SCOPE AND RANGE NHS Tayside covers some 7,558 square kilometres with a population of approximately 394,000. The TCAT programme is an initial 2 year funded project to address a fundamental priority of cancer care the transformation from a disease-surveillance model of follow up to a person-centred model of aftercare which enables people to live healthily with and beyond cancer within NHS Tayside and will involve primary and secondary care and local authorities within Dundee, Perth and Angus. This project has three core aims: To introduce systematic holistic needs assessment and an end of care treatment summary across 3 cancer site specific teams of Head and Neck, Colorectal and Prostate cancer through a co-ordinated programme of education and support, led by an Advanced Cancer Specialist Nurse and an AHP co-ordinator. To develop pathways for enhancing health and wellbeing for cancer survivors, which promote self-management and make maximum use of the many existing services and resources available in local communities To support the development and evaluation of transformed models of follow up for patients with complex needs who require on-going multidisciplinary rehabilitation. The advanced specialist nurse will be project manager and professional lead for the TCAT team which would consist of an AHP Lead and administrative support and has responsibility for: As project manager leading on the TCAT Programme project the outcome of which will contribute to delivering strategic service change and modernisation plans. Acting independently and in collaboration to direct, develop, design and manage the project with realistic but firm timescales for delivery; The post holder will engage with key stakeholders (Clinicians, Patients, Staff, Local Authorities and Voluntary Organisations) in the design and implementation of TCAT Programme to ensure that the outcome is patient centred and co-ordinated. Engagement will be achieved through a range of methods but in particular through the post-holder designing, leading and facilitating appropriate workshops and communication events. Lead and facilitate team participation in ongoing research at all levels in nursing practice in Cancer Specialty. From critical awareness of research, through implementing into practice evidenced based research, to being identified principal investigator for nursing research in specialty. 1.3 Continuous Quality Improvement Continued development and evaluation of the electronic HNA using the Macmillan I pad Patient Concerns Inventory(MiPCI) for head and neck cancer Liaison with NHS Tayside IT dept to embed end of treatment assessment template into existing systems to facilitate electronic communication of treatment summary to primary care Lead the identification and development of frameworks and mechanisms to monitor standards and quality of clinical practice, ensuring a culture of continuous quality improvement through the use of audit, patient feedback and reflection on practice by self and other members of the team. Lead collaboration within and external to team in relation to quality improvement. Formulate and present evidence and needs assessments for improvement, action plans for quality improvement and monitor and report on progress and impact. Commission and work effectively with organisational development and service improvement General Conditions 05.08 colleagues to focus on identified and agreed quality improvement projects. 1.4 Patient Safety Responsible for promoting a safe and clean environment for staff, patients and visitors by ensuring compliance with legislation, policies and protocols e.g. health and safety, healthcare associated infection, risk management and critical incident reporting and analysis, assessing and managing actual and potential risks to health and well-being. Responsible for data entry and storage of data using computer based systems. 1.5 Patient Care Using a risk stratification approach identify patients who have particularly complex needs after the end of treatment and work with existing teams to establish alternative and person-centred models of care which foster multidisciplinary working and are designed to address complex problems such as incontinence, pain and eating difficulties. Provide a coordinated advanced specialist nursing and AHP contribution to the clinical management of a defined caseload working autonomously and in collaboration with multidisciplinary professionals in order to improve the follow up care of patients with cancer Participate in multidisciplinary team decision making, presenting patients, discussing treatment options whilst documenting outcomes in patient’s notes, discussing proposed treatment plan with patient, relevant clinicians and general practitioner. Deliver Nurse/AHP led clinics for follow-up for patients with complex needs who require ongoing multidisciplinary rehabilitation. Undertake ongoing evaluation and revision of patients with complex problems with respect to clinical interventions which have been suggested. Perform and interpret the results of tests undertaken to diagnose and/or manage the patients’ with complex symptoms at end of cancer treatments. Plan and initiate follow-up management of patients depending on the results of examinations. Discuss results with patient recommending medications, further investigations or treatments. Assess the health education needs of patients initiating health promotion, providing patients with written information where appropriate. Assess patients presenting with unscheduled problems by telephone or as an out-patient, providing advice, requesting appropriate investigations and organising admission to hospital if appropriate Initiate ongoing referrals for further assessment, diagnostic tests and to other professionals. Prescribe and/or make recommendations within the multi-disciplinary team and within primary care about patients’ medications particular to the speciality, working within the scope of their own professional practice, adhering to the NMC Standards for Medicines management and the NHS Tayside Safe and Secure Handling of Medicines Manual. TO ENHANCE THE PATIENT’S EXPERIENCE 2.1 Coordination of the Patient Journey Scope out existing services to promote health and wellbeing across Tayside, utilising the ALISS and TIPS resources and health and social care and third sector partners to enable development of package of options for signposting and referring patients onto programmes which promote physical activity, smoking cessation, nutritional care, vocational rehabilitation, financial support and psychological wellbeing 2.2 Clinical Expertise Responsible for introducing systematic holistic needs assessment and an end of care treatment summary for patients across the 3 cancer site specific teams ensuring a co-ordinated programme of education and support in collaboration with AHP coordinator. Responsible for ensuring co-ordination of specialist nursing interventions, influencing clinical decisions and monitoring the quality of patient care provided in acute and primary care, and within community hospitals, through using expert clinical knowledge relevant to own field of practice, underpinned by theory and experience. 2.3 Promote a Culture of Person Centred Care Promote a greater focus on recovery, health and well being after cancer treatment Promote patient participation in decision making including the process of obtaining informed consent for examinations/ interventions ensuring that the nature, consequences, alternatives and risks of treatment are understood by the patient. Within a multidisciplinary team environment, develop a culture of person-centred care, communicating regularly with patients, relatives and/or carers; promoting a caring environment where equality and diversity issues are respected and patients are enabled to be partners in their care. Identifies opportunities to develop care and services by ensuring that there are effective systems in place to ascertain patient and carer experience/feedback and complaints are managed in line with organisational policy including the dissemination of learning points. 3. To support the development and evaluation of transformed models of follow up for patients with complex needs who require on-going multidisciplinary rehabilitation. This post will be a key team member helping to deliver on the following: Develops pathways for enhancing health and well being for cancer survivors, which promote self management and make maximum use of the many existing services and resources available in the local communities. Lead and evaluate new models of management and rehabilitation for patients with complex needs in a community environment. Leads implementation of holistic assessment and personalised care planning based on individual risks, needs and preferences. Leads development and implementation of end of treatment care summary for three cancer pathways across Tayside. Co-ordinates care across acute and primary health care teams Demonstrates clinical and leadership skills, providing wide-ranging professional advice to other professional, patients and their carers. Operates as an advanced practitioner with highly specialist knowledge of cancer care, Leads service development throughout Tayside by working closely with a range of colleagues to enhance experience and quality of care. Lead workforce development and appraisal of service and staff within the TCAT team. Maintain financial and budgetary control in line with Standing Financial Instructions Responsible for performance management of staff and compliance with Environment of Care Tayside-wide Primary and Secondary Care Outpatients departments, clinical areas, patient’s homes. MAIN DUTIES/RESPONSIBILITIES Support the implementation of the TCAT Programme project Work in collaboration with Clinical Leads and the Multi-disciplinary/agency team Provide leadership and facilitate engagement of AHPs and nurses within the TCAT team Develop wider professional engagement and involvement in Cancer care. Provide leadership, expert advice and support to managers and clinicians for the three cancer pathways; head and neck, colorectal and prostate to achieve better patient experience and clinical outcomes. Work in collaboration with staff and services across Tayside and in conjunction with other organisations / partners to develop approaches that systematically improve the aftercare of patients living with cancer. Enable AHPs to build capacity and capability for improvement and practice development through the application of service improvement and quality improvement methodologies. Promote and support a culture of learning, research, development and integrated working. Use data to evaluate the effectiveness of the service and to develop programmes for improvement. To develop and sustain programmes of multidisciplinary rehabilitation. Implement evidence based practice and develop a cycle of continuous service improvement. Lead and facilitate team participation in ongoing research. Implement a rolling programme of education for all staff to support effective use of Macmillan assessment templates and end of care treatment summaries Develop plans and reporting mechanisms to demonstrate implementation. Lead development of policies, guidelines and protocols in the specialty in line with NHS Tayside policy. 6. COMMUNICATIONS AND RELATIONSHIPS The role involves communicating with other disciplines and staff across a range of clinical groupings. Working relationships associated with this post are complex and need highly developed interpersonal and communication skills. It is essential therefore that the postholder is able to effectively communicate confidentially and sensitively and have a good understanding of issues to be able to resolve difficulties where appropriate. The nature of this post necessitates excellent communication and interpersonal, facilitation, influencing and negotiation skills. The postholder has a key communication role and is expected to establish and maintaining effective communication and productive working relationships with a wide range of personnel and agencies. The key stakeholders will include NHS Tayside Committees, NHS Board members, Non-Executive Directors, Senior Managers and Clinical Leaders. The postholder is also required to communicate with the Scottish Government, Healthcare Improvement Scotland, other Health Boards, Special Boards, Universities and further education establishments, local authorities, partner agencies, service users and carers. There is a need for highly developed inter-personal, communicate and presentational skills. These skills are necessary to inform, discuss and persuade all staff members from senior managers, senior clinicians and non clinical staff as the direction and necessity of quality improvement. The current salary scale for the post is £39,632 to £47,559 per annum (pro rata for part-time staff).
