2. JOB PURPOSE
Summary
In collaboration with an appointed AHP the post holder will have the responsibility for the
planning, delivery and evaluation of a project Transforming Care after Treatment across
NHS Tayside. The project will initially focus on services providing care for patients within the
three main tumour sites of Head & Neck, Prostate and Colorectal cancer. The post holder
will be responsible for the overall project management of the Tayside TCAT programme
using robust project management techniques and ensuring appropriate resources are
available to support its delivery within agreed timeframes and adhere to key milestones of
project. The post holder will provide professional and clinical leadership to health and
partnership organisations involved across Tayside and Fife to:







Ensure safe and effective clinical practice
Design, implement and evaluate the Transforming Care after Treatment programmes of
care
Enhance the patients experience of care
Manage and develop the performance of the team
Contribute to the delivery of the organisations objectives
Enable and promote patients’ independence
Ensuring interpretation of national policies for local implementation within specialist
service and providing specialist advice to NHS Tayside Board

4. SCOPE AND RANGE
NHS Tayside covers some 7,558 square kilometres with a population of approximately 394,000.
The TCAT programme is an initial 2 year funded project to address a fundamental priority of
cancer care the transformation from a disease-surveillance model of follow up to a person-centred
model of aftercare which enables people to live healthily with and beyond cancer within NHS
Tayside and will involve primary and secondary care and local authorities within Dundee, Perth
and Angus.
This project has three core aims:  To introduce systematic holistic needs assessment and an end of care treatment summary
across 3 cancer site specific teams of Head and Neck, Colorectal and Prostate cancer
through a co-ordinated programme of education and support, led by an Advanced Cancer
Specialist Nurse and an AHP co-ordinator.
 To develop pathways for enhancing health and wellbeing for cancer survivors, which promote
self-management and make maximum use of the many existing services and resources
available in local communities
 To support the development and evaluation of transformed models of follow up for patients
with complex needs who require on-going multidisciplinary rehabilitation.
The advanced specialist nurse will be project manager and professional lead for the TCAT team
which would consist of an AHP Lead and administrative support and has responsibility for:

As project manager leading on the TCAT Programme project the outcome of which will
contribute to delivering strategic service change and modernisation plans. Acting
independently and in collaboration to direct, develop, design and manage the project with
realistic but firm timescales for delivery;

The post holder will engage with key stakeholders (Clinicians, Patients, Staff, Local
Authorities and Voluntary Organisations) in the design and implementation of TCAT
Programme to ensure that the outcome is patient centred and co-ordinated. Engagement will
be achieved through a range of methods but in particular through the post-holder designing,
leading and facilitating appropriate workshops and communication events. Determining
appropriate points of contact and individuals key to this particular project. Engagement will be

an ongoing process throughout the project and will be focussed on gaining commitment and
ownership of plans and agreed actions.
Ensuring that standards of practice are maintained, reviewed and continuously developed and
that staff across Tayside, deliver safe, effective, patient-centred, efficient, timely and equitable
care within their scope of practice as part of the Transforming Care after Treatment team
Implement a rolling programme of education for hospital and community based nurses and
AHPs ,using Macmillan templates to facilitate the introduction of holistic needs assessment
and end of care treatment summaries and develop action plans and workable systems for
their use in practice
Creating an environment in which effective learning can take place underpinned by evidence
based practice
Mapping the involvement of the multidisciplinary teams within the designated areas
Developing the patient pathway from point of referral at end of primary treatment through to
follow-up and discharge
Piloting Nurse/AHP led clinics for new referrals and following up patients to ensure that they
are seen timeously by the appropriate person at the appropriate time
Leading the development of policies, guidelines and protocols in the specialty area and
ensuring that they are approved, implemented, monitored, evaluated and updated.
Instigating investigations following incidents/adverse events/complaints taking appropriate
action and follow up in line with the organisational policy/disciplinary procedures
The Advanced Specialist Nurse:
 Will introduce systematic holistic needs assessment and accompanying care planning at end
of primary treatment based on assessment of individual risks, needs and preferences.
 Will introduce the use of an end of treatment care summary across the three cancer sites
initially in a variety of settings throughout Tayside.
 Is an advanced practitioner with highly developed specialist knowledge and skills in this field
of practice whilst acting as a resource and providing advice in a wide range of situations.
 Will develop pathways for enhancing health and well being for patients living with and after
cancer, which promote self management and make maximum use of the many existing
services and resources available in the local communities.
 Support the development and evaluation of transformed models of follow up for patients with
complex needs who require ongoing multidisciplinary rehabilitation
 Leads service development throughout Tayside by working closely with a range of colleagues
to enhance patient care /experience.
 Ensures that methods of communication in relation to the patient is coordinated between
acute and primary health care teams through implemented documentation,
 Ensures the appropriate level of expertise and skill mix and managing annual leave and study
leave entitlement to ensure that service is appropriately covered.
 Has budgetary responsibilities for the project including supplies ordering, use of equipment,
effective use of physical, human and financial resources within the area

5. MAIN DUTIES/RESPONSIBILITIES
Within the governance framework of the organisation (including clinical, staff and financial
governance) the post holder will focus and lead on the following key result areas:
1. TO ENSURE SAFE & EFFECTIVE CLINICAL PRACTICE
1.1 Clinical Leadership and Teamwork
As clinical leader promotes teamwork within a multi-professional environment in conjunction with
teams of the three cancer sites of head and neck, colorectal and prostate cancer, demonstrating
critical analysis and decision making skills, leading the delivery of a clinically excellent, high
quality cancer follow up service influencing and facilitating change throughout Tayside and where
appropriate, the organisation.
Adhere to key milestones within project plan
Lead effective teamwork and healthy working relationships through consistent and visible support
To support the development and evaluation of transformed models of follow up for patients with
complex needs who require ongoing multidisciplinary rehabilitation
Implement systems to facilitate a sensitive and positive approach to the review of complaints
learning from the experience of users to improve healthcare experience
1.2 Evidence Based Clinically Effective Practice
Act as a change agent leading the development of clinically effective practice through the
effective utilisation and integration of evidence; setting, implementing and monitoring evidence
based policies, procedures and protocols.
Lead and facilitate team participation in ongoing research at all levels in nursing practice in
Cancer Specialty. From critical awareness of research, through implementing into practice
evidenced based research, to being identified principal investigator for nursing research in
specialty.
1.3 Continuous Quality Improvement
Continued development and evaluation of the electronic HNA using the Macmillan
I pad Patient Concerns Inventory(MiPCI) for head and neck cancer
Liaison with NHS Tayside IT dept to embed end of treatment assessment template into existing
systems to facilitate electronic communication of treatment summary to primary care
Lead the identification and development of frameworks and mechanisms to monitor standards
and quality of clinical practice, ensuring a culture of continuous quality improvement through the
use of audit, patient feedback and reflection on practice by self and other members of the team.
Lead collaboration within and external to team in relation to quality improvement. Formulate and
present evidence and needs assessments for improvement, action plans for quality improvement
and monitor and report on progress and impact.

1.4 Patient Safety
Responsible for promoting a safe and clean environment for staff, patients and visitors by
ensuring compliance with legislation, policies and protocols e.g. health and safety, healthcare
associated infection, risk management and critical incident reporting and analysis, assessing and
managing actual and potential risks to health and well-being.
Ensures high standard of record keeping in accordance with Nursing & Midwifery Council,
national legislation and local standards, facilitating effective communication with multiprofessional team regarding patient care.
Responsible for data entry and storage of data using computer based systems.
1.5 Patient Care
Using a risk stratification approach identify patients who have particularly complex needs after the
end of treatment and work with existing teams to establish alternative and person-centred models
of care which foster multidisciplinary working and are designed to address complex problems
such as incontinence, pain and eating difficulties.
Provide a coordinated advanced specialist nursing and AHP contribution to the clinical
management of a defined caseload working autonomously and in collaboration with
multidisciplinary professionals in order to improve the follow up care of patients with cancer
Participate in multidisciplinary team decision making, presenting patients, discussing treatment
options whilst documenting outcomes in patient’s notes, discussing proposed treatment plan with
patient, relevant clinicians and general practitioner.
Deliver Nurse/AHP led clinics for follow-up for patients with complex needs who require ongoing
multidisciplinary rehabilitation.
Undertake ongoing evaluation and revision of patients with complex problems with respect to
clinical interventions which have been suggested.
Perform and interpret the results of tests undertaken to diagnose and/or manage the patients’ with
complex symptoms at end of cancer treatments. Plan and initiate follow-up management of
patients depending on the results of examinations. Discuss results with patient recommending
medications, further investigations or treatments.
Assess the health education needs of patients initiating health promotion, providing patients with
written information where appropriate.
Assess patients presenting with unscheduled problems by telephone or as an out-patient,
providing advice, requesting appropriate investigations and organising admission to hospital if
appropriate
Initiate ongoing referrals for further assessment, diagnostic tests and to other professionals.
Prescribe and/or make recommendations within the multi-disciplinary team and within primary
care about patients’ medications particular to the speciality, working within the scope of their own
professional practice, adhering to the NMC Standards for Medicines management and the NHS
Tayside Safe and Secure Handling of Medicines Manual.

Contribute to the avoidance of unnecessary hospital admissions through improved patient self
management and enhanced symptom control.
2. TO ENHANCE THE PATIENT’S EXPERIENCE
2.1 Coordination of the Patient Journey
Scope out existing services to promote health and wellbeing across Tayside, utilising the ALISS
and TIPS resources and health and social care and third sector partners to enable development
of package of options for signposting and referring patients onto programmes which promote
physical activity, smoking cessation, nutritional care, vocational rehabilitation, financial support
and psychological wellbeing
2.2 Clinical Expertise
Responsible for introducing systematic holistic needs assessment and an end of care treatment
summary for patients across the 3 cancer site specific teams ensuring a co-ordinated programme
of education and support in collaboration with AHP coordinator.
Responsible for ensuring co-ordination of specialist nursing interventions, influencing clinical
decisions and monitoring the quality of patient care provided in acute and primary care, and within
community hospitals, through using expert clinical knowledge relevant to own field of practice,
underpinned by theory and experience.
2.3 Promote a Culture of Person Centred Care
Promote a greater focus on recovery, health and well being after cancer treatment
Promote patient participation in decision making including the process of obtaining informed
consent for examinations/ interventions ensuring that the nature, consequences, alternatives and
risks of treatment are understood by the patient.
Within a multidisciplinary team environment, develop a culture of person-centred care,
communicating regularly with patients, relatives and/or carers; promoting a caring environment
where equality and diversity issues are respected and patients are enabled to be partners in their
care.
Identifies opportunities to develop care and services by ensuring that there are effective systems
in place to ascertain patient and carer experience/feedback and complaints are managed in line
with organisational policy including the dissemination of learning points.
3. TO MANAGE & DEVELOP THE PERFORMANCE OF THE TEAM
3.1 Role Model
Act as a role model, creating a supportive ethos to empower staff to contribute to the delivery of
high quality person-centred care.

strategies (mentoring, coaching, clinical supervision and action learning) and planning on-going

knowledge of current clinical and professional issues and legalities.

3.3 Managing the Practice Setting
Manage the practice setting, ensuring effective use of resources and workforce planning by
monitoring workload and maintaining appropriate staffing levels taking account of role and
competence of staff when delegating work, contributing to the management of the service budget.
Manage the nursing/midwifery team by ensuring compliance by self and others with professional
standards including (NMC Code of Professional Conduct), legislation, national and organisational
policies, leading recruitment and selection, attendance management, ensuring grievance and
disciplinary matters within own department are identified, actioned and reported to the appropriate
manager
Ensure systems are operational to ensure all staff have current professional registration
Participate in attendance management completing return to work interviews and follow-up action

4.4 Service delivery and organisation
Works with managers to suggest and improve the services provided to the local community,
representing nursing in local service improvement meetings
Be responsible for establishing and management of the planned TCAT Stakeholder group which
will include membership from the relevant cancer site specific teams, primary care, community
health partnerships and local authority and voluntary agencies and patient support groups.
6. EQUIPMENT & MACHINERY
The Registered Nurse is expected to have the knowledge and skills necessary to use all
equipment safely in the area The post holder will be required to manage the use of the
following equipment for the reasons stated and is responsible for ensuring that
systems/policies/procedures are communicated to staff to ensure safe use, maintenance and
storage of equipment in the area:






Frequent use of moving and handling equipment used within wards and clinics
Medical and Technical equipment to record vital signs and administer treatments
Near patient testing to monitor physiological status
IT equipment including local and national systems to read, analyse, record and transmit
patient and staff information within the boundaries of local and national policies and
legislation

7. DECISIONS & JUDGEMENTS

The post holder
Will be responsible to Head of Nursing for guidance and professional management, work
review and formal appraisal of performance.

Will be responsible for providing senior management teams with expert advice relating to
the specialist field.
Makes highly complex clinical decisions including diagnosis and clinical management
based on in-depth expert knowledge and interpretation of clinical and other findings such
as laboratory and radiological investigations.
Acts as an autonomous practitioner, acting on own initiative. Plans, manages and
prioritises own caseload whilst appropriately delegating the weekly workload to other
nurses working within the specialist nurse team.
Will have continuing responsibility for setting and monitoring standards and quality of
clinical practice and ongoing management of the specialist nursing team.
Has responsibility for supporting the nursing team to reflect upon and review their
decisions in relation to assessing, monitoring, evaluating and interpreting patients’
condition and effectiveness of their care programmes.

Physical Skills (several times per shift)
The post holder will require a range of physical skills relevant to clinical area examples are
IT key board skills, general use of IT systems, use of diagnostic equipment for clinical
examination and technical skills.
Physical Demands (several times per shift)
The physical demands include movement and transportation of bulky equipment
associated with patient care, movement of tables, chairs etc for meetings and clinical
consultations, and travel on a daily basis within NHS Tayside.
Mental Demands (several times per shift)
The mental demands include concentration required when preparing reports, analysing
data with unpredictable interruptions and changing priorities; concentration required when
observing patient behaviours/physiological status; balancing the competing demands of
the role; maintaining high level and consistent professional behaviour in unpredictable and

4. SCOPE AND RANGE
NHS Tayside covers 7,558 square kilometres with a population of 400,000. TCAT is a 2 year
funded project to address a fundamental priority of cancer care – transformation from a follow-up,
disease-surveillance model to a pro-active, person–centred model of aftercare that enables
people to live healthily with and beyond a cancer diagnosis within NHS Tayside. The model will
bring together primary and secondary care and local authorities across Angus Dundee, Perth and
Kinross
This project has three core aims: To introduce systematic holistic needs assessment and an end of care treatment summary
across 3 cancer site specific teams of Head and Neck, Colorectal and Prostate cancer
through a co-ordinated programme of education and support, led by an Advanced Cancer
Specialist Nurse and an AHP co-ordinator.
 To develop pathways for enhancing health and wellbeing for cancer survivors, which promote
self-management and make maximum use of the many existing services and resources
available in local communities.
 To support the development and evaluation of transformed models of follow up for patients
with complex needs who require on-going multidisciplinary rehabilitation.
This post will be a key team member helping to deliver on the following:











Develops pathways for enhancing health and well being for cancer survivors, which promote
self management and make maximum use of the many existing services and resources
available in the local communities.
Lead and evaluate new models of management and rehabilitation for patients with complex
needs in a community environment.
Leads implementation of holistic assessment and personalised care planning based on
individual risks, needs and preferences.
Leads development and implementation of end of treatment care summary for three cancer
pathways across Tayside.
Co-ordinates care across acute and primary health care teams
Demonstrates clinical and leadership skills, providing wide-ranging professional advice to
other professional, patients and their carers.
Operates as an advanced practitioner with highly specialist knowledge of cancer care,
Leads service development throughout Tayside by working closely with a range of colleagues
to enhance experience and quality of care.
Lead workforce development and appraisal of service and staff within the TCAT team.
Maintain financial and budgetary control in line with Standing Financial Instructions
Responsible for performance management of staff and compliance with
Environment of Care
 Tayside-wide Primary and Secondary Care
 Outpatients departments, clinical areas, patient’s homes.

5. MAIN DUTIES/RESPONSIBILITIES
Support the implementation of the TCAT Programme project














Work in collaboration with Clinical Leads and the Multi-disciplinary/agency team
Provide leadership and facilitate engagement of AHPs and nurses within the TCAT team
Develop wider professional engagement and involvement in Cancer care.
Provide leadership, expert advice and support to managers and clinicians for the three cancer
pathways; head and neck, colorectal and prostate to achieve better patient experience and
clinical outcomes.
Work in collaboration with staff and services across Tayside and in conjunction with other
organisations / partners to develop approaches that systematically improve the aftercare of
patients living with cancer.
Enable AHPs to build capacity and capability for improvement and practice development
through the application of service improvement and quality improvement methodologies.
Promote and support a culture of learning, research, development and integrated working.
Use data to evaluate the effectiveness of the service and to develop programmes for
improvement.
To develop and sustain programmes of multidisciplinary rehabilitation.
Implement evidence based practice and develop a cycle of continuous service improvement.
Lead and facilitate team participation in ongoing research.
Implement a rolling programme of education for all staff to support effective use of Macmillan
assessment templates and end of care treatment summaries
Develop plans and reporting mechanisms to demonstrate implementation.
Lead development of policies, guidelines and protocols in the specialty in line with NHS
Tayside policy.
6. COMMUNICATIONS AND RELATIONSHIPS
The role involves communicating with other disciplines and staff across a range of clinical
groupings. Working relationships associated with this post are complex and need highly
developed interpersonal and communication skills. It is essential therefore that the postholder is
able to effectively communicate confidentially and sensitively and have a good understanding of
issues to be able to resolve difficulties where appropriate. The nature of this post necessitates
excellent communication and interpersonal, facilitation, influencing and negotiation skills.
The postholder has a key communication role and is expected to establish and maintaining
effective communication and productive working relationships with a wide range of personnel and
agencies. The key stakeholders will include NHS Tayside Committees, NHS Board members,
Non-Executive Directors, Senior Managers and Clinical Leaders. The postholder is also required
to communicate with the Scottish Government, Healthcare Improvement Scotland, other Health
Boards, Special Boards, Universities and further education establishments, local authorities,
partner agencies, service users and carers.
There is a need for highly developed inter-personal, communicate and presentational skills.
These skills are necessary to inform, discuss and persuade all staff members from senior
managers, senior clinicians and non clinical staff as the direction and necessity of quality
improvement. The postholder will be required to identify the most appropriate and applicable
communication method which will require to be used. Communications can be to individuals, staff
groups, organisation wide conferences and at public meetings.

7. KNOWLEDGE, TRAINING AND EXPERIENCE REQUIRED TO DO THE
JOB

















Educated to degree level or equivalent with demonstrable ability to learn and develop at
Masters level evidence through a formal qualification or a combination of others forms of
study or learning.
A commitment to life long personal learning and development in the management,
improvement and delivery of healthcare services.
Previous experience within the health service, or equivalent related service, demonstrating
knowledge and understanding of complex organisations and the challenges of working within
such an environment.
Experience working with AHP services and an understanding of the various professions is
essential.
An in depth knowledge of corporate governance and organisational policies and procedures.
Self-motivated with the ability to work as part of a team in a flexible manner liaising with senior
and clinical staff.
Ability to work independently using effective self-management of time and priorities and
deliver projects / work to specified timescales including working under pressure.
Able to demonstrate that ability to develop quickly and retain an understanding of the working
of the organisation.
Excellent oral, written and visual communication skills.
Good interpersonal and teamwork skills.
Advanced computer skills and literacy are essential. The ability to work proficiently within the
full range of current software packages (Word, Access, Excel, and PowerPoint) is paramount.
Well developed personal and communication skills are required in the information gathering
element of the role.
The ability to work against a background of continual change is essential.
The post holder will demonstrate experience in project management across various
disciplines and a clear understanding of quality improvement methodologies and change
management.

The terms and conditions of service for this post are those
determined by the NHS Staff Council.
The current salary scale for the post is £39,632 to £47,559 per
annum (pro rata for part-time staff). Placing on the scale on
appointment is normally at the minimum but may be higher
subject to verification of previous relevant service.
Salary is paid monthly by Bank Credit Transfer.
The hours of the post are 22.5 per week for initial 6 months then
37.5 per week for 18 months.
This is a fixed term/secondment post for 2 years as a result of
fixed term funding
Internal applicants: Written approval must be sought from your
line manager prior to applying for this secondment. You will be
required to bring this if shortlisted for interview.
Superannuation
Start and finish times will be determined by the needs of the
service.
Membership of the NHS Superannuation Scheme is not
compulsory but is open to all staff between the ages of 16 and
70 (65 in some instances).
The contributions paid are a percentage of superannuable pay,
which is essentially basic pay excluding, for example, overtime
or travelling expenses.
