Nottingham NHS Treatment Centre NHS referrer guide Issue 2 At CircleNottingham, we want to build a great company dedicated to our patients. We focus our efforts on this passionate cause and what we can become best at. Our credo commits us to, above all else, be ‘the agents of our patients’; we aim to exceed your patients’ expectations on every visit so that we earn their trust and loyalty and, through them, yours. We passionately believe in the caring side of healthcare and strive to continuously improve the quality and the value of care we provide for your patients. Our people are our greatest asset and we empower our people to do their best. Everyone who works at CircleNottingham matters, and our clinical unit structure gives everyone, from the most junior healthcare assistant to the senior clinician, an opportunity to contribute ideas to improve clinical outcomes and patient experience. Pursuing our ambition to be a great healthcare provider is a never-ending process. We embrace innovation and for Circle, ‘good enough’ never is. I hope you will find our referrer guide a useful reference document, and please do not hesitate to contact me by phone or email with any ideas you have to help us improve further. Sarah Pearce, Service Transformation Lead 0115 924 8449 [email protected] 2 0115 970 5800 Why refer your patient to CircleNottingham? Quite simply because we believe hospitals can be better. Why refer your patient to CircleNottingham? • One-stop clinics for many specialties with direct access to diagnostics and phlebotomy. • Short wait times and fast access to surgery. • Specialist on-site facilities, including Mohs, light therapy, photodynamic therapy (PDT) and bowel screening. • Convenient location with easy parking. • 99.4% of patients surveyed would recommend CircleNottingham to their family and friends, and we publish all patient feedback on our website.* • Zero cases of healthcare-acquired infections.** • Short Stay Unit with en-suite facilities. * Based on patient feedback submitted between January and December 2013. ** Based on all patients treated since opening in July 2008. Correct at time of print. Clinical excellence We are committed to providing clinical excellence and firmly believe those closest to the patients are best placed to make decisions. We have over 150 experienced NHS consultants, who are leaders in their specialist fields, working at our treatment centre. We embrace innovation and continuous learning, and measure everything we do. Our clinical outcomes data is available on our website for all to see. circlenottingham.co.uk 3 4 0115 970 5800 99.4% of patients would recommend CircleNottingham to friends and family* 50+ consultants in over 20 specialties CircleNottingham is committed to teaching and training, both internally and with our colleagues in primary care. We regard the training of the next generation of doctors and healthcare professionals as inseparable from our delivery of healthcare services. CircleNottingham is contracted to train undergraduate and postgraduate doctors, nurses, and operating department assistants. We are passionate about the caring side of healthcare delivery, the way in which patients are treated with compassion and kindness, in addition to the highest clinical standards of technical excellence and patient safety. The best patient experience We aim to exceed your patients’ expectations every time so, not only will they find a purpose-designed building, equipped with the latest technology, but perhaps more importantly, they will experience a warm welcome and a calming environment. We have long opening hours and often run theatres on a Saturday to provide short waiting times and fast access to surgery. Many of our patient pathways have been designed as one-stop clinics and we aim to undertake all the necessary tests and diagnostics on your patient’s initial visit. If your patient requires surgery, they may be pre-assessed on the same day as the decision to treat is made, and will often leave with a date for their operation. Our treatment centre is conveniently situated on the Queen’s Medical Centre Campus with good road connections situated at the junction of the Nottingham A5614 ring road and the A52. There is plenty of parking, and disabled parking is situated under our building. * Based on patient feedback submitted between January and December 2013. ** Based on all patients treated since opening in July 2008. Correct at time of print. Zero reported cases of hospital-acquired infections** A number of bus routes operate via the Queen’s Medical Centre Campus and, for patients living nearer to Nottingham City Hospital Campus, the Medilink buses run every 10 minutes during the day. We also offer a patient transport service for those who might otherwise have difficulty being treated at our treatment centre. Please contact 0115 970 5800 ext. 10014 to find out if your patient qualifies for this service. Facilities at CircleNottingham Our treatment centre has five operating theatres, three skin surgery theatres, an endoscopy suite and an on-site diagnostics imaging service, including MRI, CT, x-ray and ultrasound. We offer light therapy, PDT, Mohs, foam sclerotherapy, and a number of other specialist treatments which are not generally available at all NHS facilities. We also have an on-site pharmacy. We have 65 outpatient consulting rooms, 26 day case bays and 11 overnight beds. All of our theatres and treatment rooms are fitted with the latest technology and provide a purpose-designed platform for our clinicians to carry out surgery and outpatient procedures, minimising your patients’ time in hospital. For more information, please contact CircleNottingham on 0115 970 5800. Short Stay Unit Our Short Stay Unit consists of 11 overnight beds with en-suite facilities. The unit enables us to offer patients a wider range of treatments and surgery, and allows us to treat patients that previously did not meet our admission criteria. circlenottingham.co.uk 5 Circle Partnership Circle is an employee co-owned partnership with a social mission to make healthcare simpler, better and smarter value for patients. To enable this empowerment, our treatment centre is organised into nine clinical units, each led by a consultant, nurse, administration and management lead, working together as a team. On pages 8–17, you can see the names of each clinical lead and lead nurse, and a direct contact number for the gateway co-ordinator (lead administrator) responsible for that unit, so any queries or suggestions for improvement that you may have, we are here to help and answer. Referral criteria CircleNottingham welcomes all requests for consultation, as most patients can be treated as an outpatient or day case. However, unfortunately we cannot treat: • children under 14 years of age. • clinical emergencies. Generally speaking, for surgical patients we follow the ASA classification of anaesthetic risk: • ASA Grade 1: a normal healthy patient. • ASA Grade 2: a patient with mild systemic disease. • ASA Grade 3: a patient with severe systemic disease, and functional limitation. A rule of thumb is that patients who can climb two flights of stairs without shortness of breath or chest pain are classified as ASA 1, or ASA 2 if they have co-existing mild systemic disease. We will undertake surgery on a significant proportion of those patients who are ASA 3, but unless they are having minor surgery or procedures under local anaesthetic, they will need pre-assessment by one of our nursing team. The NHS constitution Patients have a right to: • start their consultant-led treatment within a maximum of 18 weeks from referral for non-urgent conditions; and • be seen by a cancer specialist within a maximum of two weeks from GP referral for urgent referrals where cancer is suspected. 6 0115 970 5800 Exceptions The right to treatment within 18 weeks from referral will cease to apply in circumstances where: • the patient chooses to wait longer. • delaying the start of their treatment is in their best clinical interests, eg. where smoking cessation or weight management is likely to improve the outcome of the treatment. • it is clinically appropriate for their condition to be actively monitored in secondary care without clinical intervention or diagnostic procedures at that stage. • they fail to attend appointments which they had chosen from a set of reasonable options, or the treatment is no longer necessary. Overseas visitors Most patients will not have to pay for their treatment. A person who is not ordinarily resident in this country at the time of treatment is not automatically entitled to NHS hospital treatment free of charge. Anaesthetic pre-assessment Once a patient has been listed by the surgeon as requiring an operation, he or she will be helped by one of our clinical staff to fill in our health screening questionnaire (HSQ). This is then reviewed by our pre-assessment nurse, and the patient may be called for blood tests, ECG or CXR following our anaesthetic protocol. Patients found to have no major problems on both the tests and HSQ go ahead for surgery. HSQ or tests revealing significant health problems are reviewed by an anaesthetist and either passed fit, or called for formal anaesthetic pre-assessment. Following this, they are either passed fit, or arrangements are made for the surgery to be undertaken at an alternative hospital, or postponed until further medical management is carried out (eg. cardiology/echo/pulmonary function tests etc.), or cancelled because the risks outweigh the benefits. opinion, the patient does not necessarily need to travel to the hospital. We believe that care should be delivered closer to home, at the convenience of the patient, not the other way round, and only if necessary, either for further tests or treatment will they need to visit the hospital.
