Health Centre nurses and health officers
Role
o Establish rapport and therapeutic alliance
o Monitor response to treatment
o Adjust medication as needed
o Manage side effects of medication
o Detect and manage non-adherence
o Monitor suicide risk
o Psychoeducate patient and caregivers
o Address current psychosocial stressors
o Reactivate social networks
o Provide support to patient and caregivers
o Conduct a physical health review
o Screen and brief intervention for co-morbid
substance misuse
o Provide regular follow-up (minimum 3monthly)
o Detect those who drop out of care and
arrange outreach by HEW
o Know when to refer for specialist review

o Establish therapeutic rapport
o Psychoeducate and support patient and
caregivers
o Facilitate rehabilitation
o Support adherence and engagement with
care
o Detect relapse
o Detect side effects of medication

o Active suicide intent
o Inadequate food/fluid intake
o Persistently non-adherent to medication
o Acutely disturbed and not manageable at
home
o Pregnancy/breastfeeding
o Medical complications
o Unmanageable side effects
o 2 or more relapses in 12 months prior to
review (indicating poor control)
o Treatment-resistance
Referral criteria
o Active suicide intent
o Inadequate food / fluid intake
o Persistently non-adherent to medication
o Acutely disturbed and not manageable at
home
o Pregnancy / breastfeeding
o Medical complications
o Unmanageable side effects
