We
are pleased to welcome you to Specialty Surgery of Ocala, an ambulatory surgery and
diagnostic facility. The physicians, healthcare professionals and staff are committed to providing
our patients with the highest quality health care in a comfortable environment.
Specialty Surgery of Ocala is a same-day surgical center; a medical facility where certain elective
surgical procedures are performed and patients return home the same day. Outpatient surgery is a
planned elective, non-emergency surgery, which your doctor has recommended and you have
chosen.
You are encouraged to carefully read this brochure, as it will answer many questions you may
have regarding your scheduled visit here at Specialty Surgery of Ocala.
Pre-Admission Visit / Phone Call
Depending on the type of surgical procedure you have scheduled, you may be required to have a
pre-admission visit (i.e. pre-op appointment) scheduled in order for us to obtain lab work, x-rays
and an electrocardiogram (EKG). If this is required, you will be given a specific pre-op
appointment usually within 1-2 weeks prior to your surgery. If a pre-op appointment is not
required, then a nurse from our facility will contact you by phone prior to your scheduled date of
surgery. You will be asked questions regarding your current list of medications and dosages, any
previous surgeries, medication allergies and your overall health status. This call may take 20 – 30
minutes. This is also an opportunity for us to explain what will take place during your visit with us.
Day of Surgery
You should arrive at Specialty Surgery of Ocala at the time you were given during your preadmission visit / phone call.
Upon arrival at Specialty Surgery of Ocala you will be escorted to the Pre-Operative area. Your
nurse will prepare you for surgery by asking you to sign a consent form authorizing the surgery,
starting an intravenous line and performing any additional tests required by your physician. Please
follow all pre-surgery instructions – otherwise your surgery may be delayed or cancelled.
Pre-Operative Instructions for Surgery Requiring Anesthesia
The following pre-operative instructions will help you understand your responsibilities prior to your
visit to help make your stay pleasant and safe:

Please arrange for a responsible adult to drive you home. The Specialty Surgery staff would
like the opportunity to discuss your post-operative instructions and care requirements with
family or a significant other.
DO NOT eat or drink anything after midnight the night before your surgery. (Take only
those medications that you were advised to take with small sips of water).
DO NOT chew gum, candy or tobacco the day of your surgery.
Please bathe or shower the night before or morning of your surgery (follow any additional
bathing instructions you may have been given). Wear loose, comfortable clothing. A short
sleeve, button down shirt, loose fitting shorts/pants and socks is preferred.

DO NOT bring jewelry or valuables with you on the day of surgery. Specialty Surgery of
Ocala cannot be responsible for them.
Please bring a case for your eyeglasses and hearing aides. These will be removed from you
just prior to surgery and returned to you in the recovery room.
A consent form must be signed prior to the administration of any medication or anesthesia.
A parent or guardian must sign the form for a minor child. If you have Durable Power of
Attorney and are required to sign the consent form, please bring a copy of this authorization
on the day of the pre-admission visit. If your pre-admission is done by phone, please bring a
copy of this authorization on the day of surgery.
Please advise your physician of any change in your physical condition prior to surgery such
as a cold, fever, or infection. It may be necessary to reschedule your surgery.
Get a good night’s rest before your surgery.

Anesthesia
The Anesthesia Department at Specialty Surgery of Ocala provides the highest level of care to our
patients. Your anesthesia will be administered by expert anesthesia personnel.
Discharge
Following your surgery and recovery, you will be given a list of discharge instructions to
complement any post-operative instructions given to you by your physician’s office. It is advisable
to have someone stay with you for the first 24 hours after surgery. The next day you will receive a
call from a member of our nursing staff to check on your recovery from your surgery.
Financial Information
Fees for medical services provided at Specialty Surgery of Ocala vary and are determined by the
complexity of the individual procedure scheduled. Facility fees include the use of the surgery
center, all surgical support staff services, and most supplies, implants and drugs normally used in
the specific procedures. Some additional supplies, implants, drugs, etc. may be billed after surgery
if they were required during the course of your surgery. You will receive separate bills from your
surgeon and anesthesiologist, radiologist, cardiologist and/or pathologist. As a licensed ambulatory
surgery and diagnostic center, we offer a less costly alternative to typical hospital-based care in a
convenient and comfortable environment. We are concerned about health care costs and welcome
your comments and suggestions. Our office has highly trained and qualified personnel available to
answer your insurance questions. Specialty Surgery of Ocala participates in Medicare.
You and Medicare
As a participating provider for Medicare, we accept the amount the Medicare allows for covered
services. Medicare will generally pay 80% of this Allowable Charge after you have met your
annual deductible. You are responsible for payment of your deductible and the remaining 20%
Medicare allowance. Most patients have supplemental policies. Many of these policies pay the

remaining 20% of the allowable charge. Please bring your Medicare card and your supplemental
insurance card so that our insurance specialist may file your claims accurately.
Private Insurance
Private insurance plans vary and may cover anywhere from zero to 100% of your charges. Our staff
will advise you of your coverage limitations as well as the charges for which you will be
responsible. Patient deductibles and co-payments are due at the time of service.
Some insurance carriers require pre-certification or second opinions prior to certain surgical
procedures. Our staff will be happy to assist you. However, you are responsible for making this
determination.
Self Pay
Patients without insurance coverage will be responsible for all charges incurred. At the time of
service, payment in full is expected and customary. Should you need financial assistance, we will
work with you so that charges will not be a burden.
Billing
We prefer not to send monthly statements due to the cost of paperwork and postage. Therefore, we
request that your portion of the payment due to Specialty Surgery of Ocala be paid at the time of
your visit. We accept Medicare and insurance assignments, and we offer several payment options
including MasterCard, Visa and personal checks. If you have any questions, please call 237-3387

Directions – Our facility is a white building with a green sign stating Specialty Surgery of Ocala.
From I-75 take Exit 350 (SR200/College Road) and head east. Turn right at SW 32nd (the light
between the Publix shopping center and the Paddock Mall). The facility is the third to the last
building on the left.
From the Villages– Take 475A heading north into Ocala. Turn left onto SE 34th Street (see Palm
Gardens Nursing home on the left). The facility is the third building on the right.
From Hwy 441 – Take SW 17th toward SR 200. Turn left onto SW 19 Ave Road (Easy Street).
Turn left at SW 27th Ave (McDonalds/Goodwill). Turn right at SE 34th Street (see Palm Gardens
Nursing home on the left). The facility is the third building on the right.

IMPORTANT BILLING INFORMATION
Services rendered in the Surgical Facility are not covered by your office
visit co-pay. Please take the time to discuss fees with
Ms. Kennedy after being scheduled or you may call (352) 237-3387 ask
for extension #204.
You will be billed by four (4) providers when using Specialty
Surgery of Ocala (surgery center)
1)
Physician – The Urology Health Team, PLLC
2) Facility - Specialty Surgery of Ocala (surgery center)
3) Pathology – Urology Health Team, PLLC, Lab Corp
or Quest Diagnostic (depending on your insurance plan)
4) Anesthesia – Michael Donohoe, CRNA (if applicable)
