Introduction
Welcome to the 5th Edition. The aim of this directory is to provide
you with information about services that are available to patients
who have been diagnosed with cancer and their family and carers.
There is a wealth of advice and support available in West Yorkshire
and this booklet aims to help you find what you need from reliable
sources.
The cancer information directory has been produced by Mid
Yorkshire Hospitals NHS Trust with advice and support from
the Cancer Partnership Group. We have tried to ensure that the
information in this directory is accurate at the time of going to press,
however we cannot guarantee its complete accuracy and we accept

Blue Badge Scheme
The Blue Badge scheme provides a range of parking concessions
for disabled people with severe mobility problems who have difficulty
using public transport. Some people may qualify for a blue badge
automatically, others may need to have an assessment first.
You may qualify if you:

• are receiving a War Pensioner’s Mobility Supplement
• are a registered blind person

Can provide financial help through their telephone advice service.
It is for people with cancer, their family and carers who need help
to access benefits and other kinds of financial support. Calls are
answered by experienced welfare rights advisors. They can check
exactly which benefits you are able to claim and help you to fill in
the necessary forms to make a claim.

You can apply for an exemption certificate by collecting form FP92A
from you GP surgery or hospital oncology clinic. The form must be
countersigned by your GP or hospital doctor.

Holiday Travel
You may find this section useful if you have cancer or have recently
had treatment and would like to travel. Difficulties can be avoided
by good planning. Below is a check list to help you plan your travel
arrangements.
• Are you fit to travel safely?
• Will you need a travel companion or helper to accompany you?
• Will you need any special equipment?
• Have you let the airline/ferry company/car hire company etc know about any special needs that you have?
• Do you need particular vaccinations for the area you are
travelling to? Remember to take any relevant certificates with you
• Have you got enough medicines/medical supplies for the whole time away?
• Carry all supplies as hand luggage - check with latest airport security information
• Make sure you have your passport and an additional form of proof of UK residence, such as a driver’s licence.
• Take your European Health Insurance Card (EHIC, this replaces E111’s which were no longer valid from January 2006) if you are visiting European Union countries.
• Take your travel insurance policy and certificate.
• If you are flying, book in early and ask for a seat with plenty
of leg room.
• Do you have a medical summary from your doctor to take with you?
• If you have a stoma, does your hotel have en suite facilities?
(It may be helpful to ask the place where you will be
staying whether they have facilities to dispose of

113 Bus Service
A free bus service takes patients and visitors between Dewsbury,
Pontefract and Pinderfields hospitals 7 days a week. The service
operates hourly between 8am and 8pm Monday-Friday. It operates
1-8pm on Saturdays, Sundays and Bank Holidays.
The service will only pick up / drop off at the hospitals.

Access Bus
This is a dial-a-ride local bus service providing door to door local
transport aimed at people who are unable to use conventional
bus services. The buses are all low floor, easy access minibuses,
they operate everyday between 9-5pm. The service is open to all
passengers and are aimed for people of limited mobility. Access
bus is a demand responsive local bus service offering trips to local
shops. Passengers must first register with Metro’s booking service.
A flat rate of £3 per journey applies. Passengers holding English
National Concessionary Fare passes travel free of charge.

Getting Involved
Mid Yorkshire Hospitals is keen to involve cancer patients, their
families and carers to improve the provision of cancer services.
You can get involved in a variety of ways:
The Cancer Partnership Group
This was established in 2003. It consists of current and former
patients, carers and family members as well as health professionals.
The role of the group is to influence cancer services to ensure
the user and carer perspective is integral to all aspects of cancer
service developments.
Meetings take place every two months. This is not a support group
or a complaints service but a forum for giving a voice to users of
cancer services to help plan and develop new services and make
real improvements to existing ones.
