Begin to recruit "subscriber support" from community partners within the Health
Priorities Consortium to continue annual updates to the Community Health Status
Package as a source of outcome measurement.

Youth Development.
Develop a Community Health Report Card for each county and the region, based on the
Community Health Status data, and work with the community partners to identify
shared priorities for targeting community and private intervention effort.
Design and produce a geographically based package of all Healthy People 2000 &
Healthier NY 2006 related indicators that can be tabulated from WNY record-level
vital statistic and hospital discharge files. All community partners will receive tabular
and diskette versions of the package.
Conduct a series of workshops with the community partners to achieve the adoption of
standards for health risk assessment by evaluating existing instruments developed by the
WHO, CDC, HEDIS 3.0, etc. for pubic health and managed care use. The package will
also include morbidity and health risk behavior estimates based on a small pilot survey
using the health risk screening instrument adopted by the group.
Recruit community partner representation from the major HMO's, rural health
networks, interested hospital and community wellness programs, Medicaid Managed
Care Coalition, the voluntary health agencies and others.
