Management Projects-Cabagan, Isabela 1't District, Sta.000+000-Sta.000+510-Sect. 2; Sta. 000+000Sta. 000+135-Sect 3, awarded to you with a Contract price equivalent to Two Hundred Forty Nine
Million Two Hundred Sixteen Thousand Two Hundred Eighty Eight Pesos and 99/100
(?249,216t288.99).
You are therefore required, to submit within ten (10) days from your receipt of this Notice of Award, to
submit the following documents:

Performance Security in any of the forms specified in Clause 32.2 of the Instructions to
Bidders;

Contractor/s All Risk Insurance;
3.
Duly signed PERT/CPM Network Diagram, Bar Chart with S-Curve, Cash Flow Projection
and Payment Schedule on monthly basis;
4.
Duly signed Manpower Schedule;
5. Ouly signed Equ\pment Ut\\rzation Schedute;
Duly signed Project Organizational Char$
Duly signed Construction Method in detailed narrative form;
B. l-atest Income Tax and Business Returns duly stamped and received by BIR and duly
validated with the tax payments made thereon
Tax Clearance from the BIR to prove full and timely payment of taxes;
10. Certification under oath stating that the contractor is free and clean of all tax liabilities to

11. Construction Safety and Health Program approved by the DOLH
Failure to comply with these requirements shall constitute a sufficient ground for the cancellation of this
award.
