There are still an unknown number of blood banks operating independently under state or private sponsorship, without registration and uniform quality standards or regulations. The level of training undergone by the staff entrusted with blood transfusions or administration of blood products varies 81174735-81940517 1 considerably between services. Blood products are often not used appropriately, i.e. to a large extent doctors do not use blood components rationally in accordance with the standard recommendations issued by the World Health Organisation (WHO). Consequently, patients are not always given the correct product, potentially resulting in serious risks to their health. Blood products are administered which are of inadequate quality and in some cases infected. This gives rise to the risk of transmitting infectious diseases (i.e. HIV/AIDS, hepatitis B and C) whose prevalence in Pakistan is estimated at 9%. Preventing infection with these diseases by using safe blood products for transfusion can curb the need for costly long-term treatment of those infected and reduce the suffering of patients. It is frequently the case in Pakistan that blood products are made available by family members or paid donors. With both practices it is to be expected that an above-average proportion of specific donor types will have a higher risk of infectious diseases. The WHO recommends setting up a blood donation system based on voluntary donors, as these are among the safest blood donors. Concepts for recruiting voluntary, unrecompensed blood donors do exist, but so far it has only been possible to implement them in part and not on a sustainable basis. In March 2009 the Government of Pakistan adopted a national framework plan for blood transfusion services (National Blood Policy and Strategic Framework 2008-2012), which was drawn up with support from GTZ. This plan aims at reforming and restructuring the organisational setup and revising the legal framework of a national blood transfusion system at the federal and provincial level. Further it deals with the needs for training of experts and managers at the respective private and state-run institutions and refers to building a voluntary blood donation scheme. The "National Safe Blood Transfusion Programme" was established. With the amendment to the constitution in 2011 (18th amendment) the mandate for the health service delivery, including blood transfusion was given to the provincial governments. The firm of consultants is responsible to provide technical support to federal and provincial partners in the implementation of Output B of HSS II in following main 81174735-81940517 2 intervention areas: Providing advisory framework for the development of policy, law and standards related to provincial blood systems Support and facilitation to harmonize and coordinate provincial blood safety policies and laws Establishment of quality standards for the provincial blood system including blood processing Establishment of management information systems (MIS) (including feedback mechanisms) Establishment of an effective “Blood Donor Organization” (BDO) Network Providing technical advice for establishing a regulatory framework and for the design of accreditation processes (via Pakistan National Accreditation Council) applicable to secondary facilities i.e. blood banks and professional associations of haematologists Concept design and implementation of human capacity development in staff at the Regional Blood Centers (RBCs), Hospital Blood Banks (HBBs) and Blood Transfusion Authorities (BTAs) Personnel: Specification of inputs and necessary qualifications One long-term expert (up to 15 person-months, fulltime) to provide consultancy services to the national and the provincial "Blood Transfusion Authorities (BTA)" and the "Blood Transfusion Services (BTS)" The expert offered by the Firm of Consultants must have a postgraduate degree in public health and at least ten years of experience in health system development with particular emphasis on blood transfusion systems. Regional experience and experience of working in a fragile environment will be of advantage. English and German languages are mandatory. National long-term experts for a total of up to 45 person-months The experts offered by the Firms of Consultants must have the necessary sociocultural skills. Client and partner orientation, efficient work practices, interdisciplinary thinking and training experience are mandatory, especially for the long-term experts. Fluency in verbal and written English is mandatory. 81174735-81940517 3 In particular: - - - Senior health expert with not less than ten (10) years of experience in organizational development, health systems development, and experience in coordinating a team of experts at national level. This senior expert will assist and support the team leader in his technical and management tasks. One (1) qualified health professional with a degree in Quality Management in health care with working experience of not less than 5 years in this area after the post graduate degree One (1) qualified health professionals with proven experience in capacity development (Human Resources for Health); an expert in health care with at least 5 years of practical working experience in this field after the post graduate degree. International short-term experts for up to a total of 8 person-months National short-term experts for up to 4 personmonths All short-term experts must have a post graduate degree in relevant subjects, at least 10 years of experience in the respective area of expertise and a proven track record of similar assignments in other countries. Fluency in verbal and written English is mandatory.
