On motion of Mr. Templet, seconded by Ms. Lee-Sheng, the following resolution
was offered:
RESOLUTION NO. 126654
A resolution authorizing the Department of Risk
Management to renew the subscription with Insurance
Services Office, Inc. to the Marine Index Bureau
segment of the ISO Claim Search at a cost not to
exceed $3,500.00 per year for a period of up to three (3)
years. (Parishwide)
WHEREAS, a need exists to determine the past claims history of persons
bringing claims against the Parish of Jefferson; and
WHEREAS, the Insurance Services Office, Inc. Marine Index Bureau
segment of the ISO Claim Search maintains a claims history database and provides
an information service, which will assist significantly in the claim evaluation process.
NOW, THEREFORE, BE IT RESOLVED by the Jefferson Parish Council of
Jefferson Parish, Louisiana acting as governing authority of said Parish:
SECTION 1. That the Department of Risk management is hereby authorized
to renew the subscription with Insurance Services Office, Inc. to the Marine Index
Bureau segment of the ISO ClaimSearch at a cost not to exceed $3,500.00 per year
for a period of up to three (3) years.
SECTION 2. That the cost of the Marine Index Bureau, Inc. services be
charged to Account Number 66760-5009-7341.6 (67696.004) in the amount not to
exceed $3,500.00 per year.
SECTION 3. That the Department of Risk Management be authorized to
renew the subscription for the additional two years if the cost does not increase by
more that 15% (fifteen percent) per year.
SECTION 4. That the Chairwoman, or in her absence, the Vice Chairman, is
hereby authorized to sign any and all documents necessary to give full force and
effect to this resolution.
The foregoing resolution having been submitted to a vote, the vote thereon
was as follows:
