FACULTY Evelyn Cook, R.N., C.I.C. Evelyn Cook is a Nurse Clinician and Nurse Liaison with the Duke Infection Control Outreach Network (DICON) for the Department of Medicine and PDC, PLLC at Duke University Medical Center. Currently, she is also a part‐time SPICE consultant and participates in training, education, and consultation to hospitals, long‐term care facilities, and other medical facilities to prevent and control healthcare‐associated infections. She has assisted long‐term care facilities in writing and developing infection prevention risk assessments, infection prevention plans and policies specific to the long‐term care setting. Ms. Cook, who is also a Registered Nurse and has National Board Certification in Infection Control, is active in several professional organizations including North Carolina Association for Professionals in Infection Control (President 2005‐06), National Association for Professionals in Infection Control and Epidemiology. She was named Practitioner of the Year in 2002 by the North Carolina Association of Professionals in Infection Control. Lauren DiBiase, M.S. Lauren DiBiase is the Healthcare‐Associated Infection Surveillance Program Manager and Public Health Epidemiologist for the University of North Carolina Health Care System. She received her M.S. in Epidemiology from the Harvard School of Public Health and her B.S. in Biology from the University of North Carolina at Chapel Hill. Previously, she worked as a Research Associate at the University of North Carolina Center for Public Health Preparedness and as a City Research Scientist at the New York City Department of Health and Mental Hygiene. Maria Gergen, M.T., A.S.C.P. Maria Gergen is the medical technologist for the Department of Hospital Epidemiology at UNC Health Care System. She received her B.S. degree from Wilkes College in Wilkes Barre, PA and did her internship at the Robert Packer Hospital in Sayre, PA. Her special areas of interest are bacteriology and research pertaining to sterilization and disinfection. Kirk Huslage, R.N., B.S.N., M.S.P.H., C.I.C. Kirk Huslage is the Associate Director of SPICE, serving as a principal lecturer at all SPICE infection control courses; consultant to all healthcare facilities (including nursing homes) in the state on infection prevention issues; conducts applied research projects in the area of infection control or epidemiology of infectious diseases at the State level. Kirk has been the Infection Control and Safety Compliance Officer for UNC Health Care Ambulatory Care, Home Health and Hospice, and Triangle Physician Network since 2008. Before that, he worked for seven years as a registered nurse in a variety of care environments (Burn Center, Bone Marrow Transplant, and Medical/Surgical). He has teaching experience in infection control and environmental health, and is conducting research in the field of infection control, having several publications as lead or contributing author. Kirk has served as a SPICE consultant in recent months answering infection control inquiries from healthcare facilities (including nursing homes) around the state. Kirk has a BS in Zoology from North Carolina State University, a BSN from Johns Hopkins University, and a MS in Public Health from the University of North Carolina School of Public Health. He is completing his Occupational Health Nursing Certificate at UNC. Karen K. Hoffmann, R.N., M.S., C.I.C., FSHEA Karen K. Hoffmann is the Infection Prevention Specialist with the Centers for Medicaid and Medicare Services (CMS). Her responsibilities include education, and policy development for the Survey and Certification Group at CMS where she serves as a liaison to the CDC. Previously, she was the Associate Director of the Statewide Program for Infection Control and Epidemiology (SPICE) for 23 years. First working in the critical care units, she began her career in infection control at Detroit Medical Center in 1981. She received her B.S.N. from Indiana University and her M.S. in Healthcare Epidemiology from the University of Virginia. Karen is a Clinical Instructor in the Division of Infectious Diseases at the University of North Carolina School of Medicine. She is Fellow in the Society of Healthcare Epidemiology of America (SHEA) and is certified in Infection Control. Donna Rutala, B.S.N., J.D. Donna Rutala is a litigation attorney with the firm of Young Moore and Henderson, P.A. in Raleigh, North Carolina. She received her B.S. in nursing from the University of North Carolina at Chapel Hill and her J.D. from North Carolina Central University School of Law. She has a unique combination of experience, having previously worked as an infection control professional and as a claims manager at the UNC Health Care System. William A. Rutala, M.S., M.P.H., Ph.D. William A. Rutala is a Professor in the Division of Infectious Diseases at the University of North Carolina School of Medicine; the Director of the North Carolina Statewide Program for Infection Control and Epidemiology; and Director of Epidemiology, Occupational Health and Safety Program at the UNC Health Care System. He received his B.S. from Rutgers University, M.S. from the University of Tennessee, and Ph.D. in Microbiology and M.P.H. in Public Health from UNC‐CH. He is a member of several national, state, local, and hospital committees and is an editorial board member for the American Journal of Infection Control and Infection Control and Hospital Epidemiology. He has authored many publications relating to disinfection, sterilization, infectious waste disposal and the control of nosocomial infections. Emily Sickbert‐Bennett, M.S., Ph.D., C.I.C. Emily Sickbert‐Bennett is the Associate Director for Hospital Epidemiology at the University of North Carolina Health Care System and a Research Assistant Professor in the Division of Infectious Diseases at the University of North Carolina, School of Medicine. She received her Ph.D. in Infectious Disease Epidemiology and M.S. in Environmental Microbiology from the Gillings School of Global Public Health at the University of North Carolina at Chapel Hill. Previously, she has also served as the Director of the Healthcare‐associated Infection Surveillance Program and Public Health Epidemiologist for UNC Health Care System as well as a Disease Investigation Specialist on a regional public health surveillance team. David van Duin, M.D., Ph.D. Dr. van Duin is a Visiting Associate Professor of Medicine, serving as Director of the Immunocompromised Host Infectious Diseases Section. He received his medical degree from the University of Amsterdam, The Netherlands, and completed an Infectious Diseases Fellowship and Ph.D. in Investigative Medicine at Yale University. He has board certifications in internal medicine and infectious diseases. Dr. van Duin's research interests include the study of multi‐drug resistant gram‐negative bacterial infections, and infections in iatrogenic immunocompromised patients, such as solid organ transplant recipients, and hematopoietic stem cell transplant recipients. David J. Weber, M.D., M.P.H. David J. Weber has been on the faculty of the University of North Carolina at Chapel Hill since 1985 where he is currently a Professor of Medicine and Pediatrics in the School of Medicine, and a Professor of Epidemiology in the School of Public Health. Dr. Weber serves as the Associate Chief Medical Officer for UNC Hospitals. He serves as the Medical Director of the Statewide Program for Infection Control and Epidemiology; and Medical Director of Epidemiology and also of the Departments of Hospital Epidemiology (Infection Control) and Occupational Health. He is Board Certified in Internal Medicine, Infectious Disease, Critical Care Medicine, and Preventive Medicine. He serves as the Associate Editor of Infection Control and Hospital Epidemiology. His research interests include healthcare‐associated infections, new and emerging pathogens (norovirus, SARS‐coV, CA‐MRSA, pfiesteria), infections in child care centers, and antibiotic resistant pathogens.
