Department of
Pediatric Dentistry
A specialty dealing with the oral care of children, infancy to adolescence,
including children with special needs.
A
Dentist whose specialty deals with oral care of children from infancy to
adolescence, including children with special needs is called a Pediatric
Dentist. Today parents are advised to start bringing their children in to see the
Pediatric Dentist for semi-annual check-ups when the child is 12 months old.
The experience of `going to the dentist' today is a very pleasant one in which
your child is slowly introduced to personal dental care. Children are shown how
to brush their teeth and are treated with special care and attention. Our pediatric
staff is trained to work with children of all ages.
During early childhood, the dentist
is concerned about baby bottle
tooth decay, which may cause
pain and suffering in small
children. It is a dental problem
which can destroy the teeth of
infants and small children and
have lifetime consequences.
Our Pediatric Dentists
Lisa Gilkes DDS
Yu-Hsiung Wang DDS, PHD
4
Baby bottle tooth decay occurs when the liquid is exposed to a child’s teeth for
a long period of time. For example when a baby takes a bottle to bed, milk,
formula, juice, and other liquids may cause tooth decay. The child’s
“baby teeth” can be damaged. This can lead to wide spread decay,
cavities, difficulty eating, delayed speech, development problems,
infection, poor eating habits, gingivitis, bone structure damage,
and general health problems.
When you begin your child’s dental care with early
childhood dental checkups you help prevent these
problems. It is important to check your children’s teeth
for brown spots. We recommend you clean your child’s
teeth with a clean gauze pad or wash cloth after
feedings, and don’t allow the baby to fall asleep with
the bottle in his/her mouth. In addition we suggest
you never dip a pacifier in sugar or honey. Teach
your child to use a cup at about six months of age
so bottle feeding can be eliminated. Please make sure
your child receives some form of fluoride.
Fluoride is a natural mineral that hardens and
protects teeth making them more resistant to decay.
Fluoride also significantly reduces cavities in children and
adolescents. Some people get fluoride in toothpaste. Others
get it from treatments they get at their dentist.
Tooth decay is the most common dental problem
amongst children. Dental sealants may reduce tooth decay.
Sealants are a thin plastic coating applied to the chewing
surface on the back teeth as a way of helping prevent
tooth decay.
Sealants, along with a healthy diet, and proper dental
checkups may help reduce the likelihood of tooth
decay in children.
As your children enter their
teen years, our pediatric dentist
will advise you as to whether they
might need braces to straighten
crooked teeth or help fill in a
gap or space between teeth.

B
races can be considered as treatment for
both adults and children. They are used
to straighten teeth, which may also help avoid
tooth decay, prevent gum disease, and potential
tooth loss.
Some people also get braces to correct their
bite or malalignment of their jaw. Correcting
these problems help with chewing, speech, and
reduces the stress and daily wear on your teeth,
supporting bone, and jaw joints.
All orthodontic treatment includes diagnosis
and planning. For both children and adults there
are various types of braces. Cost varies depending
on the complexity of the treatment.
A dental specialist, called an orthodontist will
treat you at Columbia Dental.
Wearing braces will later help you have
a healthy smile. Length of treatment
varies. We offer patients a free
evaluation and are glad to explain
the various types of braces
available today.
Department of
Endodontics
A specialty dealing with root canal therapy.
I
Our Endodontist
Keivan Zoufan DDS, MS
t's not uncommon for your dentist
to say you need a `Root Canal.'
This is the field of Endodontics. The
procedure really is about saving your
tooth or teeth, rather than removing
it/them. Root canal therapy treats
disorders in the nerve or pulp of the
tooth, especially when the nerves and
the blood supply to the tooth is
diseased or damaged.
Unfortunately, a damaged or
diseased tooth will not heal by itself.
Infection can spread, bone around
the tooth can degenerate, and the
tooth could fall out.
A dental specialist who specializes
in the treatment of root canals is
called an `Endodontist.' After
examining the tooth, he or she will
determine if a Root Canal is needed. If treatment is needed a series of
appointments will be scheduled:
First, the diseased pulp is removed, while leaving the canal intact.
The canal is rinsed out and medication is placed inside it. A temporary
crown is placed on the tooth.
Sometimes root canal treatment can be completed in one visit.
If a second appointment is needed the canal is again sterilized and filled
with a permanent material.
Please be assured our Endodontist will administer medication to
insure that the area is numb so you won't feel pain during the procedure.
Sometimes, the gum tissue around the tooth may be sensitive after the
procedure, and over-the-counter medication is usually suggested to help
eliminate any discomfort.
Later you may need to have a permanent crown placed on the treated
tooth. A follow-up exam helps monitor the tissues healing. Interestingly,
following a root canal, your tooth is not dead! It receives a source of blood
and the nerve supply from surrounding tissue. The tooth will no longer
respond painfully to hot or cold and will be functional. With proper care
the tooth needing a Root Canal will now last as long as your other teeth.

Department of
Oral & Maxillofacial Surgery
A specialty dealing but not limited to dental or jaw surgery,
removal of wisdom teeth, and cosmetic surgery.
O
ral and maxillofacial surgeons are the surgical specialists in the dental
profession whose experience qualifies them to diagnose, treat and
manage conditions, defects, injuries, and the aesthetic aspects of the mouth,
teeth, jaws, and face.
After four (4) years of dental
school, oral and maxillofacial
surgeons complete four to
six (4-6) more years of hospitalbased surgical residency training.
Their residency includes internal
medicine, general surgery,
anesthesiology, otolaryngology,
plastic surgery, emergency
medicine, and other medical
specialty areas.
Some surgeons also earn
medical or advanced degrees.
Others often complete fellowships
in sub-specialty areas.
Oral Surgery & Anesthesia
This incorporates extensive
anesthesia training that enables
the oral and maxillofacial
surgeon to perform a wide
variety of procedures in both our office or if necessary a hospital setting.
They're able to administer local anesthesia, nitrous oxide, intravenous sedation
or anesthesia safely meeting all medical requirements.

Maxillofacial Surgeons

Dentoalveolar Surgery
Removal of diseased and impacted teeth is commonly done by the oral
and maxillofacial surgeon. Tooth extractions may be warranted because of
crowding, decay, gum disease, or bad positioning.
Often these services are provided with the use of adjunctive intravenous
sedation.
Reconstructive Surgery & Implants
Injury or trauma, tumors, long term denture wear, can be among the reasons
patients have inadequate bone structure in their upper or lower jaw. Using
bone grafts, the oral and maxillofacial surgeon can improve both the quality
and quantity of the hard tissue. Skin grafts and soft tissue corrections can be
utilized to improve the soft tissues in the mouth. Oral reconstructive surgery
is a solid foundation for dental rehabilitation which in turn aids nutrition and
speech. Oral surgeons use dental implants to replace lost teeth and other structures.
Implants are also used to anchor both oral and facial prostheses.
Facial Trauma—Types of Surgery
Oral and Maxillofacial surgeons are experts in treating facial injuries, fractures of the upper and lower jaw
repairing facial lacerations, and damage to teeth and supporting structures.
At Columbia Dental an oral and maxillofacial
surgeon will help you avoid more time consuming and
costly hospital emergency room visits. Using safe and
effective sedation, prompt, comfortable, and successful
treatment of your dental injuries will be performed.
Surgery
Oral and maxillofacial surgeons understand the harmony
between facial appearance and function. They perform
corrective jaw surgery to produce a balanced functioning
skeletal relationship for the patient. Skeletal differences
between the upper and lower jaw can cause problems psychologically, in chewing,
swallowing, speech, or TMJ. Restoring the structure will also help it to function normally.
TMJ—Temporomandibular Joint
Oral and maxillofacial surgeons are trained to treat Temporomandibular Joint
problems which occur in the area where the jaw and the skull meet. TMJ is the ball
and socket joint that enables the jaw (mandible) to move and function.
People suffering from TMJ problems often complain of earaches, neckaches and a
limited range of jaw movements. They sometimes report clicking or grinding noises in
the joint, pain when opening their mouths and headaches.
Non-surgical management is the first type of treatment. Some patients require
minimally invasive jaw surgery.
Wisdom Teeth
People generally have 32 teeth by the age of 18. Wisdom teeth are the last to come
in and usually appear during the teen years.
When they have no place to grow and become impacted they need to be removed.
This generally occurs around age 16. Removal of your wisdom teeth is recommended,
since the roots are not fully formed and bones surrounding the impacted tooth is less
rigid. They are usually removed by an oral surgeon.
Since the teen years are considered the age of wisdom... these teeth became
known as ™wisdom teeth!∫ Sometimes wisdom teeth aren't visible, but can cause
problems including pain in the back of the jaw or infection. When bacteria in the
mouth gets under the tissue it causes painful infection, swelling, stiffness, and illness.

Bridges
When a tooth is missing, other teeth can shift out of place. This can change
your bite or make your jaw sore. Your teeth can become harder to clean and
gum disease could become a problem. Shifting teeth could
even change your smile!
A Fixed Bridge is when one or more replacement teeth are attached to the natural teeth next
to them. Once in place, it's not meant to be
removed.
It takes two or more dental visits to prepare
and fit your bridge. For a conventional bridge
the teeth on either side of the space to be filled
must be reshaped. These reshaped teeth hold
crowns that support the bridge. Impressions
are taken. You may wear a temporary bridge
while your permanent bridge is being shaped
and made.
Your new bridge is adjusted until it fits
right and feels good. Then it's cemented or
bonded into place. It's important to brush after
each meal using an interdental brush to clean
between large spaces. Flossing at least once
a day is also recommended. It is advised you visit the dentist twice a year for cleanings and checkups.
If you have a problem with your bridge, never try to fix it yourself. Call and
make an appointment with Columbia Dental’s prosthodontist right away!
Crowns
When do you need a crown? You need a crown when your tooth is fractured, an
old crown or filling has failed, had root canal therapy, have an old cavity, or you
would like to improve a tooth’s appearance.
What do Crowns do?
Crowns restore and protect teeth. They are a custom made
restoration that completely cover your tooth. They not only
repair and straighten a damaged tooth, they also can be
used to improve its appearance by covering a tooth
that’s misshapen or discolored.
What about the procedure to get a crown?
The procedure usually requires at least two office
visits. We’ll prepare and shave the tooth, take
impressions, and our own in-house dental lab will
make a model from the impressions to create your
new crown. It’s usually made from porcelain.
Our Prosthodontist personally measures and
fits your crown. At Columbia Dental you are given
personal attention by our dentist and dental specialists.
They work as a team to better serve you.

Department of
Periodontics
Periodontics is a specialty dealing with gum diseases
and the prevention of tooth loss.
P
Our Periodontist
Michael Isreal DDS
12
eriodontal Disease is a term used to describe various
diseases that affect the gums, bone and surrounding
structures of your teeth. The 2001 Guinness Book of World
Records recorded `Periodontal Disease as the number
one (#1) disease which affects mankind.'
The most common adult periodontal diseases are gingivitis
and periodontitis. Gingivitis causes the reddening and bleeding
of your gums. Periodontitis damages the bone and connecting
tissues that support your teeth.
Plaque and tartar accumulating and covering your teeth often
signify the beginning stage of periodontal disease. Tartar is long
standing plaque that has absorbed calcium on the tooth's surface.
Plaque is a paste like substance that occurs when bacteria increases en
mass on a tooth or teeth.
At Columbia Dental we have a wonderful Periodontist, an expert
specialist who received special education and experience caring for patients
with gum disease.
The risk factors for getting periodontal/gum diseases include: poor dental
hygiene, poorly contoured fillings and crowns, poor nutrition, smoking,
chewing tobacco, high levels of stress, Downs' Syndrome, Diabetes, AIDS,
taking certain medications such as steroids, contraceptives, and blood pressure
medications. Interestingly, children of patients with periodontitis are more
likely to have the bacteria which could cause them to develop gum disease.
Columbia Dental wants to bring to your attention that the existence of
bone destruction under the gums is not visually detected. That explains why
regular semi-annual dental check-ups are very important.
Some of the warning signs you may have periodontal/gum problems
include: gums that bleed, gums that are red, puffy, painful, or tender, or gums
that feel itchy. Other warning signs include loosening or shifting of your
teeth, constant bad breath or a bad taste in your mouth, changes in your bite,
changes in the fit of your dentures or bridges, and gums that are receding
(gums which are pulling away from your teeth). Now people may realize if left
untreated periodontal/gum disease can lead to loss of teeth. Yet, interestingly
now medical research indicates periodontal disease statistically correlates to:
• diabetes
• respiratory diseases
• heart attack
• strokes
• and premature childbirth
Columbia Dental has a Periodontist on staff. His treatment
includes an extensive evaluation using a periodontal probe
to measure the depth of space between the teeth and gum.
X-Rays are also taken to see if there is bone damage.
Treatment varies depending on the progression of your
periodontal disease.
13
Department of
Implantology
An implant is the premier choice for replacing missing teeth. It’s a natural looking,
permanent restoration.
Columbia Dental has
exciting news for you.
We are one of the
few dental centers
in the United States
capable of placing
dental implants and
permanent teeth
all in one day.
T
oday, replacing natural teeth with dental implants is a common choice.
Implants replace the roots of missing teeth. An implant-retained restoration will be created and placed on the dental implants to rebuild your teeth
and have them look as natural as your own.
They are secured in place by tiny anchors. Only you and your dentist will
know are there. Dental implants help you chew better and look better.
Dental implants help minimize the amount of bone loss in the area of
missing teeth. Replacing missing teeth also aids your nutrition. Proper
chewing helps with digestion and your overall health.
We recommend visiting our dentist for an evaluation to see if you are a
candidate for implants. An x-ray will help us determine your eligibility.

A Traditional 3-Unit Bridge or Dental Implants?
Bridges use the teeth on either side of your missing tooth to provide support for
the artificial tooth that fills in the space where the tooth is missing. Bridges restore
your ability to chew and also improve esthetic function.
Dental implants on the other hand provide a more advanced replacement to
your missing teeth. They are small metal screws designed specifically to replace
your natural tooth root. They are manufactured from titanium providing a highly
biocompatible surface, encouraging the bone to attach to it during the healing
process to ensure long term results.
Look and Feel Natural
Everyone deserves a beautiful smile. With the tremendous advances in esthetic
dentistry, there's no reason to put up with anything less. With dental implants,
crowns and bridges, missing teeth can now be easily, comfortably, and
permanently replaced.
Besides functioning properly, the ideal replacement must look like a natural
tooth, both to you and others. The solutions possible at Columbia Dental. Take
advantage of the best esthetic advances possible in dentistry. Implant supported
crowns and bridges look amazing and work just like your original teeth; they
are nearly indistinguishable from your real teeth.

olumbia Dental proudly announces having an on site dental laboratory where our certified dental technicians work closely with our clinicians in
order to provide you with the highest quality dental prosthetic.
Most dentists or other specialty offices take impressions and then send the
impressions to labs out of state. It can take a week or more to have the product
made. Then it is mailed back to the dentist. This adds more time to the process of
your receiving the product.
Columbia Dental is one of the very few dental practices in the northeast to
have an In-House lab with experienced trained lab technicians prepared to help
you with this type of a problem especially during an emergency!

Department of
Radiology
Radiology is a diagnostic tool enabling dental professionals to view the underlying
tissue that is not visible to the naked eye.
C
olumbia Dental is pleased to have an i-CAT scan for 3-D data.
Our Certified i-CAT
Radiology Technician
Carlos Buitrago DDS,
18
Did you know the i-CAT offers a 3 dimensional dental scan? It’s
convenient, fast, with low dose radiation, and is done in an open environment.
Quite simply, it provides the dentist and patient with dramatic advances
for dental care. Virtual treatment planning is easy because the i-CAT combines
high-tech-in-office scanning with three-dimensional digital reconstruction of a
patient’s anatomy.
The 3-D anatomically accurate imaging allows the dentist to develop
the most concise treatment plan for a positive and successful outcome.
It provides 3-D data for: implant planning, third molar extractions,
impactions, TMJ, supernumeraries, airway assessments, panoramic,
and cephalometrics.
The patient benefits because
it’s only a 20 second scan that
provides 3-D data for improved
diagnosis. It’s also much lower in
cost than a medical CT scan.
Because it’s done in our office,
there’s no need for a hospital
appointment. Doctors easily share
the 3-D data using i-CAT vision
viewing software to discuss your
case with their colleagues or
other doctors.
Columbia Dental’s therefore
able to provide you with an
improved diagnosis and treatment plan thanks to i-CAT.
This is very important in the
advanced treatment of dental
problems and procedures.
Our office also offers low
dosage computerized x-rays. They
are taken only as necessary and as
recommended by the American
Dental Association. Most insurance
benefits cover their cost. Thanks to
new digital radiography, they are
also safer, better, and faster than
those taken in previous years.
X-rays are an important role in the diagnosis of dental problems. Some people
still express concerns about the radiation involved. Interestingly, Columbia
Dental switched to digital radiography. It provides the accurate images needed
to treat you properly while using 85% less radiation than regular x-rays.
The images are also instantaneous. There’s no film to process or mount. No
dark rooms are used (like in the past). All images taken lead to a better diagnosis
of your dental problems. The system we use is also able to magnify
the image, manipulate the color contrast and even discover other hidden
dental problems.
Since there are no harsh chemicals used, patients need not worry about
hazardous waste. Today we’re now able to take x-rays using less radiation and
safer dentistry.
