SPRINGFIELD, IL - The Illinois Department of Financial and Professional Regulation
(IDFPR) announced today that the Director of the Division of Professional Regulation, Daniel E.
Bluthardt, signed the following disciplinary actions taken during the month of May 2006.
BARBER, COSMETOLOGY, ESTHETICS AND NAIL TECHNOLOGY
Gina M. Heffron, Bartlett – cosmetologist license (011-198620) suspended for 30 days
followed by indefinite probation after submitting a false application stating she had successfully
completed continuing education requirements.
Tracey Jones, Chicago – unlicensed, ordered to cease and desist the unlicensed practice
of cosmetology and barbering.
Kenneth McCord, Rockford – barber license (006-061803) placed on indefinite
probation for a minimum of five years and cosmetologist license (011app219725) issued and
placed on indefinite suspension for a minimum of six months due to felony conviction.
Barron M. Walker, Chicago – barber license (066app20863) issued and placed on
indefinite probation for a minimum of two years.
DENTAL
Bruce B. Costagli, Schaumburg – dentist license (019-018837) placed on probation for
one year due to billing irregularities and failure to maintain appropriate treatment records.
Wook San Kim, Orland Park – dentist license (019-025563) reprimanded for failure to
adequately document services provided to a patient.

Vincent Madda, Chicago – dentist license (019-006572) ordered to cease and desist the
unlicensed practice of dentistry and fined $4,500 for practicing without a valid license.
DETECTIVE, ALARM, SECURITY AND LOCKSMITH
Day & Zimmermann, LLC, Oak Brook – security contractor agency license (122000925) reprimanded and fined $750 for operating the agency for approximately three weeks
prior to the issuance of the license.
Eric S. Beard, Aurora – permanent employee registration card (129-214751) placed on
probation for one year due to a criminal conviction.
Jeffrey Alan Lavery, Tinley Park – permanent employee registration card (129272986) placed in refuse to renew status due to felony conviction for three counts of mail fraud
as well as his current status of being on supervised release from incarceration.
Joshua John Moore, Manito – permanent employee registration card (129-284561)
placed on probation for three years due to a criminal conviction.
Paul J. Parziale, Athens – permanent employee registration card (129-292686) issued
and placed on probation for two years due to a criminal conviction history.

conditions due to unprofessional conduct by engaging in a romantic relationship with a patient.

controlled substance license (336-030044) temporarily suspended for immoral conduct of a
sexual nature, unprofessional conduct, administering controlled substance for non-medically
accepted therapeutic purpose, and failure to report action taken by a health care institution.

reprimanded due to a sister state discipline in Florida.

restored to indefinite probation.

suspended for a minimum of one year due to a sister state discipline in Indiana.
James Wagner II, Vandalia – physician and surgeon license (036-078649) reprimanded
due to gross negligence and unprofessional conduct for failure to order proper test to determine
presence of arterial clot and prescribed medication without keeping proper documentation.
Christopher Evans, Chicago – chiropractor license (038-009989) indefinitely
suspended followed by probation for three years due to immoral conduct of a sexual nature with
a female patient.
Mariel Gelinas, Sherbrook, Quebec, Canada – chiropractor license (038-010689)
issued with reprimand after being disciplined in Canada.
Mark Leonard May, Oak Brook – chiropractor license (038-008796) reprimanded for
practicing without a license after failing to renew in 2002.
Michael McLaughlin, Palatine – chiropractor license (038-004181) indefinitely
suspended for failure to pay the fine from a previous consent order.

professional(s), on several ALCON forms to order supplies and therapeutic medications from
ALCON.
Theresa McDonald, Joliet – unlicensed, ordered to cease and desist the unlicensed
practice of optometry and assessed a $150 civil penalty for unlicensed practice.
PHARMACY
Danielle Covey, Chicago – pharmacy technician license (049-031279) placed on
probation for one year after diverting Promethazine with Codeine, a Schedule V controlled
substance, and Viagra from her pharmacy employer.
Brenda Jaraczewski, Kankakee – pharmacy technician license (049-072926)
indefinitely suspended after diverting several doses of various Schedule II controlled substances
from her pharmacy employer.
Nanci-Rae Rohm, Worth – pharmacy technician license (049-085133) indefinitely
suspended for diverting Viocodin, a Schedule III controlled substance, from her pharmacy
employer.
Vaishali Pharmacy Inc., Darian and Kiran Anantray Joshi, Chicago – pharmacy
license (054-013957) reprimanded and pharmacist license (051-034647) reprimanded and fined
$750 for relocating the pharmacy prior to properly notifying the Division and having the
prerequisite inspection.
Gary Mantese, Austin, TX – pharmacist license (051-031441) indefinitely suspended
due to self-reporting a relapse and sister state disciplines in Texas, Missouri and Louisiana.
Susan Rhoades Saal, Pekin – pharmacist license (051-031132) reprimanded for
committing various dispensing errors during the period from 2001 through 2003.

refuse to renew status for being more than 30 days delinquent in the payment of child support.
ROOFING CONTRACTORS

status for operating on a non-renewed license.

SPEECH LANGUAGE PATHOLOGY/AUDIOLOGY
Shelbi D. Shumaker, Chenoa – speech-language pathologist license (146-007118)
indefinitely suspended for a minimum of one year and fined $1,000 for unprofessional conduct
and misrepresenting the timing of her education credentials and in applying for employment as a
speech-language pathologist with her former employer, Easter Seals, in August 2001.
VETERINARY MEDICINE
Cheryl Helsing, Decatur – veterinarian license (090-005558) and controlled substance
license (390-002157) reprimanded for negligently allowing a former employee to self-medicate,
while under the employ of the respondent.
Hunt Tainter, Owensville, MO – unlicensed, ordered to cease and desist from the
unlicensed practice as a veterinarian.
