New post SECTION 1 JOB IDENTIFICATION Must be completed Job Title: Health Care Support Worker Department(s): Prison Health Care Location: HMP & YOI Grampian Job ID: RP6926 Grade: Band 3 Hours: 5 x 37.5 hours per week Salary: £16,271 - £19,268 per annum Closing Date: Wednesday 27th November 2013 1 SECTION 2 Job Purpose statement To actively participate in the delivery of patient care, as an unqualified nurse in the multi-disciplinary team within the Service Area. Health Care Manager Clinical Manager Senior Practitioner Nurse Practitioner Nurse Healthcare Assistant Please refer to appendix B(i) for definitions 1 Communication and relationship skills The post holder is required: To communicate in a professional manner at all times. The main purpose of communication is to relay information regarding patients conditions and other issues relating to patient care to appropriate team members. To be polite and courteous in all communications and work co-operatively with patients, relatives, senior nurses and other members of the multi-disciplinary team. This includes support services, secretarial, reception and 3rd sector colleagues. At times the post holder may require to assist with distressed, upset, angy or anxious patients, carers and relatives. On occasions the post holder may be required to deal with untoward incidents such as self harm, attempted suicide or aggressive behaviour. 2 2 Knowledge, training and experience The post holder required to: Be of a caring, friendly disposition, have a genuine commitment to working in health care. Have previous relevant healthcare experience desirable but not essential. Have a Level of theoretical knowledge through formal training or equivalent training to SVQ3 level. Have knowledgeable of physical/mental illness, cultures, actions and behaviours. This would be gained through in house training, short courses and/or SVQ’s. The following statutory/mandatory training will be required – Moving & Handling Basic Life Support Hand Hygiene E.KSF Fire Safety Awareness Personal Protection Training Child Protection ACT 2 CARE (SPS Suicide Risk Management system) Other relevant training e.g Awareness of basic legal and ethical issues e.g Mental Health (Care and Treatment) (Scotland) Act 2003, Criminal Procedure (Scotland) Act 1995 and Adults with Incapacity (Scotland) Act 2000. Understanding of various NHS Grampian Policies Dementia New to Forensic Bereavement and Loss Basic IT Skills 3 Analytical and judgemental skills The post holder will: Demonstrate delivery of a high standard of health care provision under the guidance and supervision of qualified nursing staff. Actively participates in Health Improvement initiatives. Demonstrates adherence to NHS Grampian/local policies and procedures. Demonstrates ability to perform specific clinical skills required within the department and prioritise which duties to respond to first. These skills may 3 include: Primary Health Care Mental Health Care Substance Misuse Medications Management Phlebotomy Dressings Urine Sampling Report on patients condition. 4 Planning and organisational skills The post holder will: Organise own day to day work tasks or activities, plans own work. Accurately records relevant information on appropriate charts. Actively delivers care to patients in custody in accordance with care plans. Respects the privacy and dignity of patients at all times. Completes appropriate documentation and adheres to Code of Conduct. Completes evidence of development, objectives and training on e.ksf 5 Physical Skills The post holder will: Be required to use and maintain equipment and moving and handling aids as used within specific service areas. Be required to use IT Systems. At times be expected to assist immobile patients. Be expected to adapt to the shift pattern required within the European Working Time Directive and may be required to work a variety of shifts. 6 Responsibilities for patient/client care The post holder will be Required to use their own initiative but will work under the supervision of a Registered Nurse. Required to report any clinical changes to relevant disciplines. 4 Delegated responsibility for the delivery of care to a certain number of patients in custody, this will be allocated by the nurse in charge who will have overall responsibility. May be required to provide personal care to patients in custody e.g assisting with feeding, appearance, and other activities of daily living. Required at times to assist in other areas of the service as delegated. Able to carry out programmes of care and treatment developed by others, which may entail making modifications to the care programme within prescribed parameters and reporting back on progress. Able to support individuals or group sessions within overall programmes of care/treatment. Unsupervised at times with patients in custody during observations, when post holder needs to proactively anticipate potential hostility, abuse or violence and initiate de-escalation techniques and/or alarmed response. 7 Responsibilities for policy and service development implementation The post holder will Work with all staff within the multi-disciplinary team and the senior management team to ensure a process of continuous service improvement is met through appropriate quality assurance mechanisms Demonstrates an understanding and adherence to local and national Health & Safety policies and procedures Demonstrates an understanding of Infection Prevention and Control and adhering to Policies. 8 Responsibilities for financial and physical resources The post holder will Work with all staff within the multi-disciplinary team and the senior management team to ensure continuous service improvement and the efficient and effective use of time and other resources. Demonstrate economy in the use of supplies 5 Demonstrate awareness of stock control systems and how they should be used. Participating in the management of stock/supplies and to have the ability to work within an allocated busget. Actively participates in the care and maintenance of stock/equipment. Undertakes general housekeeping of the clinical environment to ensure a welcoming atmosphere to all visiting the area. 9 Responsibilities for Human Resources The post holder will Demonstrate own duties to new or less experienced or Bank staff. Maintain responsibility for Own personal Development with guidance from qualified staff in line with Staff Governance Standards. 10 Responsibilities for information resources The post holder will Ensure all relevant documentation is completed in a timely manner. Report any incidents according to the Occurrence Recording System. 11 Responsibilities for research and development The post holder will 12 Demonstrate adaptability towards changes in practice through the introduction of evidence based practice. Freedom to act The post holder will Work with supervision close by and within well established procedures. Actively contributes to the provision of continuity of care for all patients. Maintain the highest standard of health care provision within a complex multidisciplinary environment. Adapt to a wide variety of situations as patients conditions can change rapidly and report this to nurse in charge. Actively deliver patient care under supervision in accordance with the care plan as prescribed taking patients choice into consideration. 6 Be guided by standard operating procedures, good practice and established precedents – senior staff available for reference and work may be checked on a sample/random basis Be responsible for the observation and management of patients during therapeutic activities. Initiate a dialogue with supporting agencies depending on severity of the situation and take lead role in violent incident management if required. 13 Physical effort The post holder May be required to drive. At times work within confined spaces, require moving and handling skills which should be kept up to date as agreed by NHS Grampian. The post holder may be exposed to situations where there is violence, aggression and a high risk to the well being and safety of patients in custody and staff. 14 Mental effort The post holder May be required to close Observe Patients following agreed policy. 15 Emotional effort The post holder May have to deal directly with challenging behaviour. On occasions the post holder may be required to deal with untoward incidents such as self harm, attempted suicide or aggressive behaviour. On occasions may frequent distressing or emotionally circumstances e.g Care of terminally ill patients deaths. 16 Working conditions The post holder will Occasionally be exposed to bodily fluids.
