There is a commitment to an ambitious and inclusive vision to improve health and well-being for the population from all local stakeholders across health and social care, and that vision has been awarded Pioneer status by NHS England. The development of an Integrated Care Organisation, (ICO) through the acquisition of Torbay and Southern Devon Health and Care NHS Trust by South Devon Healthcare NHS Foundation Trust has now taken place and is one of the major enablers in the transformation of services, and developing a new approach to health and social care. The ICO allows us to radically re-think and re-shape the way we deliver care, free from traditional organisational constraints. We will develop and redesign services that meet the needs of our local population and that are coordinated and integrated in ways that benefit service users. We will develop services that are responsive and flexible and which link, without break, with services of our partner organisations such as primary care and mental health services. Opportunities to be part of designing and delivering innovative new services in Torbay and South Devon, including more integrated and streamlined care pathways for patients is available to all current and newly appointed staff. Torbay and South Devon has a significantly older and ageing population and this demographic of over 65s, which is ahead of some other parts of the country by as much as 20 years, means that healthcare need is expected to increase by 15 to 20 per cent over the next decade. This increased demand will have to be met within existing resources which means we must seek a sustainable and innovative solution to change the way we provide care. A major feature of the services we develop will be the promotion of self-care. This will cover the promotion of healthy lifestyles and include support and training of service users and carers in selfmanagement of even the most complex chronic health conditions. Many of the services will be about supporting people in their lives rather than focusing on ill-health. We will base service development on the life-course model to cover all phases of people’s lives, with a particular focus on ‘family-centred care’. This is a substantive replacement post, funded by Torbay and South Devon Healthcare NHS Foundation Trust. The post-holder will principally be expected to provide Oncology services for Breast and Lower GI cancers as well as to support the Torbay Acute Oncology Service. The successful applicant should be committed to working in a smaller department where they may, in the future take on managerial and teaching responsibilities. The post holder would be expected to represent the department, and to integrate as fully as possible into the hospital providing a visible and consistent presence. JOB DESCRIPTION FOR THE POST OF CONSULTANT IN CLINICAL ONCOLOGY (10PAS) Divisional Deputy Medical Director Dr Joanne Watson Clinical Director Dr Deborah Turner Lead Cancer Nurse Sam Brenton Cancer Services Quality Manager Emma Wheatfill THE DEPARTMENT Torbay and South Devon NHS Foundation Trust is one of 5 hospitals (along with Royal Devon & Exeter NHS FT, Plymouth Hospitals NHS Trust, Royal Cornwall Hospital NHS Trust and North Devon District Hospital) within Devon and Cornwall providing non-surgical cancer therapy. Radiotherapy is available in 4 of these hospitals (Exeter, Torbay, Plymouth and Truro), chemotherapy is currently administered in all 5 hospitals in the network. The recent NHS England document, ‘Modernising Radiotherapy’ has been read with interest, and we are making plans to work collaboratively with our neighbouring radiotherapy providers. We envisage closer working for peer review of tumour volumes and plans, continuing to treat our patients in our centre. There are two designated MDT rooms with video conferencing facilities, and these are used for networked MDTs . The Oncology department at Torbay comprises both radiotherapy and chemotherapy facilities, providing non-surgical oncology services to a population of 375,000 and because of the population demographics there is a relatively high incidence of cancer. Approximately 1100 new patients are seen per annum in the Torbay Oncology Unit. Patients travel to Exeter for brachytherapy or treatment with unsealed sources. Services for testicular tumours are centralized in Exeter and Plymouth. Outpatient chemotherapy takes place in a dedicated unit shared with the haematologists (The Ricky Grant Unit). Cannulation and cytotoxic chemotherapy are carried out by specialist nurses, who also provide a PICC line service. There are dedicated nurse-led services including oral chemotherapy clinic, telephone triage service (24hrs) and emergency patient review, and image-guided paracentesis service. We are currently recruiting for a new CNS post for metastatic breast cancer patients, who will work closely with our breast cancer oncologists. There is broad experience with chemo-radiation for the full range of tumour sites, including colorectal and anal cancer. Chemotherapy is prescribed using the Chemocare system hosted for the east of the peninsula by Torbay. Acute Oncology is currently provided by a consultant holding the Oncologist of the Day phone, supported by two Acute Oncology nurses The radiotherapy department has a full range of planning and external beam equipment including: Elekta Versa HD Energy Linear Accelerator. Elekta Dual Energy Linear Accelerator with 6 – 18 MeV electron facilities, multileaf collimator and SYNERGY Image Guided Radiotherapy System. This will be replaced by May 2017 with a matched Elekta Versa HD Linear Accelerator. CT scanner shared with radiology but largely for Oncology use 3D treatment planning system (Pinnacle) Virtual Simulation Software (Prosoma) IMRT and VMAT is routine for radical pelvic radiotherapy. We have particular expertise in IGRT, with daily CBCT embedded in routine treatments. We are participating in the Fast-Forward clinical trial, using IMRT as required. The radiotherapy department has a Quality Assurance system achieving accreditation under ISO 9002 We have a dedicated Oncology out-patient department, a research oncology clinic, and access to medical outpatient clinic suites. Within the oncology department there are several patient interviewing rooms used as required by members of the multi-disciplinary teams to see patients on treatment. In-patients are accommodated on Turner ward, which is shared with haematology and medical oncology. STAFF The senior management of the Directorate comprises a Clinical Director (Deborah Turner) and Divisional Manager, supported by a Cancer Services Quality Manager (Emma Wheatfill). Day to day staffing management of the Department is the responsibility of the Oncology Service Manager and Practice Manager. The medical staff currently comprises 7 clinical oncologists, 3 medical oncologists and 1 specialty doctor. Junior cover is shared with haematology and is provided by a specialty registrar (Haematology) and 4 trainees from the medical rotation. Other Key Specialities: Site specialisation is practised. There are weekly local MDTs for all tumour sites, and regional for Head and Neck, Urology, Upper GI, Gynae and Anal. There are joint clinics in Head and Neck cancer, Urology and Gynaecology. Palliative Medicine There is a hospital based specialist palliative care service run by a consultant with specialist nurses, and fully integrated with Rowcroft Hospice There is a weekly palliative care clinic held in the oncology department. Radiography and Physics The Oncology Centre (Clinics and Radiotherapy) and Medical Physics are managed by the Clinical Oncology Services and Medical Physics Manager, Mrs Linda Gordon who heads a staff of radiographers, support staff (clinics) and A&C team. Pre-treatment activities including CT scanning/virtual simulation and dosimetry is undertaken by the radiographers, the latter overseen by the Principal Radiotherapy Physicist. Radiographers are encouraged to develop their roles and currently are trained to perform breast mark up, treatment review and IV contrast for the pretreatment service. There is a radiographer establishment of 21.70WTE. There are 2 research radiographers who both have approximately half their job devoted to clinical trials. One role is as Radiotherapy Technical Lead and Research Radiographer for radiotherapy trials. The unit also has a full time Macmillan support radiographer. The Medical Physics Department consists of 5.48WTE physicists (includes RPA and RWA), 2.8 WTE Medical physics engineers and 1.0WTE senior dosimetrist Specialist Nurses The Lead Cancer Nurse/manager and Matron for non-surgical Cancer Services is Miss Sam Brenton. There is a comprehensive Cancer support team (CST) that compromises of Clinical nurse specialist and Allied healthcare professionals, the team provide cover for all cancer sites. The CST is actively involved in all the MDTs and peer review. Social Support & Information The Lodge Cancer Support and information centre is an established resource within cancer services; the service provides information and support to all Torbay and South Devon patients and their families living with and beyond a cancer diagnosis. The lodge is located at the Annexe site, just outside the main hospital perimeter. Emotional & Psychological support There is an established team of 3 Clinical Psychologists and a counsellor who integral to the emotional and psychological well are being service provided. The team lead on “Moving On” programmes for patients who have completed their treatment and a carers support group. Living with and Beyond Cancer As part of the five year strategy the team across all cancer sites are actively involved in implementing key elements of the living with and beyond programme, this includes Holistic needs assessments, End of treatment clinics, treatment reviews (community), implementing Stratified pathways, and Health and wellbeing events. Clinical Research Torbay prides itself on its involvement in clinical trials and recruitment rates to NCRN sponsored clinical trials. Commercial studies also take place. The Research & Development Manager is Dr Fiona Roberts. There is a team of research nurses, radiographer and administrative staff. Diagnostic Support A full range of CT, MRI Ultrasound and Nuclear Medicine are available. PET scanning is available in Plymouth Pharmacy There is a full aseptic facility with dedicated chemotherapy pharmacist. The prescribing of chemotherapy is on the Chemocare computerised prescribing system. Radio Iodine Treatment All diagnostic scans and therapy with radioiodine for thyroid cancer are coordinated through the Medical Physics department at Exeter. Thyrotoxic patients also receive outpatient radio iodine in Exeter subject to satisfactory completion of a checklist by the referring endocrinologist. DUTIES/SPECIFICS OF THE POST Patient safety, dignity and care are the highest priority and at the centre of all that we do. To provide high quality consultant delivered care to patients including clinical leadership and supervision of junior medical staff, ensuring the highest standards of clinical care are provided to all patients attending the hospital. To be proactive in ensuring that all patients receive rapid assessment, treatment and clinical decision making, whilst ensuring our standard to deliver safe quality and performance are delivered and sustained. To provide teaching and training at undergraduate and postgraduate level and supervised and train the juniors and specialty doctors. To be proactive the in the department’s clinical governance agenda and take on leadership roles to support continuous learning and improvement, thereby abiding by the clinical and corporate governance policies of the Trust. To liaise with clinicians and managers within and outside the department to address any issues arising that may affect the services. To participate in relevant audit and quality assurance processes. To communicate with local GPs. Other responsibilities include: Junior Staff: Recruitment, appointment and educational supervision including educational contracts, regular appraisal, organisation of study leave, annual leave and rotas. Budget: Co-operate with the Clinical Director for Cancer Services and Divisional General Manager on budget issues. Communication: This will include the responsibility for and arrangement of communication and liaison with all other medical and service departments within the Trust as well as other Trusts. Teaching: To have an active role in the junior doctor training programme. This is a replacement post to provide Oncology services for Breast and Lower GI cancers as well as to support the Torbay Acute Oncology Service. It is expected that the job plan would be reviewed to ensure that there is an equitable distribution of work amongst the consultants. This post is fully funded by Torbay and South Devon NHS Foundation Trust and it is anticipated that all 10 PAs will be spent in Torbay. It is expected that the appointee will eventually take a role in the management of the Torbay department. As a result, experience of management and Clinical Governance would be an advantage. This is a 10 PA post but any candidate who is unable for personal reasons to work full-time will be eligible to be considered for the post. Please note that the agreed level of duties in relation to supporting professional activities, additional responsibilities and other duties will be reviewed after 6 months in post and there is an expectation that your SPA will decrease to 1.5 PAs with an increase to 8.5 PAs for direct clinical care. Part-time consultants need to devote proportionately more of their time to Supporting Professional Activities. This should be agreed on an individual basis. ON-CALL COMMITMENTS The present job plan does not include an on call commitment. This may be subject to change depending on job planning negotiations across the entire department in Exeter and Torbay. If the postholder is asked to take part in the on call rota this will either attract extra payment or the time will be taken off the current job plan pro rata. Consultants are expected to comply with GMC/GDC guidance and all Clinical Risk Management processes. Consultants must audit their own clinical outcomes, report clinical incidents, alert the Trust to risk, participate in annual appraisal and work to foster good team relationships. The appointee will be given a mentor upon commencement. It is expected that the appointee will plan a programme of personal development and continuing medical education (CME) and through the appraisal process ensure that adequate training is undertaken both to maintain their skills, knowledge and expertise and to ensure revalidation. There is extensive local support and the Trust provides reasons costs for formal study leave. An annual review of the job plan will take place between the Clinical Director and the Consultant in accordance with the arrangements in the new Consultant contract. This will include discussions regarding the needs of the service, with agreement and objectives for clinical standards, goals and workload. All Consultants have responsibilities for teaching doctors, student and non-medical healthcare professionals and should ensure they use the time allowed in their working week for duties of teaching and supervision. Consultants should also include some element of educational development within their personal development plan. Clinical Services at the Trust are delivered within five Service Delivery Units and each clinical directorate sits with these five Units: Community Services Medical Services Surgical Services Emergency Services Women’s Children, Diagnostics and Therapies Services INFECTION CONTROL In accordance with the Health Act (2006) all staff are expected to comply with National and Local Infection Control policies and procedures and any other related infection prevention policies or procedures. In addition, all staff whose normal duties are directly or indirectly concerned with patient care should ensure they have received annual mandatory training (including hand hygiene). It is expected that all staff take responsibility to ensure the workplace is kept clean and tidy so that it is sage for all other users of that area. All staff will be expected to communicate any identified infection risks to the Infection Prevention and Control Team in line with the Trust’s Incident Reporting Policy. Consultants will lead by example in this area which receives maximum attention. Salary: The Consultant salary scale is currently £76,761 rising to £103,490 per annum (pro rata where applicable). You can make your DBS check portable by registering with the DBS update service. If you then change role your employer can check your DBS status online and you won’t incur any future costs of DBS checks. Occupational Health: All appointments are conditional upon prior health clearance by Optima, the Trust’s Occupational Health Service. Annual, Study and Professional Leave: Annual leave entitlement is 32 days per annum (pro rata for part-time staff). Staff who have held a substantive NHS Consultant post for seven years will be entitled to an extra two days leave. Study leave entitlement is 30 days (pro rata for parttime staff) over a three year period commencing 1 April. A minimum of six weeks notice is required to take leave so that adequate cover arrangements can be made. Relocation Expenses: The appointee will be required to reside within a radius of 10 miles or 30 minutes of travelling time, unless specific approval is given by the Chief Executive and Medical Director. PERSON SPECIFICATION FOR CONSULTANT IN CLINICAL ONCOLOGY WITH AN INTEREST IN BREAST AND COLORECTAL ONCOLOGY REQUIREMENTS ESSENTIAL DESIRABLE Qualifications - Full registration with the GMC with licence to practice - Completion of higher specialist training in Clinical Oncology - Entry onto the Specialist Register (or entry expected within 6 months of interview date) - Fellowship of the Royal College of Radiologists - Able and experienced to run an IMRT service for Breast and Colorectal Cancer - Able to offer expert clinical opinion - Ability to take full and independent responsibility for clinical care of patients Good communication skills - Ability to organise a practice in an efficient manner with due attention to timeliness and dealing with administration and enquiries - Membership of Royal College of Physicians or equivalent qualifications Teaching/Research Experience - Desire to teach junior medical staff and professions allied to medicine and to participate in trust teaching programme. - Interested in teaching Medical School students - Experience of audit Personal Attributes - Desire to work in a smaller oncology centre - Ability to work in teams and maintain good working relationships across geographical and professional boundaries - Honesty and Reliability - Commitment to continuing medical education and professional development - Caring attitude to patients and relatives - Good leadership and organisational skills - Experience of research and ability to apply research outcomes to clinical practice - Evidence of teaching experience at undergraduate and post graduate levels and ability to teach clinical skills - Publication in peer reviewed journals - Attendance at Communication Skills Course Clinical Experience Management & Administrative Experience - Management experience - Interested in taking on a management role - Governance experience Respect & dignity We actively support peoples’ rights to have their voice heard in decisions about how their needs are met.
