Dear Colleagues & IVPNeers,
Building on the success of last year’s inaugural event, it gives me great pleasure to welcome you to the Second Intravenous and
Parenteral Nutrition (IVPN) Symposium, featuring a full-day Antimicrobial Stewardship Symposium, to be held on November
18-19, 2016 in Abu Dhabi, UAE.
Organized by the Cleveland Clinic Abu Dhabi Pharmacy team, this one-of-a-kind Symposium is designed for pharmacists,
physicians, nurses, clinical dietitians and other healthcare providers looking to enhance their knowledge of intravenous and
parenteral nutritional therapies and antimicrobial stewardship. The first day of the Symposium will cover safety considerations,
strategies for compliance with current and future compounding standards, technologies and clinical advances in IV and
parenteral nutrition therapy while the second day will feature a comprehensive hands-on workshop on antimicrobial
stewardship. The sessions provide up-to-date information and state-of-the art presentations delivered by reputable international
and regional experts.
In addition to an exciting scientific program, attendees will experience how Cleveland Clinic Abu Dhabi has established a
world-class pharmacy & Sterile Preparation Centre as well as an advanced Antimicrobial Stewardship Program. Attendees will
leave the Symposium with best practices from the experts that can be immediately implemented at their institutions,
irrespective of the level of resources at their disposal.

General Information
Registration and Badge Scanning
Registration will be open on November 18 from 7:30am – 5:00pm and on November 19 from 8:00am – 5:00pm.
Badges will be issued upon registration and must be worn at all times during the Symposium. Badges will
be scanned at the entry point to the Symposium sessions and anyone without a badge will be refused
entry to the session hall.
Food and Beverage
Complimentary tea, coffee and light refreshments will be provided in the morning and afternoon coffee breaks at the foyer
of Al Maryah Ballroom. Lunch will be provided during the designated lunch periods at Liwa Ballroom.
Parking
Ample parking for complimentary use is available at Galleria, the mall next to Four Seasons Abu Dhabi.
Valet Parking
Valet parking is available free of charge.

ACPE CE Accreditation
Cleveland Clinic Abu Dhabi is accredited by the Accreditation Council for Pharmacy Education to provide continuing education
for pharmacists and pharmacy technicians.
Cleveland Clinic Abu Dhabi is accredited by the Accreditation Council for Pharmacy Education as a provider of continuing
pharmacy education and complies with the Accreditation Standards for continuing education activities.
The Symposium will offer Accreditation Council for Pharmacy Education (ACPE)-approved knowledge-based and
application-based continuing pharmacy education credit hours. The maximum number of contact hours (CEUs) of
pharmacy continuing education credit that will be awarded for a pharmacist attending all the accredited activities at the
Second Intravenous and Parenteral Nutrition (IVPN) & Antimicrobial Stewardship Symposium is 14.0 hours.
The Symposium activities will be released November 18th, 2016 and expire November 19th, 2017. There is a registration
fee to attend this Symposium. Financial support was obtained for this Symposium.
Participants will be provided with an evaluation tool to evaluate this activity and will receive a Statement of Credit once the
evaluation is completed and submitted. This information will be automatically reported to CPE Monitor for US pharmacists.
Evaluations must be completed by January 17, 2017. Failure to complete evaluations by this date will result in a loss
of continuing pharmacy education credit.

POLICY ON CONTINUING EDUCATION CREDIT
Participants are provided instructions to access the electronic evaluation forms in order to evaluate the successfully completed
activity and generate an online Statement of Credit.
ACPE/NAPB will no longer accept hard copies of statements of credit. All information must now be submitted electronically by the
ACPE accredited provider. This information will be automatically reported to CPE Monitor; therefore, it is imperative that
participants complete their evaluations for the Second Intravenous Parenteral Nutrition (IVPN) & Antimicrobial Stewardship
Symposium by January 17, 2017. Failure to do so will result in a loss of Continuing Pharmacy Education Credit.
Participants are encouraged to carefully check their NABP e-Profile number and date of birth (MMDD) to ensure this data is
accurately recorded. Entering incorrect data will result in a rejection of CPE credit.

Ann Williamson, PhD, RN, NEA-BC, joined Cleveland Clinic Abu Dhabi in 2014 as the Chief Clinical & Nursing Officer.
In her current role, Ann oversees activities of all of Cleveland Clinic Abu Dhabi’s Nurses and Allied Health Professionals to
provide patients with world-class care.
Previously, she was the Associate Vice President for Nursing & Chief Nursing Officer at the University of Iowa Hospitals and
Clinics, an academic medical center, where she oversaw nursing services and patient care. Ann also served as the Associate
Dean for Practice at the University of Iowa College of Nursing.
Ann’s previous experience also includes serving as Patient Care Director at the University of California San Francisco (UCSF)
Medical Center and an Associate Clinical Professor at the UCSF School of Nursing. She held a number of leadership positions
at UCSF Medical Center between 1992 and 2008, including serving as Interim Chief Nurse, Director of Nursing Education and
Research, Manager of the Emergency Department, and Joint Commission Coordinator. In 2007, Ann received the Chancellor's
Award from UCSF for exceptional university management.
Prior to joining UCSF, she served in various capacities in nursing emergency services, including Director, Clinical Nurse
Specialist & Staff Nurse at hospitals in California, North Carolina and Texas, US. She also held positions in nursing education
in North Carolina, including instructor and lecturer.
Ann earned a Bachelor's Degree in nursing from the University of North Carolina at Chapel Hill, a master's degree in nursing
from the University of Texas at Austin, and a doctorate from UCSF, with a concentration in nursing informatics and health
systems.

Osama has joined CCAD in November 2010. Before joining Abu Dhabi, Osama was the Director of Pharmacy Services
Administration at King Fahad Medical City (KFMC), a 1000-bed hospital, for 4 years. Osama also served as the Director of
Pharmacies of Al-Rahba and Mafraq hospitals, Abu Dhabi, UAE or 3 years.
Osama also served at King Faisal Specialist Hospital (KFSHRC), Riyadh as a Clinical Pharmacist and Supervisor of the TPN
Team and IV Room for 11 years. Before joining KFSHRC, Osama worked for 9 years at Al-Hada Military Hospital as Outpatient
Pharmacy Supervisor and Drug Information Pharmacist.
Osama Tabbara graduated as Pharmacist in 1984; he has an executive MBA and is Board Certified in Nutrition Support.
He was a guest speaker in more than 100 symposia & seminars in topics related to Nutrition, IV Fluid Management, Pharmacy
Technoclogies, and Quality Management. He had given more than 1000 presentations and has many publications and
Performance Improvement Projects in the field of TPN, Fluid & Electtrolytes Therapy, and Quality Management.

Eric S. Kastango, M.B.A., B.S.Pharm., FASHP, is president of Clinical IQ LLC, a health care consulting firm and CriticalPoint,
LLC, a web-based education company.
Mr. Kastango received his Bachelor of Science degree in pharmacy from the
Massachusetts College of Pharmacy and Allied Health Sciences and his Master of Business Administration degree from the
University of Phoenix. He completed 65 hours of training in nuclear pharmacy at Purdue University and 80 hours of didactic
training for the Six Sigma-Green Belt certification that he started with BD Medical Systems.
Eric received a coveted ISMP Cheers Award in 2015 for his work related to sterile compounding safety. He is one of four
recipients of the 2013 Outstanding Service Award from the Massachusetts Society of Health-System Pharmacists and the 2014
recipient of the NABP Henry Cade Memorial Award that recognized his efforts and assistance to the states and NABP to address
the compounding tragedy that occurred in 2012.
Since 1980, he has practiced pharmacy in a number of practice settings, including hospitals, community, and home care, in a
number of different of roles, including the Corporate Vice President of Pharmacy Services for Coram Healthcare Corporation. He
has also managed a FDA-registered cGMP manufacturing operation for Baxter Healthcare Corporation.
He is an active member and Fellow of the American Society of Healthcare Pharmacists and served on the USP Sterile
Compounding Committee from 2005-2010 and 2010-2015 USP Council of Experts, Compounding Expert Committee until
April 2013. He has served on the USP Hazardous Drug Expert Panel since 2010. He is actively working with NABP and state
boards of pharmacy to provide training to their sterile compounding inspectors.
Eric served on the Expert Panel for ASHP Research & Education Foundation in the development of the 2015 Outsourcing Sterile
Products Preparation Vendor Assessment Tool and ASHP’s Insourcing Readiness Assessment Tool. He is also the author of the
2004 ASHP Discussion Guide on Sterile Preparation: Summary and Implementation of USP Chapter 797, the ASHP Sterile
Product Preparation CD-ROM: A Multimedia Learning Tool, the ASHP webbased 797 Compliance Advisor Gap Analysis Tool for
USP Chapter 797. He assisted in the development of the CriticalPoint web-based educational series on Sterile Compounding
and the Annual National USP <797> Compliance Survey now in its fifth year. Eric has over 200 invited national and
international professional presentations on various pharmacy practice topics such as pharmacy compounding, patient safety
and quality systems.

Ramona Davis, Pharm.D., R.Ph. is Director, Medication Safety Services at the Cleveland Clinic (CC) in Cleveland, Ohio.
CC is a staff physician practice model employing over 1,800 staff physicians and consists of a 1,350 bed tertiary care referral
hospital, Family Health Centers and Ambulatory Surgery Centers. Ramona has responsibility for identifying and defining
priorities for quality improvement activities related to medication safety. This includes structuring and coordinating interventions
to correct procedural deficiencies and to enhance or implement safety systems. Ramona is also responsible for the review,
collation and analysis of all medication-related events. Ramona received a BA in chemistry from Fisk University in Nashville,
Tennessee. She received her Pharm.D. from Hampton University in Hampton, Virginia. Ramona completed the American
Hospital Association-National Patient Safety Foundation Patient Safety Leadership Fellowship. Prior to her current position,
Ramona served as Coordinator, Medication Regulatory and Accreditation Services, Quality Assurance and Accreditation
Pharmacist and a Clinical Staff Pharmacist all at CC. Ramona is a member of the American Society of Health Systems
Pharmacists. She is a member of the American Society of Medication Safety Officers. She holds a Clinical Assistant Professor
of Pharmacy Practice title at the Northeastern Ohio Universities Colleges of Medicine and Pharmacy.

Michael J. Rybak, Pharm.D., MPH, FCCP, FIDSA, is a Professor of Pharmacy at the Eugene Applebaum College of Pharmacy
and Health Sciences (EACPHS). Dr. Rybak is also an Adjunct Professor of Medicine in the Division of Infectious Diseases at the
School of Medicine at Wayne State University in Detroit, Michigan. He is the Director of the Anti-Infective Research Laboratory
at the EACPHS and is affiliated with the Detroit Medical Center. Dr. Rybak received his B.S. in Pharmacy from Northeastern
University in Boston, Massachusetts, his doctorate in pharmacy degree from the College of Pharmacy & Health Sciences and
his Master’s of Public Health degree from the School of Medicine at Wayne State University. Dr. Rybak’s primary focus of
research is antimicrobial pharmacokinetics and pharmacodynamics (PKPD) and their relationship to
bacterial resistance. His most recent work is in the areas of combination therapy for methicillin and vancomycin-resistant
Staphylococcus aureus and enterococci. Dr. Rybak has published more than 300 manuscripts in medical and scientific journals
and authored several book chapters on antimicrobial PKPD, resistance, assessment of patient outcomes and
antimicrobial stewardship. He is the recipient of the ASHP and ACCP awards for sustained literature contributions and
most recently the ACCP Frontier’s Lecture Award.

Kenneth Klinker, Pharm.D. is a graduate of the University of Maryland, he completed his residency training at UF Health Shands
Hospital in internal medicine. Following completion of his residency training, he spent five years providing care to critically ill
patients in the Medical Intensive Care Unit at UF Health Shands and in 2003 transitioned to Infectious Diseases in order to
participate in the development of an Antimicrobial Management Program. Currently, he is a clinical associate professor at the
UF College of Pharmacy, and a pharmacy specialist in Infectious diseases at UF Health Shands Hospital.
He is a co-director of the Antimicrobial Management Program and program director for the Infectious Diseases Residency
Program. Dr Klinker is active in the Society of Infectious Diseases and has served on the Antimicrobial Stewardship Committee.
Additionally, he is a past president of the North Central Florida Society of Health Systems Pharmacists, or NCFSHP, and served
as a board of director for the Florida Society of Health Systems Pharmacists.

Ahmad Chaker, R.Ph., MBA, is the Sterile Preparation Specialist at Cleveland Clinic Abu Dhabi and has been with the
organization for over 2 years now. He manages the Sterile Preparation Center moreover he continues to work in the Inpatient
Pharmacy, OR and ICU satellite Pharmacies.
He joined the hospital as Senior Automation and Informatics Pharmacist. He established the latest Pharmacy-related
technologies like Pyxis, Pharmogistics, C2 Safe to name a few. He previously worked at Mafraq Hospital from 2006-2014 as
a Senior Pharmacist and assumed full responsibilities of Inpatient pharmacy Supervisor that includes Quality of service,
budgeting, inventory, business plan, staffing, and resource allocation and setting goals of objectives. He also has established
the Dialysis Pharmacy in Mafraq. Ahmad continuously seeks ways in enhancing his leadership and skills, fostering personal

Lamia Omer Yahya is a Neonatal Clinical Pharmacist at the Corniche Hospital, a 300-bed specialized maternity and neonatal
hospital in Abu Dhabi, UAE. She holds the Board of Pharmaceutical Specialties certification in nutrition support pharmacy and
the Canadian board of qualification in pharmacy.
Her first place of practice was Mafraq Hospital, where she practiced hospital pharmacy and clinical pharmacy for almost 10
years.
Lamia has a great passion towards enhancing the safety of the neonate patient while being treated with the different
pharmacotherapies in the neonatal intensive care unit. She has worked on several projects related to the standardization and
optimization of parenteral nutrition in this high risk patient population.
She is also involved, along with multidisciplinary teams, in a number of quality improvement projects aiming at enhancing the
safe prescribing, dispensing and administration of Neonatal medications. Lamia is the recipient of 2 awards at SEHA’s level and
numerous hospital level awards in recognition of her efforts in the clinical pharmacy field.

Dublin and Nutricia Clinical. She began her career in New Zealand where she worked at Nutricia Clinical NZ, Lea Stening

Bachelor of Consumer and Applied Sciences in Nutrition from the University of Otago.

Dustin Balzan is currently a Senior Pharmacist at MDH Pharmacy Quality Assurance Section, and a visiting assistant lecturer
at the University of Malta on Pharmaceutical Quality Assurance and Quality Control. From time to time he is also involved in
lecturing on quality to other university departments at post graduate level. Institutions requesting presentations on quality care
concepts from Mr. Balzan include the Malta College of Pharmacy Practice, the Malta Pharmacy Students Association and Mater
Dei Hospital. Dustin Balzan studied Pharmacy at the University of Malta, and graduated in 2005. In 2011, Mr. Balzan
obtained a Masters of Science Degree in Pharmaceutical Technology and Quality Assurance from the University of Leeds, UK
and is currently reading for a Pharm D collaborative programme set between the University of Malta and the University of
Illinois, Chicago. Language portfolio includes Maltese, English, Italian and French and is currently studying Arabic (Modern
Standard Arabic). Dustin Balzan has been practicing as a pharmacist for eleven years and has worked in both community and
hospital pharmacy. Mr. Balzan is an avid supporter of multicultural input and networking and regularly takes part in
International Medication Safety Conferences. Furthermore, Dustin is also actively involved in international (UK, US and Middle
East and North Africa) online fora regarding Pharmaceutical Quality Assurance, especially within Hospital environments.
During his free time, Dustin also invests time in a number of outdoor and fitness activities such as cycling, limited resource
camping and trekking, fishing, small boat navigation, photography and off-road driving. He is also interested in various quality
systems of different nature. The core belief is that quality is essentially a lifestyle and that one can never aim to attain quality

different solid organ transplant teams at KFSH&RC. He teaches advanced therapeutics and clinical immunology to the master
of clinical pharmacy and Pharm.D. students at the college of pharmacy, King Saud University, Riyadh, and Alqassim University,
Qassim, Saudi Arabia in which he holds an adjunct faculty position. He is a member of the advisory board of 4 different schools
of pharmacy in Saudi Arabia and many peer reviewed journals. He also holds several consultancy positions at the world Health
Organization (WHO), Saudi Food and Drug Authority (SFDA), Saudi Ministry of Health (MOH), and the Saudi Commission for

Outpatient Infusion Area. Zeina holds a Master’s Degree in Nursing, Adult Health Track from the American University of Beirut
(AUB), and completed her clinical residency at MD Anderson Cancer Center, Houston Texas. Following her degrees, Zeina
served AUB Medical Center for 7 years in so many areas like medical surgical, infectious, bone marrow and liver transplant,
hematology and oncology, where she transitioned from Staff Nurse to Nurse In charge, and was highly involved in the Magnet
Designation; AUBMC the first hospital to receive Magnet Designation in the Middle East. She worked at the AUB School of
Nursing as a Clinical Instructor as well. Zeina is certified by the California Board of Registered Nursing. She is a member in the
Oncology Nursing Society and the American Academy of Ambulatory Care Nursing. Zeina is currently the vice-president of
Oncology Nursing Society (ONS) Focus Group in UAE. Zeina is highly interested in Nursing Research. Her hobbies is traveling

Dr. El-Lababidi is the Pharmacy Education and Training Manager at Cleveland Clinic Abu Dhabi. Dr. El-Lababidi received her
Doctor of Pharmacy degree from the Lebanese American University in Byblos, Lebanon. Dr. El-Lababidi then completed a
pharmacy practice residency at St. Michael’s Medical Center in Newark, NJ, followed by an Infectious Diseases pharmacy
residency at Yale-New Haven Hospital in New Haven, CT.
Following residency training, Dr. El-Lababidi served as the Infectious Diseases clinical specialist at Florida Hospital Orlando.
She then assumed the role of Assistant Director for Clinical Services, during which she established the hospital’s antimicrobial
stewardship program. Dr. El-Lababidi since relocated to Abu Dhabi to join Cleveland Clinic Abu Dhabi as the Pharmacy
Education and Training Manager for the Department of Pharmacy Services. In her current role she overseas education and
training and established the hospital’s antimicrobial stewardship program and serves as the Co-Chair of the ASP at CCAD.
Dr. El-Lababidi is a Board Certified Pharmacotherapy Specialist with Added Qualifications in Infectious Diseases and is
credentialed by the American Academy of HIV Medicine. Her areas of interest include antimicrobial stewardship,
pharmacokinetics and pharmacodynamics of antimicrobials, and MRSA infections. Dr. El-Lababidi is an active member of the
American Society of Health-System Pharmacists, the American College of Clinical Pharmacy, and the Society of Infectious
Diseases Pharmacists. Articles by Dr. El-Lababidi been published in Pharmacotherapy, the Annals of Pharmacotherapy and the
American Journal of Health-System Pharmacists. Dr. El-Lababidi has spoken at both National and International conferences in
the area of Infectious Diseases and Pharmacy Practice.

Dr. med. Jens Thomsen MPH has joined the Health Authority – Abu Dhabi (HAAD) in 2007 and is currently holding the position
of Section Head, Environmental Health at the Public Health and Research Division of HAAD.
Dr. Thomsen is a Germany-trained physician and a German Medical Board certified Specialist in Clinical Microbiology and
Infectious Disease Epidemiology. He is also holding a Master of Public Health (MPH) degree from Rollins School of Public
Health, Emory University, Atlanta, Georgia, with majors in epidemiology and biostatistics.
Dr. Thomsen is German and U.S. board-certified in Healthcare Infection Control & Epidemiology and has 20+ years of medical
and public health experience from the US, Germany and the United Arab Emirates, in the area of clinical microbiology, hospital
infection control, and antimicrobial resistance surveillance, but also in the wider public health area, with a focus on
environmental health, occupational health and safety, injury surveillance and injury prevention, health promotion, and public
health policy.
Dr. Thomsen is leading both, the Abu Dhabi Emirate working group on antimicrobial resistance surveillance, as well as the UAE
National working group on antimicrobial resistance surveillance, and he is a member of the UAE Higher Committee for
Antimicrobial Resistance prevention and control.

Dr. Rayhan Hashmey is the Deputy Chief Medical Officer and Consultant Infectious Diseases at Tawam Hospital. He holds
Associate Professorship in Internal Medicine at the UAE University Faculty of Medicine and Health Sciences. A graduate of Dow
University of Health Sciences, Pakistan (1989), he completed his post-graduate medical training at Baylor College of Medicine
in Houston, and is Board Certified in Internal Medicine and Infectious Diseases from the American Board of Medical Specialties.
His areas of interest are infection control, HIV infection, antimicrobial stewardship and public policy matters regarding
antimicrobial resistance and stewardship. He is actively working with Health Authority of Abu Dhabi to implement greater
antimicrobial oversight in the Emirate.

Dr. Nusair obtained his medical degree from Jordan University of Science and Technology in 1997. He did his internship in
internal medicine and worked as a Trauma House Officer between 1998 and 2002. In 2005, Dr. Nusair completed training in
Internal Medicine at Western Reserve Care System. Upon completion of Fellowship in Infectious Diseases in the combined
program at Creighton University and the University of Nebraska Medical Center in 2007, Dr. Nusair joined the medical school
faculty at Marshall University in Huntington, WV. Dr. Nusair returned to Marshall University in 2011 after completing
fellowship training in Critical Care Medicine at Mayo Clinic in Rochester, MN. He was promoted to Associate Professor in 2011.
In addition to his academic and clinical responsibilities at Marshall University Dr. Nusair served as the Chief for the section of
Infectious Diseases at Marshal University. He also served as the medical director for Infection Control and Antimicrobial
Stewardship Program and the medical director for the Intensive Care Unit.
Dr. Nusair received many awards for excellence in clinical mentoring and practice. He is an active member of a number of
national and international medical societies. Dr. Nusair is recognized as a Fellow of prestigious medical societies including: The
American College of Physicians, The Infectious Diseases Society of America and The Society for Healthcare Epidemiology of
America.
Dr. Nusair has numerous academic contributions including peer reviewed publications, multiple national and international
presentations and lectures and research projects in infectious diseases and critical care medicine. His special interests include
device related infections and quality improvement and resource utilization in the critical care setting.

Unit), in addition to an active role in the anticoagulation clinic. He also served as an assistant professor at the College of

Dr. Bonilla completed Medical School at Universidad del Valle in her birth country, Colombia. She subsequently pursued her
Internal Medicine residency and Infectious Diseases fellowship at the Cleveland Clinic in Cleveland, Ohio, US.
Following her training, she worked in the Northeast Ohio, Akron area for three years where she built up a busy practice as an
Infectious Diseases Consultant at Pinnacle Infectious Diseases and as Clinical Assistant Professor of Internal Medicine at Ohio
University College of Osteopathic Medicine.
Dr. Bonilla moved to Abu Dhabi in 2014 where she resides with her husband and their three children.
