If a GP recommends a referral to a consultant, patients then have the choice to see a consultant privately. Patients should tell their GP that they have private health insurance or can self pay for their treatment under our fixed cost care scheme. Once this has been done, either the patient or GP can contact us to arrange the first out-patient appointment. Medical insurance If patients have a private medical insurance policy and wish to attend a Ramsay facility patients must check with their insurance company prior to attending to ensure the policy covers them for the condition in question, and to gain an authorisation code. The level of cover varies between different insurers and this should be checked directly. Insurance companies may also require a visit to the GP to obtain a referral letter. Our Fixed Cost Care scheme is designed for people who wish to fund their own private healthcare directly without the use of an insurance policy. Fixed Cost Care offers a range of packages to cover treatment paths, from initial consultation through to the operation and aftercare, in one fixed price quote. This gives patients the peace of mind of knowing everything they need is covered. Should patients need to stay in hospital longer than initially thought and quoted for we cover the cost and this will include any additional treatments and tests. It does not, however, include the initial outpatient consultation, outpatient tests and the cost of follow-up consultations. Who can benefit? Our treatments are aimed at people suffering from chronic back pain which does not seem to be resolving - or who have a diagnosed condition for which surgery or injections is recommended. We offer a wide range of treatments for back problems, from pain management through to complex spinal surgery. We have the specialist consultants and diagnostic equipment to pinpoint the cause of pain and recommend an appropriate intervention. Our care does not end when patients leave hospital after a procedure. In many cases, we will arrange physiotherapy as an important part of rehabilitation and patients may need to return for a follow up appointment. Weight loss surgery - also known as bariatric surgery - can help many people but it is not recommended for every one. It is a lifelong - and often life-changing decision. However there is increasing evidence that it can lead to health benefits including improvements or even reversal of some conditions, such as Type II diabetes. The National Institute for Clinical Excellence recommends patients should be considered for bariatric surgery if their body mass index is 40kg/m2 or more or if it is over 35kg/m2 with a significant disease which could be improved by weight loss (body mass is calculated by looking at weight and height). Patients must have tried non-surgical methods to lose weight without much success for at least six months. Patients must also receive intensive specialist weight management, be fit for anaesthesia and surgery, and commit to the need for long-term follow up. What treatments are available? Injections Injections to relieve pain and allow other therapies a better chance of working are an important weapon in the armoury of our consultants. Injections are carried out as day case procedures while under sedation, and patients are able to leave hospital within a few hours. While they don’t ‘cure’ a bad back, they should help patients get better. 5 Ramsay Health Care Facet joint injections These are normally carried out in either the cervical (neck) or lumbar (lower back) joints. Facet joints are situated between each vertebra; they help to stabilise the spine while allowing movement, however they can become painful if the joint becomes overloaded. This pain can make it hard to benefit from other treatments, such as physiotherapy. Nerve root blocks Surgery Persistent back pain can sometimes be due to irritation of the nerves in the spine. This can be caused by tissue adhesions and may be associated with former trauma or surgery to the spine. Injecting the nerve root with a mixture of local anaesthetic and anti-inflammatory called cortisone can be an effective way of reducing the pain in these cases. Surgery can sound frightening but it often has very good outcomes and does not involve a lengthy stay in hospital. Our consultants will be able to advise whether surgery is suitable for a condition and expected outcomes. What sort of weight loss procedures are available? All of these procedures are carried out by experienced consultants supported by a specialist team. Our care does not end with the operation. Patients will be supported by a multidisciplinary team of dieticians, clinical behaviourists and physiotherapists. We offer a structured year-long programme to help patients safely lose the maximum amount of weight and cope with any problems along the way. Information for Private & Self Pay Patients Back pain and spinal care »» Many overweight patients with diabetes find this improves after surgery and weight loss. Improvements in conditions such as high blood pressure and sleep problems. The majority of patients with nonalcoholic fatty liver disease who lost weight after surgery saw an improvement in their condition - or in some cases the condition disappeared. A reduction in chances of dying in the next 10 years from causes such as cancer, heart disease and diabetes. Sleeve gastronomy. Part of the stomach is removed, reducing the amount of food the patients can eat and affecting how the body feels hunger. Gastric bypass. Parts of the intestine are bypassed and the size of the stomach is reduced. This is a radical approach which reduces the amount of food eaten and means that less is absorbed from it. Gastric balloon. This does not involve surgery; instead a balloon is inserted into the stomach and inflated with saline. This is removed after six months but patients feel full and want to eat less. MRA Musculo skeletal Digital Mammography Symptomatic Screening (over 40’s) For plain films, appointments can be offered as early as the day of referral. For other procedures, depending on the preparation and speciality, an appointment will be offered within the next 5 days. GP’s can now refer directly for MRI, CT and Ultrasound. Appointments are normally offered within 3-5 days and reported back to the GP within 5 days. 9 Ramsay Health Care Increased Speed The scanner performs its scans in less time than before which means less time for the patients to hold their breath and also less chance of patient movement during the scans. MRCP Intravenous Urograms (IVU) The department also offers diagnostics for sports and back injuries New detector technology inside the machine means we can produce much crisper images than before and also look at details within the body as small as 0.6mm. This means subtle pathologies, such as small liver lesions, are now easier to detect than before. New Services created thanks to many of the advantages of the new scanner. CT Coronary Angiogram Direct Access Services Diagnosing the problem CT coronary angiograms are starting to become ever more popular amongst Cardiologists now that CT technology has moved forward. Our scanner has the latest technology, which allow us to create snapshots of the heart when they are still while keeping the doses extremely low. Pain management We are starting to provide pain management services in CT which include; CT Guided Nerve Root Injections, CT Guided Facet injections. The scanner has a second screen within the scanner room which allows our radiologists to perform the whole procedure without ever having to leave the room. The advantages we are seeing by using CT for pain management is that for the patient no sedation is required, only local anaesthetic. The images created can be as thin as 0.6mm which allows the radiologist to place the needle with extreme accuracy to ensure the best possible outcome. It is a weekly mobile service run in conjunction with InHealth. The mobile imaging scanner and is the latest in technology. PET CT has revolutionised many fields of medical diagnosis e.g. in oncology, surgical planning and cancer staging. The service supports the oncology practice at The Rivers and saves the patients travelling into central London. The Rivers has a dedicated PET CT Coordinator who liaises with the patients, the consultants and InHealth to ensure the referral pathway is as smooth and seamless as possible and the reports are back with the referring clinician within 24 hours. In March 2011, the Department of Health authorised The Rivers to be an accredited site for NHS PCT working with InHealth. The Rivers is the first private hospital in the UK to have this accreditation. It is used by over 130 Consultants covering 40 specialities. (These are detailed fully within this brochure) The department receives approximately 1000 patients per week, and performs approximately 300 procedures per week. Minor ops performed in our outpatient department include Cystoscopy, Colposcopy, Hysteroscopy and Vasectomies. We endeavour to offer a very high standard of customer care. Our customers include patients and consultants. All patients are treated as individuals and respect for their dignity is maintained at all times. Patient education is important and information leaflets are given as appropriate. Pathology specimens are tested by CPA accredited Laboratories with fast turnaround times; most results can be made available in 24-48 hours. Rivers Hospital 12 Physiotherapy and Sports Injury Rivers Pharmacy is registered as a retail Pharmacy business with the Royal Pharmaceutical Society of Great Britain and can therefore dispense all private prescriptions. Physiotherapy is a science based approach to maintain and restore physical social wellbeing. The exercise of clinical judgement and interpretation of treatment modalities are at its core. Modalities include electrotherapy, hydrotherapy, cryotherapy, manipulative and mobilizing techniques, deep tissue massage, and individually tailored exercise programmes. The department offers a general pharmaceutical service to in-patients, outpatient visitors and staff. Specialised Oncology Services are also provided through the newly upgraded in house chemotherapy reconstitution facility. The Pharmacy carry a limited range of “Over the Counter” medicines for purchase by visitors and staff. Specific requirements can often be obtained quickly through the pharmaceutical wholesalers. An “On Call” service is provided by a Pharmacist outside business hours for urgent advice and information. The Rivers Pharmacy department also plays host every year to Pharmacy students from Memorial University of Newfoundland, in St John’s, Canada. The students spend six weeks here and gain insight into healthcare practice in the independent sector at Rivers and also in the local NHS through ward rounds, an MDT meeting and outpatient clinics at Princess Alexandra and St Margaret’s Hospital, Epping. We work to Physiotherapy Chartered Society Standards supporting best practice and will strive to provide the best in patient care. The department contains 6 spacious treatment rooms and a large fully equipped gym. All physiotherapy patients have a treatment plan established on assessment by their physiotherapist, which is regularly reviewed during the course of physiotherapy treatment. We aim to see urgent referrals within 48 hours of referral and non urgent referrals within 5 working days. Referrals are accepted from consultants and GPs. Self paying patients may refer themselves for physiotherapy. Discharge reports are sent to the patient and their GP or consultant when the patient has completed treatment. Packages of care can be provided for patients with long term rehabilitation requirements. We also have links with a local Health Club which enables us to provide regular Hydrotherapy Sessions. Consultant anaesthetists are qualified doctors who have spent at least eight years in postgraduate specialist training, including pain management and intensive care. The anaesthetist helps to care for the patient's wellbeing during surgery and acts as a pre-operative physician from the immediate pre-operative period until full anaesthetic recovery which may mean 24 hours or more post-operatively. The surgeon will continue to oversee the patients care during this time. Theatre Rivers Hospital has four operating theatres with a team of dedicated and competent staff. All theatres have a laminar flow system which changes and filters the air in the operating theatres hundreds of times per minute. This makes the operating theatres ultra clean. To maintain this consistent high level a Consultant Microbiologist participates in the Hospital Infection Control Team and advises on all relevant issues. Microbiology reports are analysed and collated to determine any indicators of hospital acquired infection. There are various anaesthetic options available such as general, local and spinal anaesthesia. The surgeon and anaesthetist will work together to ensure patients receive the best anaesthetic option for the individual needs of the patient. The nurses caring for patients in theatre and the aard after the procedure will ensure they receive effective pain management and will liaise with the surgeon and anaesthetist as necessary. Orchard Ward which specialises in caring for Orthopaedics, General Surgery and Medical Patients and Meadow Ward which looks after ENT, Gynaecological, Urology, Cosmetic Surgery, Ophthalmology, Oral & Maxillo Facial and Chemotherapy Patients. The Rivers Hospital was listed in the Top 100 places for nurses to work in The Nursing Times Surveys in 2007 (14th place) and in 2009 (4th place). All patient rooms have en suite bathroom facilities. Patients are looked after by trained nurses supported by healthcare assistants. Rivers has close association with Anglia Ruskin University and Hertfordshire University and actively supports student nurse training. The Rivers Hospital has been highly commended by ARU for the training environment that we offer their students. All patients must be admitted under the care of a consultant. A Resident Medical Officer is available 24 hours a day for on-site medical support. We have a Matron, Senior Nurse Managers and Senior Nurse Specialists for Oncology /Chemotherapy, Cosmetic/Plastic surgery and Orthopaedics, Critical Care and Breast Care. Nationally the hospital acquired infection rate (all infections) is 5%. (National Audit Office), however at Rivers this figure is 0.2%. Cosmetic Surgery Although we very rarely have patients who develop MRSA in the Rivers, we do have patients that have been transferred from another hospital or nursing home who are positive for MRSA. These patients are isolated until they are free of infection. All patients undergoing major surgery are screened at our Pre-Admission clinic for MRSA. If a patient is transferred in from another hospital, the patient will be MRSA swabbed and barrier nursed until we have clear results. All of our staff are also trained to understand the importance of hand washing when dealing with patients. Urgent admissions policy Admissions for urgent cases can be arranged by the senior nurse on duty, who will seek the appropriate consultant who is willing to care for the Patient and will call the GP. Rivers will arrange a private ambulance if required. The cost of the ambulance (if not covered by the patient’s own insurance company) must be paid by the patient.
