This User Guide demonstrates the basic features of Lexi-Comp’s online software. ONLINE Accessing Applications From The Search Panel When you log on to Lexi-Comp ONLINE you will see the Welcome Page A with the Search Panel B . A B 2 1 3 4 5 6 Within the SEARCH panel are a number of Applications: 1 Lexi-Interact™ - provides the capability to REVIEW a comprehensive list of interactions associated with a specific drug, or to ANALYZE a profile for the presence of potential interactions. 2 Lexi-Drug ID™ - a tool used to identify a drug based on any of the following characteristics: imprint, dosage form, shape, or color. 3 Lexi-CALC™ - a comprehensive medical calculations application covering Infusions, Pediatric and Adult Conversions, and others. Lexi-CALC is a stand-alone application that can be integrated into fields within Lexi-Drug monographs. 4 Lexi-I.V. Compatibility™* - combines content from the King Guide to Parenteral Admixtures with complementary information from Lexi-Comp.It provides the ability to determine if drugs are compatible and with which solutions compatibilty is maintained. 5 Patient Care Module - delivers patient-specific education materials on medications, diseases, conditions, and procedures in up to 18 languages. Written at a 5th to 6th grade reading level, our Lexi-PALS™ (Patient Advisory Leaflets) are organized in a concise and straightforward format ideal for patient counseling. 6 Web Search - complements Lexi-Comp ONLINE by expanding the search capabilities to other medically-based web sites and the primary literature. A single search provides links to trusted, qualified healthcare web sites, eliminating the need to search sites independently. ® For further information visit www.lexi.com ® LEXI COMP Lexi-Comp ONLINE User Guide ONLINE The Search Panel Search for a topic in the Search panel (upper left corner of the Welcome Screen). Select a Lexi-Comp clinical database from the Search Results panel. Review Lexi-Comp’s most current information from a single screen in the Content panel. The Search Panel contains controls that allow you to access information. Using these controls, you may search for information directly 1 , by indexes 2 , or access applications 3 . Searching: Enter the desired search word or phrase in the window labeled "Search for". Click on the Search button or hit Enter, and the search results will be displayed in the Search Results panel. Wildcard characters may be used when only part of a word is known. The asterisk (*) matches any sequence of characters, and the question mark (?) matches any single character. When searching for a drug by name, there is no need to include any trailing wildcards. For example, to find all drugs commencing with the letters "war", enter “war” in the “Search for:” window. The results will be listed by brand and/or generic name. Note: A wildcard must be used at the beginning of the search text if looking for a drug where the name contains the search text. For example, entering “arin” will only produce a list of drugs that begin with the name “arin”. The Drop-down selection list labeled “Within:” allows you to limit the search to a particular section of the stored documents. The default is a NAME search. Options include: The Name field limits searching to generic and brand drug names, other names for lab tests, natural products, and synonyms, etc. There is also the option to search for Canadian Brand Names and International Brand Names. The All Text selection searches for any text that matches your search anywhere throughout the library of databases. The Charts/Special Topics selection limits your search to the material in the free-text sections of the database (Introduction, Appendices, and Special Topics). These sections include general reference material as well as guidelines, articles, graphs, and tables. The other choices (Adverse Reactions, Contraindications, Drug Interactions, Methodology, NDC, Therapeutic Category, Use, and Warning/ Precautions) are self-explanatory; they limit the search to specific fields of information in the documents contained in the databases. Clicking on a link in the list will display the content and fields of information in the Content Panel. You may use your browser's scroll bar to see the full list. All titles are listed in the Search Results Panel with any available indices. For each database, links to the following indices will appear: View a list of drugs starting with a selected letter (Generic Names, U.S. brand names) View a list of Charts/Special Topics New documents, last 90 days View all changes that have been made in the past 7 and 30 days View all of the Pharmacologic/Therapeutic Categories View all Special Alerts The Content Panel The monograph selected in the Search Results Panel (lower left) is displayed in the Content Panel (on the right side). The monograph and database titles appear at the top of the page. The search text that was entered in the Search Panel will be highlighted in the monograph. Access to drug identification information (Images), drug interaction information (Interactions), and Patient Advisory Leaflets (Adult-PALS and Pediatric-PALS) is provided through the Module Access buttons. There are three ways to search the Content Panel: the Jump To Field (Select Field Name) 2 , or by option (1) 1 , the scroll bar on the right (2) using the field menu that appears in the Search Results Panel once a monograph is displayed. (3) 3 Jump To Field (Select Field Name) window: Here the fields are listed in alphabetical order. Select a field of interest from this window to jump directly to that particular field within the monograph. Next to each field title is an arrow that will return you to the top of the monograph. 2 1 3 The field listing on the left side displays the fields in the order in which they appear within the monograph. Use this option for quick navigation, and if you want to see your search results again, just click on the Search button (leaving everything else the same). Clicking on a link in the Pharmacologic Category Field will search the database for monographs in that particular category. An updated Search Results Panel will then be displayed. Other links within the monograph include: Pronunciation of the drug name and Related Information (which includes links to additional content within the library that relates to the monograph presently displayed). An enhanced navigation feature is now included within the Lexi-Drugs database. Once a monograph is accessed from the search results panel, two things happen: 1 The monograph is displayed in the Content Panel on the right, and 1. 2 A new dynamic field navigation list replaces the search results on the left side. 2. Within the monograph you can still use the Jump To Field (Select Field Name) drop-down, which provides an alphabetical list of all the fields, or you can use the display on the left, which lists the fields in the order in which they appear in the monograph. Field “nesting” is incorporated into the linking to information in the AHFS databases, as illustrated by the right-pointing arrows ( ). This provides an opportunity for more focused navigation to the exact information you need. Note that while we only have links to Essentials from specific fields within Lexi-Drugs, once in Essentials you have access to the entire monograph. Click on a Reference link and a new window appears with all the references that pertain to that particular statement, or group of statements, within the monograph. These indicate that the use being described is unlabeled/investigational. Clicking on the dagger link takes you to the end of the monograph where an explanation is provided. To return to your original Search Results, with the listing of options in each database, click on the Search button again. Also, when searching for drug information, you also have the option to access the two AHFS databases directly. Drug Interactions - provides a list of drug and natural product interactions, displayed alphabetically with Significance Level, to alert you to a specific interaction quickly. Patient Advisory Leaflets - Adult-PALS and Pediatric-PALS are our medication Patient Advisory Leaflets, covering over 1200 adult and over 600 pediatric medications. They are written in a concise question-and-answer format and are available in up to 18 languages. These leaflets are accessible through a drug monograph or can be accessed using our Patient Care Module. * (Standard offering in all Hospital subscriptions, optional for all other subscribers. Please call or email to request information) Drug Images within a Drug Monograph Clicking on the Images button will display a list of photographs of the brand and generic versions of the medication (oral solid dosage forms only). Clicking on an individual image will provide a larger view and detailed information about the medication. Interactions Content Within a Drug Monograph Clicking on the Interactions button will display a complete listing of known interactions for that particular drug along with the risk rating for each. Full interaction information, including an 8-field interaction monograph can be viewed by clicking on the drug names listed. For further information visit www.lexi.com LEXI COMP Lexi-Comp ONLINE User Guide TM ONLINE Lexi-Interact™ Lexi-Interact™ is a complete drug and herbal interaction analysis program and will open in its own window after the user clicks on the Interactions button. Start by entering a patient-specific profile in the Lookup window. 1 Enter each generic or brand name separately. Either the product name will appear, or options will be listed. Select the product that fits best and it will appear in the list 2 . Click the Analyze button to perform the interaction analysis. The Analysis Panel 3 provides a summary of interactions and the respective assigned risk rating (A, B, C, D, or X). Each letter represents a different level of urgency in responding to the identified interactions. A detailed description of the Risk Rating is available through the link at the bottom of the Interations Analysis summary page. Here you can also adjust the level at which the analysis is performed. Click on the Hot Link term to review a detailed Interaction Monograph to obtain information on: Patient Management Interacting Members, Class Exceptions Risk Rating, Severity, Reliability Rating References, and more... 1 3 2 Example: You may enter a series of drugs/herbal products from a patient’s profile and click “Analyze” to review an interactions analysis. To review a drug interaction monograph, click on the interaction link in the Analysis results list and an 8-field monograph will be displayed. For further information visit www.lexi.com LEXI COMP Lexi-Comp ONLINE User Guide TM ONLINE Lexi-Drug ID™ Lexi-Drug ID is an application that allows identification of tablets, capsules, and other oral solid dosage forms. It will open in its own window after the user clicks on the Drug ID button. Start by entering the known product characteristics: Imprint Shape Dosage Form Colors If some characteristics are not known, either select the default mode or leave that part of the entry blank. By clicking on the Search button at the top, the results are displayed in the Content Window on the right. Results can be sorted by various criteria. Links are also available for more detail about the product. Note: Click on an individual image to see a larger view and detailed information about the image. Example: Enter “20” in the Imprint Window, click on ‘Capsule’ for Dosage Form and Shape, and “purple” for color. After clicking on SEARCH, the results will be displayed in the content window. In this case there were 12 matches. Web Search Web Search is a tool that allows the Lexi-Comp ONLINE user to expand a search to other medically-based web sites. After a term is entered in the Search window, click on the Web Search button and the health search portal interrogates multiple healthcare sites. The results are displayed in a separate window and allow access to the content directly within various resources, plus additional links to abstracts and primary literature. 1. Ensure desired search term is in the Search window. 2. Click on the Web Search button and a new window with search results is displayed (see example below). 3. Results are organized by resource and detailed category based on search term. 4. Links are available to numerous web sites including those that offer abstracts of primary literature. 5. Links are pre-selected based on search term (e.g. they are different for drugs, diseases, and procedures) 6. Once in the Web Search window, additional searches may be conducted by typing a new term in the Search box A . A Example: To search for information regarding Diabetes beyond the Lexi-Comp ONLINE databases, click on Web Search. After the results are displayed, click on any of these links to access more detail. For example, if you are interested in reviewing abstracts within PubMed, click on the PubMed Clinical Advanced link and a list of abstracts will be displayed. From here, if your institution has a subscription to the specific journal in which the article was published, a link to the article can be created. For further information visit www.lexi.com LEXI COMP Lexi-Comp ONLINE User Guide TM TM ONLINE Lexi-CALC ™ Lexi-CALC is a comprehensive medical calculations application covering infusions, adult and pediatric organ function assessment, and other conversions. It will open in its own window after the user clicks on the Calculations button. Once open, a list of the available calculations is included in the Search Results Panel on the left side. LEFT RIGHT Features and Functionality The functionality within the Lexi-CALC application is designed for ease-of-use, and to be intuitive for healthcare providers. As with Lexi-Interact and Lexi-Drug ID, the display screen is separated into two panels. The LEFT panel provides the list of available calculations. Select the desired calculation by searching for keywords in the 'Lookup'* window, by filtering through the calculator Categories, or by scrolling. Click on the selected calculation and it appears in the RIGHT panel of your screen. 1 and 2 3 Select units, other variables, and method, where appropriate 1 , enter the requested patient values 2 , and click the 'Calculate' button 3 . You must select units prior to entering values if changing from default units. Example: Select Creatinine Clearance: Adults and enter the following information: Method: Cockcroft-Gault; Gender: Male; Age: 66; Weight: 75kg; SrCr: 1.5mg/dl Then click the Calculate button, and a result of 51.39 ml/min will be displayed. * Note on ‘Look-up Function’: Displays calculations that match any keywords provided, e.g creatinine. The category list then disappears and a “Show All Calculations” button appears. When an input is restricted, click on the input name button SrCr to display the accepted range. This example shows the window that describes the acceptable range for serum creatinine when the SrCr button is clicked. A warning will also appear in red text when the Enter key is pressed. When a number has been entered, a conversion is performed when you switch units. While performing a calculation, additional warnings and alerts may be displayed in Red below the 'Reset' button. To enhance 'ease of use', each calculation also offers ‘help’ information containing such items as: formulas, warnings, references and guidelines for interpreting results. Compatibility Lexi-I.V. Compatibility is a partnership between King Guide® Publications and Lexi-Comp that offers information on stability and compatibility for over 400 drugs and solutions. This is a standard offering for institutional clients and is optional for other Lexi-Comp ONLINE Clients. It will open in its own window after the user clicks on the I.V. Compatibility button. The link takes the user to the King Guide to Parenteral Admixtures home page. Enter a drug name into the search box (“List Drugs Starting with: _____”), or scroll through the numeric/alphabetical drug list and click on a medication. Each drug monograph provides a description, stability information, and a list of drugs reported to have been evaluated in combination with the first drug. In addition, symbols indicating compatibility of the combination in twelve separate fluids are included in the table. A To review the compatibility and stability information for a combination, select a drug from the “2nd Drug” table listing A . For further information visit www.lexi.com Lexi-Comp ONLINE User Guide TM LEXI COMP TM ONLINE Lexi-Comp Patient Care Module Lexi-Comp's Patient Care Module provides you with an opportunity to access and print patient-focused leaflets for conditions, procedures, and medications. It will open in its own window after the user clicks on the Patient Care button. This section of the User Guide offers step-by-step instructions on: How to Search for Leaflets Packet Options How to Build a Packet Language Options How to Preview Your Packet Customizing and Printing Your Packet How to Store and Retrieve Packets The Patient Care Module includes: n Adult-PALS - Patient Advisory Leaflets covering over 1200 adult medications n Pediatric-PALS - Patient Advisory Leaflets covering over 600 pediatric medications written for the parent or guardian. Both Adult-PALS and Pediatric-PALS are available in up to 18 languages. n Health-PALS - Patient Advisory Leaflets covering over 900 conditions, procedures and tests; these are available in English and Spanish. There are also an additional 1700 topics that go beyond the fact sheets and discuss more in-depth strategies for specific Conditions and Procedures. A new window opens with the Patient Care Module application. The primary searching function takes place on the left side, where you have the Condition/ Procedure Lookup and the Medication Lookup. For further information visit www.lexi.com LEXI COMP Lexi-Comp ONLINE User Guide TM TM ONLINE Patient Care Module (continued) To get started, you may wish to check the boxes to include Patient Summary and Signature Page for your packet. You may also choose to include an English copy of foreign language leaflets. Finally, select the font size for printing : options are 8, 10, 12, or 16 point fonts. You may now begin to create your packet. Type a condition or procedure name into the Condition/Procedure Lookup window, and click on Find. If more than one option occurs in the database, all will be listed, and you will select the one that best fits your patient. Prior to searching for any leaflet, you may wish to select a different language than English. Make this determination prior to your search. Conditions and Procedure leaflets are available in English and Spanish. Click on the condition and it will be placed in the Current Contents list. Repeat this process for all conditions/procedures for which a leaflet will be printed. For some conditions and procedures, Related Leaflets will also be listed as options. In this case, for 'stroke', other options are included. Click on any, or CtrlClick on more than one to add to your Current Contents list, then click on DONE at the bottom to add additional leaflets to your packet. The Related Leaflets function identifies complementary content based on the search term. For example, the system searches for other Procedure and Condition leaflets, based on the original condition or procedure term used for the original search. Once selected, these will be listed in the Current Contents section: Procedure Leaflets The last category of leaflets you can create is for medications. Here you have more options to customize the documents, as you can select from up to 18 languages (remember: you can include an English copy with all foreign language leaflets through the check box in the Packet Options section). Additionally, you can select either Adult or Pediatric (for parents of children 12 or younger) versions. There are up to 18 language options for Medications. Health-PALS (conditions) are currently available in English and Spanish. You can also select the “Level” of medication leaflet you wish to print and give to the patient. Options are Adult (the default) and Pediatric (for parents or caregivers of patients <12 years old). Type a drug name in the Medication Lookup box, and if more than one result is in the database, a pick-list is displayed: Select the appropriate medication from this list and repeat this process until your patient's entire profile is created. You can enter either brand or generic drug names. Navigation in the leaflet is via the scrollbar on the right, or more commonly by using the Jump To Field (Select Field Name) window at the top. Note: this quick leaflet preview is to allow the healthcare provider to review the contents of the leaflet prior to packet creation. Once all the Condition, Procedure, and Medication Leaflets are selected, in the appropriate language and at the appropriate level, you are now ready to create your packet. Click the Preview button at the bottom of the left panel to create the patient packet. At the top are a number of options to customize, the packet with the Hospital/Institution name, Patient Name, Notes and various document formatting preferences. Hovering your mouse over the Edit options and clicking on them provides the opportunity to add text: Note: Lexi-Comp allows for a maximum of two (2) images to appear on your packets. The current Lexi-PALS logo may be substituted by your hospital logo. Please contact your Lexi-Comp account representative for details on substituting logos. For each of the PALs there is an option to include notes, and these notes can be customized for font size, features (text color, fill color), and other formatting to make them more appealing to the patient.
