b. To ease the controlling of equipments in Statistic and Quality Engineering
Laboratory

Scope
Started fromthe loan of laboratory’s equipmentby users untilthe laboratory’s
equipmentcan be loaned by users. Users are people who borrow laboratory’s equipments.

Definition
The loan of Laboratory’s equipmentsis one of the available facilities in Statistic and
Quality Engineering Laboratory.

5.1 Terms
The applied requirement is thepresence of laboratory’s equipmentsloan
guarantee using user’s identity card(user’s KTM, KTP,or SIM). User’s identity
cardwill be detained as long asthe equipment is loaned by the userand will be
returnedafterthe return of laboratory’s equipment.Besides,Laboratory’s equipment
loanhas to use an official letterfrom the related authoritieswith the signature of the
responsible agency or activity addressed to the Head of Statistic and Quality
Engineering Laboratory which have received ACC from the Head of Statistic and
Quality Engineering Laboratory.
5.2 Conditions
The applied conditions are:
a.
Laboratory’s equipment is loanedaccording to those in the loan permission letter.
b.
Duration of laboratory’s equipment loanis according to those in the loan
permission letter.
c.
Assistants of Laboratoryserve the loan and return of Laboratory’s equipmentson
active days, starting at 09.00 – 15.00 WIB
d.
If the return exceeds the permitted loan duration without any confirmation to the
Laboratory’s assistant,the user will be fined. Fine as much asRp100.000,/equipment/day.
6.
SQE’s Equipments Loan Procedure
6.1 Procedure
a.
Start
b.
User writes an official equipment loan permission letter with the signature of the
chief executive or person in charge of the activity to the Head of Statistic and
Qual\ity Engineering Laboratory (SQE) TI-UB.
c.
Permission letter made by the user then submitted to the Assistant of Statistic
and Quality Engineering Laboratory (SQE) TI-UBto be coordinated whether the
requested equipment is used on the loan period.
d.
If the requested equipment is used by the lab, the loan permission letter will be
returned to the user in order to change the loan period or the loan will be
cancelled.If the requested equipment is not used by the lab,the loan permission
letter will be returned to the userin order to continue submission to the Head of
Statistic and Quality Engineering Laboratory (SQE) TI-UB.
e.
After submitting the loan permission letter to theHead of Statistic and Quality
Engineering Laboratory (SQE) TI-UB, if the Head of Statistic and Quality
Engineering Laboratory (SQE) TI-UB does not give ACC,back to step 1. If the
Head of Statistic and Quality Engineering Laboratory (SQE) TI-UB gives ACC,
continue to step 6.
f.
Submitting the loan permission letter which has been ACC by the Head of
Statistic and Quality Engineering Laboratory (SQE) TI-UB to the Assistant of
Statistic and Quality Engineering Laboratory (SQE) TI-UB.
g.
User leaves an ID card (valid KTM/SIM/KTP) and the contact of user and
person in charge of the activity.
h.
Assistant takesthe requested equipment.
i.
User and Assistant check the condition of the requested equipment.
j.
User receive the permitted equipmentin terms of the equipment return must in
the same condition as the one early loaned.
k.
Finish.

SQE’s Equipments Return Procedure
7.1 Procedure
a.
Start
b.
User has finished using the equipment.
c.
Userhands the equipment to the Statistic and Quality Engineering Laboratory
(SQE) TI-UB and sees the assistant of Statistic and Quality Engineering
Laboratory (SQE) TI-UB.
d.
Assistant of Statistic and Quality Engineering Laboratory (SQE) TI-UB and user
check the condition of the loaned equipment, if the condition is not the same as
the early one, user must substitute the equipment with the exact similar
specificationof the previous equipment and back to step 3. If the condition is the
same as the early one, continue to step 5.
e.
Assistant receivedthe loaned laboratory’s equipment.
f.
ID card left at the laboratory returned to the user by the assistant.
g.
Finish
