proof of current auto liability insurance for driver with good driving record; current
health statement to be renewed every two years that states free from communicable disease and able to perform job
duty requirements; current negative PPD (to be renewed annually) or chest x-ray; passing score of 75% minimally on
appropriate pre-employment testing.
