LAT IN CLINICAL ONCOLOGY
Introduction
Applicants for the Specialist Registrar post in Clinical Oncology must have full GMC
registration, completed general professional training and must hold MBChB or
equivalent and would normally be expected to have passed all parts of the MRCP.
Whilst this post is in clinical oncology, applications from candidates interested
ultimately in a career in medical oncology, haematology or palliative care would be
welcomed as this post would also provide relevant experience and training.
Description of the Area
The area of the NHS Tayside coincides with that of the Tayside Region. It
encompasses the former counties of Angus, Perth & Kinross together with the cities
of Dundee and Perth, and the Burghs of Arbroath, Forfar, Brechin and Montrose. The
Single Delivery Unit of NHS Tayside currently provides a service to Tayside and
North Fife - a population of half a million.
Dundee is Scotland’s fourth largest city and lies 20 miles east of Perth on the north
bank of the River Tay. It houses two large Universities (Abertay and Dundee) and is
the focal point for the on-going development of a thriving biomedical technology
industry.
Perth is the regional centre for a predominantly rural county of Perthshire and
Kinross. Several international companies have their headquarters in Perth.
Both cities provide a mixture of traditional and modern living and the surrounding
area provides an extensive choice of housing and schooling. Most leisure activities
are available close by with particularly good facilities for hill-walking, climbing, ski-ing
and golf.
The Hospitals
In Tayside there are three main hospital sites – Ninewells Hospital in Dundee, Perth
Royal Infirmary in Perth and Stracathro Hospital, near Brechin. Collectively they form
Tayside University Hospitals (TUH)
Ninewells Hospital and Medical School is a major general teaching hospital and
provides a comprehensive range of services with specialist units. Ninewells Hospital
and Medical School has been designated one of the Regional Cancer Centres in
Scotland and provides, on a single site, all Surgical, Radiotherapy, and Medical
Oncology services. In addition, on the Ninewells site are all the major disciplines
including general medicine and surgery as well as specialist areas such as
gynaecological oncology, ENT, plastics, neurosurgery etc. NHS Tayside and the
University of Dundee have a number of joint professorial appointments in Surgical
Oncology, Clinical Oncology and Cancer Medicine. The major clinical development
of Oncology Services complements the international standing of laboratory
Biomedical Research under the direction of Professor Sir David Lane, Professor
Roland Wolf, and Professor Sir Philip Cohen.
Perth Royal Infirmary general surgical unit provides acute services for Breast,
Colorectal and Gastroenterology. A 36 bedded Surgical Unit is located in the new
development at Perth Royal Infirmary, which was opened in 1993. Emergency
admissions are received through an innovative integrated medical/surgical
admissions unit. NHS Tayside has expanded its diagnostic imaging services with full
cross-sectional imaging services now available at Ninewells, PRI and Stracathro
Hospitals. Perth Royal Infirmary is recognised as a Cancer Unit for Tayside and a
day chemotherapy unit will opened at PRI in 2009 . Stracathro Hospital is a currently
provides an ambulatory, diagnostic and treatment centre for patients throughout
Tayside as well as a cancer support unit.
Tayside Cancer Centre
The Oncology unit is managed within the Surgical Directorate and functions as an
integrated part of Tayside Cancer Network
The heart of the service is provided in the Tayside Cancer Centre, which serves the
population of Tayside and North East Fife. Tayside Cancer Centre consists of a 26bedded ward; a 12 bedded 5-day chemotherapy unit; a chemotherapy day area and
an out-patient department. This modern department consists of three Varian Linacs b
with MLC and EPI. Other equipment includes a Nucletron Simulix Simulator,
Planning System,. Gynaecological brachytherapy services are also provided. as well
as intra-operative radiotherapy within ongoing clinical studies. We participate actively
in national and international Clinical Trials
The current clinical establishment is 9 WTE NHS consultant staff (8.4 WTE) and 2
university appointments (each 0.5 WTE NHS) working in 4 teams. The disposition of
the teams is decided on according to service requirements and individual interests.
The estimated numbers of patients to be seen in Oncology 2007/08 is 1799, 69% of
all patients diagnosed.

There are three staff grade doctors working in the chemotherapy day area.
Clinical Practice
Outpatient Cancer Care within the Oncology department is consultant-delivered. This
provides abundant opportunities for SpR training in all aspects of clinical and medical
oncology. Each team has at least one regular teaching ward round each week. There
are trust-wide MDT meetings in all of the major tumour sites each week.
SpRs are attached to specific clinical teams and are expected to provide advice and
support for the ward staff, participate in ward rounds, MDTs and outpatient clinics.
They also take responsibility for chemotherapy prescribing using a computerised
system (Chemo care). They also provide middle-grade out-of-hours emergency
cover: the oncology team on call comprises a ward-based doctor, an SpR and a
consultant.
SpR’s in clinical oncology would be expected to prepare for the FRCR and would be
encouraged to go on the appropriate courses in physics, radiobiology, cancer biology
etc. Interested candidates are encouraged to pursue their interests in research with a
view to publication. Medical student teaching is actively promoted by the department
and candidates who show interest in teaching are encouraged to attend formal
teaching courses. The centre has a good record in training their specialist registrars
whereby all the trainees have achieved their relevant competency skills and accepted
consultant posts in the UK.
The on-call commitment is at present a 1 in 10.
