LABORATORY DECONTAMINATION CERTIFICATE
Confirmation that areas to be vacated are free of any hazardous materials (such as radiation,
biological or chemical hazards) is essential for either contractors starting refurbishment work or
new ‘tenants’. The certificate must be signed off by a senior member of the laboratory group and
sent to the Departmental Safety Advisor immediately after completion, or vacation of the facilities.
Group Leader:
Identify all laboratory areas / rooms covered by this certificate:
1. I confirm that all chemical, radiological and / or biological materials have been
removed from the areas identified above.
2. I confirm that all facilities identified above, including fixtures such as sinks / fume
cupboards / microbiological safety cabinets / benches etc are, as far as is reasonably
practicable, free of any chemical, radiological and / or biological contamination.
