Demographic details: 1. Please enter the patient study number in a 4-digit format (for example the study number for the first patient chart you abstract data from would be 0001/0101/0201 and so on). 2. Please enter the patient’s QCC ID in a 8-digit format (for example 07723456). 3. Please enter the patient’s date of retrieval in dd/mm/yyyy format (for example 01/01/2016). 4. Please enter patient’s age in years in a three-digit format (for example 018). 5. Please circle patient’s gender. 6. Please enter patients weight in kilograms in a three-digit format (for example 099). B. Sedation details: 1. For Questions 7 and 8, please specify the names of drugs used specifically for sedation (so opiates do not count even if they have sedating properties), routes of administration (IM/IV/PO/IO/SC/IN), total dose in milligrams (if patient is receiving a drug infusion, please document the calculated total dose of the drug) and, for post retrieval team arrival sedatives only, time of administration in hh:mm format. For infusions please document the start and end times of the infusion. ‘Pre-retrieval’ in Question 1 means prior to receiving handover from the referring facility. ‘During retrieval’ in Question 2 means the duration between receiving patient handover from the referring facility to giving patient handover to the receiving facility (including any road transfers to and from the airstrip/airport in case of fixed wing retrieval). 2. For Questions 9 and 10, please document the time in hh:mm format. 3. For Question 11, please document the total duration of the return retrieval leg(s) from "Arrive at patient” time to "Arrive receiving facility” time in minutes (for example 099 minutes). This would include any road legs that precede or follow an air leg. 4. For Question 15, Psychosis not otherwise specified means any psychotic symptoms in a patient without a diagnosis of schizophrenia; Drug intoxication means under the influence of any psychostimulant drug. 6. For questions relating to potential adverse effects of ketamine ie. Questions 19, 21, 22 and 24 if these findings were present BEFORE ketamine had been administered please indicate so and the time that they were first noted. 8.
