2012 SUMMARY OF BENEFITS – HOURLY CONSULTANT EMPLOYEES A. Medical Option – CIGNA OAP You are eligible to enroll in CIGNA’s – Open Access Plan. Services cover at various benefit levels for office visits, diagnostic, in-patient and out-patient care, hospitalization and prescriptions. The premium is paid in full by you through pre-tax biweekly payroll deductions. Coverage Type Biweekly Employee Payroll Contributions Single $199.40 Employee + 1 dependent $442.67 Employee + 2 or more dependents $648.06 B. Dental Option – Delta Dental You are eligible to choose among 2 options: Basic option benefit is $500/yr maximum; Enhanced 1 benefits are $1,500/yr maximum. The premium is paid in full by you through pre-tax biweekly payroll deductions. Flexible Spending Account-TASC You are eligible to enroll in account wherein you can contribute on a pre-tax basis to a health care, dependent care or transportation expense account, and later submit proof of eligible out-of-pocket expenses for reimbursement. E. 401(k) Plan - Fidelity You can contribute up to 97% of your gross salary up to a max of $17,000 per year on a pre-tax basis. There is a Catch-Up provision of $5,500 for those aged 50+. Administrative charges are ~$6.75 per quarter. F. Employee & Project Referral Bonus Program Collabera offers a cash bonus to technical employees who refer candidates who are hired for technical positions at Collabera. The referral bonus is in part based upon the client & level of position. Your referral bonus can be from $300.00 up to $1,500.00. Submit resumes via email to your CRE (Consultant Relationship Executive), ERE (Employee Relations Executive) or to [email protected]. Your CRE or ERE will document your referral through CIS (Consultant Incentive System) to insure you get credit for your referral. Once your referral is hired and continues employment for a minimum period of 30 days, you will receive 50% of the bonus. After your referral completes 6 months of continuous employment, you will receive the balance. As part of the CIS Program, there are 3 tiers: Silver, Gold & Platinum. When you make multiple referrals, you will earn points in addition to receiving the cash bonus. On accumulating these points you progress into the next higher tier, which will earn you a higher number of points as well as a higher cash bonus. G. Technical Training and Educational Assistance Reimbursement Program Online technical courses available through Skillsoft at a significant discount! H. Affinity Federal Credit Union Membership Discounted rates on credit cards, auto and mortgage loans. Checking and savings accounts available. I. Direct Deposit Electronic direct deposit of your paycheck in to a bank or credit union of your choice. Health benefits are effective from date of hire and end last day worked.
