Service and has provided services to people with cognitive disabilities in King Co. since 1986. More

The SAJD is seeking a full-time case manager to work in our Supported Living Program located on
Capitol Hill in Seattle. The case manager will work with clients of various cognitive abilities throughout
King Co. to support them to live with independence and dignity. The case manager will act as an
advocate, liaison and resource coordinator for a case load of 18-22 clients with persistent mental illness,
traumatic brain injury and developmental disabilities. The case manager will be expected to complete
psychosocial assessments, develop care plans and be knowledgeable of government and community
resources.
Applicants should have a bachelor’s degree in social work, human services, behavioral sciences,
education or an allied field. Two years of case management experience working with adults who have
functional or cognitive disabilities is strongly recommended. Knowledge and familiarity with the Jewish
culture is helpful. In addition, the Case Manager must have an appropriate professional license or
certification, or must be able to obtain such licensure or certification within two years of hire. Reliable
transportation is required and must pass all applicable background checks.
Typical work hours are Monday-Friday, 9am-5pm with a rotating evening/weekend on-call shift once a
