»4-seasons orthodontics«
Orthodontic therapy
for patients of all ages
Dear Colleagues!
Since
1988 your
competent partner
for orthodontic
products.
Again, we invite you to attend the »14th International Orthodontic
Symposium« (IOS) in Prague, November 24 – 26, 2016. This time,
the motto will be »The Four Seasons In Orthodontics«. What does
that mean? We all know that patients of all ages seek orthodontic
treatment — it is long ago that only children and adolescents were
the typical patients in our field of medicine. Children, even newborn
children may be addressed as the first season of orthodontic patients. The second season is made up by our adolescent patients,
and in the third season, our adult patients are found. And according
to the success of general dentistry, even highly aged patients live
with their own teeth, and as teeth do not stand stable within bone,
they also require orthodontic treatment in several cases. This group
of patients is our fourth season in orthodontics.
As an exception, I as the president of the IOS-Symposium, was asked
by the scientific committee to give the pre-congress course of the
EAO-Academy and give the "Tiziano-Baccetti-Memorial-Lecture" on
the first day of the meeting. Based on my background in craniofacial
developmental biology and as an orthodontist in practise, I will do
my best to present my experience to address aspects of the right timing and interdisciplinary approaches for patients in all four seasons
in orthodontics.
We have invited outstanding speakers who will cover further aspects of all of these four seasons in orthodontics. And we invite you
as the participants, coming from more than 30 nations, to share this
international orthodontic congress with its familiar atmosphere,
where our young orthodontists come together with the more advanced experts to share the knowledge and to network for ongoing
international education.
I am looking forward to seeing you all again in Prague!

14th IOS-Symposium 2016
ong ago only children and adolescents were the typical patients
in orthodontics. Today, after general dentistry has become so
successful that patients can maintain their teeth lifelong, we as
orthodontists have to accompany them during their whole life. Even
highly aged patients live with their own teeth, and as teeth do not
stand stable within bone, they require orthodontic treatment in several cases. Just to catch them and move them back onto the alveolar
bone, if retention measures were not successful. Most of the major
fixed prosthetic reconstructions show a better result when the teeth
are lined up orthodontically before. So to which extent has orthodontics become the main agent in dentistry? Is it true, that well aligned
teeth in the adolescent guarantee a long-term maintenance of the
dentition during life? And finally, which of the early treatment in children is appropriate and recognised?
‣ This EAO-Academy Pre-Symposium Course will address "The Four
Seasons In Orthodontics":
Season 1: Children from 0–10 years of age
Season 2: Adolescents from 10–18
Season 3: Adults from 18–50
Season 4: The best agers, 50+.
We raise questions, review literature, and give an abundance of
treatment examples and encourage you to improve networking with
your local general dentists.

n modern orthodontics, the treatment times have significantly
shortened. A wise treatment planning, intelligent application of
most recent technology, safe anchorage and a good cooperation
of patients contribute to this success. Nevertheless, there is a desire
to get even faster results. Our knowledge of molecular signaling and
regulation of the biological basis of orthodontic tooth movement, in
particular remodelling of bone and other periodontal tissue has increased. So, there is much hope to achieve tooth movement even
faster.
‣ In this presentation we will present the state of the art and we will
discuss, in how much it will change our way of every day orthodontic

eliminate all factors that may cause a malocclusion: habits,
parafunctions, forced bites, habitual mouth-breathing.
Retrospective cephalometric short- and long-term studies of
Class II patients treated with the functional device Bionator document the kind and magnitude of the changes during the growth periods and the following aging. A Class I occlusion is primarily achieved
by dentoalveolar compensation during prepuberty and by inherited
favorable skeletal growth adaptation during puberty. A comparison
of initial and final variables of treated groups with homogeneous
untreated groups demonstrate the real treatment effects: skeletal,
dento-basal, dento-alveolar and reveals indication and contraindication of a Bionator.

F
rom a legal point of view, you cannot simply take photos of your
patients and use them for any purpose you want. This presentation outlines the legal framework that governs the taking and
handling of patients’ photos.
‣ In a first step, we will address everything you need to know about
obtaining valid consent from your patients to take photos of them.
‣ Secondly, it will be explained to what extend you are allowed to
use these photos subsequently (i.e. use them in a lecture or on your
website). The presentation will finish off with a few practical tips on
how to make sure you’re always on the safe side, legally.
CURRICULUM VITAE
Dr. iur. Philip Radlanski studied law (focused studies: law within the information society) at the University of Regensburg, Germany, and the University of Sheffield, UK. He

This book presents useful tips and strategies on how to integrate the Invisalign system successfully into clinical practice.
The authors review the diagnostic protocols and the biomechanics of aligners before presenting Invisalign treatment
protocols. With the support of accompanying case documentation, discussion of each malocclusion includes information
on the associated symptoms, the rationale behind the selected treatment approaches, and the various outcomes achieved. The last section of this book deals with the advantages of
the Invisalign system and can help patients and clinicians in
deciding whether this system can provide optimal treatment
outcomes for a particular clinical situation. This is a practical
manual for any clinician interested in the novel treatment
modality of aligner orthodontics.

avoid extraction treatment and as a side effect of fixed class II
treatment, many orthodontic treatments are finished with the lower
incisors in a more proclined position.
‣ In the presented lecture, possible implications of this treatment
approach on stability and on soft tissues as well as possible periodontal concerns are discussed by the help of abundant clinical
cases seen in daily practise.

The role of an active
self-ligating bracket
system in formation of a
myodynamic equilibrium
Aesthetics during the
treatment without
compromises
— is it available?
T
he effect of expansion in application of self-ligating brackets is
the subject of debate by many scientists. We proposed a hypothesis about a special mechanism of impact between active
self-ligating brackets and soft tissues; thereby it was observed more
expansion of dentition with alveolar bone remodelling. A study of
biomechanical model of interactive self-ligating bracket in vitro revealed, that on dentition alignment stage in round section archwire
expansion takes place in the region of the crowns with simultaneous
vestibular movement of roots by shifting the center of rotation towards crowns, during interaction in the system orthodontic archwire
— interactive self-ligating bracket — tooth — soft tissues.
Clinical investigations indicate the stability of results of such an
approach in which the shape of the dental arch is determined by the
state of the perioral muscles. This phenomenon is particularly important in the application of the principles of neuromuscular dentistry;
where achievement of new muscular balance with relaxation of
muscle spasm is carried out before active orthodontic treatment.
A
esthetics is one of the main goals of orthodontic treatment.
Nowadays, due to increasing numbers of adult patients, orthodontists are faced with the issue of preserving aesthetics during the treatment beside achieving aesthetics as a result of
treatment only. Many adults refuse treatment just because of visible
elements on the front teeth.
As aesthetic alternatives to labial braces, modern orthodontics
offers aligners and lingual technique. Both require compromises
from orthodontists. This means going to the same purpose, causing
himself and the patient some difficulties. In the case of aligners — it
is about limitations in biomechanics as well as shortcomings of the
removable device. In the case of lingual technique compromise is
related to the damage of tongue and overall discomfort, caused by
braces and archwires on the lingual surface.
Based on this, an alternative technique, implying the absence of
labial braces on front teeth, lingual braces and arches on the lingual
surface of the laterals can be regarded as attempt to minimise compromises for the sake of preserving aesthetics during treatment.

Sleep apnea treatment
— surgical and

O
bstructive Sleep Apnea Syndrom (OSAS) is a well-known
problem in diagnosis and treatment of different disciplines.
Most of the cases occur in patients who are in their fourth
decade of age. In respect to the demographic change of our population the number is expected to rise. Reduction of obesity and ENT
Examination will often be the first step in treatment. A well accepted
overnight OSAS treatment option is still CPAP-device to obtain a sufficient oxygenation for the patient. Millions of patients worldwide are
applying OSAS treatment systems. In contrast we prefer treatment
options, which dont’ interfere as much in the patient’s daily life. These
include surgical interventions to eliminate airway obstructions.
These techniques delineate forward movement of the maxillarymandibular complex to achieve sufficient airway space, providing
undisturbed breathing during the different sleeping periods.
In our 20 years of experience we observed remarkable benefits
for the patients. The procedure has outstanding and lasting benefits
for the patients: we observed general improvement in patient activities, decrease of hypertension, regression of diabetes and other
beneficial parameters. Most of the patients where operated in close
cooperation with Orthodontists. We will show cases more than ten
years postoperative and follow up.

N
euromuscular Dentistry is a diagnostic and treatment modality that aims to ensure that muscles and TMJ function
properly as a result of creating a functional occlusion. It enables the optimum physiologic position of the jaw to be established to
assist in the correction of the underlying causes of craniofacial Temporomandibular joint, head and neck pain. Neuromuscular dentistry
is also used to determine the optimum physiologic jaw position prior
to complex dental restorative procedures, cosmetic dentistry, dental
sleep medicine procedures, dentofacial orthopedics and orthodontics. It is a treatment modality of dentistry that focuses on correcting
the physiologic "misalignment" of the jaw at the Temporomandibular
joint (TMJ).
‣ The use of bioelectrical instrumentation such as Computerized
Mandibular Scanning (CMS), Trans Cutaneous Electrical Nerve Stimulation (TENS) and Surface Electromyography (SE) is described in
detail. Clinical cases show the basic procedures of Neuromuscular
Orthodontics that describe the road to success.

rthodontics is nowadays an indispensable part from the demanding treatment concepts of the aesthetic and functional
dentistry. By the use of innovative methods and equipment
even complicated teeth misalignments can be corrected. However,
orthodontic treatments are often avoided of adolescents and adults
for cosmetic reasons, as the majority of patients consider invasive
multibracket appliances as unacceptable. Nowadays the orthodontist
has several "invisible" treatment options available, one of them is the
Invisalign System. After 15 years of experience with the system, we
believe that this technology is not only a successful but little invasive
treatment option for mild to moderate crowding, gaps in the anterior
region and tilted teeth, but also in extensive cases, such as extractions, open and deep bites, dental class II & III treatments and in the
therapy of craniomandibular dysfunctions or as part of a combined
orthodontic-oral surgical treatment. Expansion of the dental arch, inand extrusions of individual teeth or groups of teeth as in the deep bite,
torque-like movements of front teeth and the correction of the tooth
axis have become an integral part of the Invisalign treatment. With
Invisalign Teen the technique is also available for children and young
people and can be combined with functional orthodontic treatments.
‣ The presentation will show possibilities of the Invisalign System,
the treatment planning and gives tips and hints for this treatment
technique.

n modern orthodontics 3D-imaging of the dental, skeletal and soft
tissue compartments are available in the daily clinical setting for
diagnostics and therapy planning. Moreover the opportunity of
3D- imaging nowadays from infants to adulthood provides further
implementation of orthodontics in medicine: In the first year of life
a non-invasive 3D-imaging allows an early diagnostic of head and
face asymmetries. Thus, the orthodontists’ responsibility could be to
counteract a progressive growth of the head and face by orthopaedic approach already in an early stage of life.
In adolescence 3D-imaging offers a variety of opportunities to
get additional therapy-relevant information about dental malocclusion, skeletal morphology and soft tissue reaction during comprehensive orthodontic treatment. At the end of growth in treatment of
adults with periodontics or in the treatment of orthognathic patients
3D-imaging permits new perspectives in diagnostics and in planning the biomechanical treatment approach.

lot of progress has been achieved recently in modern orthodontics which can help to improve and simplify the treatment
especially in complicated cases, such as solving anchorage
problems with the application of TAD’s and SUS², solution’s for noncompliance patients, overcoming of treatment-obstacles in aesthetic
aspects with the help of various aligners and lingual systems, avoiding surgery in adult cases, and management of multiple aplasia, premature molars-loss and teeth retention/ankyloses, as well as recommendations to avoid/minimize root resorption and relapse.
‣ This lecture is to demonstrate new and approved methods of
modern orthodontics and to show its possibilities and limits. Special
focus is put on the teamwork of different disciplines like parodontology, prosthetic and dysgnathic surgery and on the consideration of
the TMJ.

T
he 14th International Orthodontic Symposium 2016 will take place
in the Kaiserstein Palace. The address of the palace is "Malostranské namesti 23/37", Prague 1.
An ideal place for meetings from the times of High Baroque
We are looking forward to hosting
you as our guests, and please consider our guest book. For more than
three centuries, we have had many
satisfied and famous guests.
The Kaiserstein Palace received
a Baroque reconstruction initiated
by Sir Kaiserstein but completed
by Petr Radecky (1699–1720). In
1859 a monument of marshal Radecký was built on the square. Up
to 1918 the square was named
after him. In 1866, Václav Petzold,
↗ www.kaiserstejnsky-palac.cz
a hotel-owner, bought the palace
and opened an exclusive restaurant. For the next hundred years it
was called "U Petzoldů". The famous opera singer Emmy Destinn, a
partner of Enrico Caruso, rented an apartment there (1904–1908).
A total reconstruction began under the architects Zdeněk Pokorný
and Jaroslav Bělský in 1977.
In 1981 the palace has been registered as a UNESCO heritage
site. In 1997 restitiution procedures were completed and the palace
was returned to its original owners.
