DETOUR HOUSE JOB TITLE: Drug and Alcohol Case Manager (Position dependent on funding availability) CONDITIONS: As per SCHCaDs Award Superannuation benefits REPORTING RELATIONSHIP: This position reports directly to the General Manager. Positions reporting to this position: Nil KEY RELATIONSHIPS: General Manager – Follow Policy and Procedure receive direction and guidance and ask questions. Colleagues – Follow Grievance Procedures: treat colleagues professionally and with respect. Provide feedback, clear guidance, hand over and provide assistance and support to colleagues as required. Clients – Follow Code of Ethics, provide case management JOB PURPOSE: Provide quality services through case management, education, mediation and support to women engaged in the program who are homeless and have been affected by alcohol and or other drug misuse. Resolve crisis’s and promote empowerment to these women to make healthy choices to improved quality of life and towards stable and secure housing. To ensure program activities are run as scheduled. DUTIES AND RESPONSIBILITIES: Case Management and Mediation Undertake initial assessment, referral, admission and orientation of clients and monitor their progress Inform clients of their rights and provide knowledge and skills to enable clients to access other services as required Identify the client’s presenting problems and underlying areas of concern, determine appropriate levels of support Enter all client information into the Client Information Management System (CIMS) Develop case management plans, review and monitor plans with clients Assist residents to organise and participate in recreational activities Provide support and educative information to the general public seeking information as appropriate Respond appropriately to clients in crisis situations Prepare client files, update progress notes and case management plans for each client using the Client Information Management System (CIMS) Provide telephone support to clients where appropriate 1 Admin/Staff/JobDescription/Detour House/Drug & Alcohol Case Manager-Updated April 2016 Follow up clients to monitor progress and provide further assistance as needed in accordance with program guidelines Provide outreach to client’s in the community. Facilitate the referral process of clients to suitable programs/agencies/specialist providers and assist in engagement of client where necessary Review after care programs with exit clients to determine appropriate support in accordance with program guidelines Ensure resident’s rent is up to date and recorded as per Detour House procedures Provide appropriate levels of support and case management to empower and enhance skills and agreed goals Comply with all Detour House Inc Policies and Procedures Inform, guide and enforce W H & S Standards ensuring a safe environment for clients and visitors on site Ensure client medications are locked in the Staff Office and follow prescribers directions when supervising clients Group Work Plan, devise and collate effective group material Prepare and facilitate groups to promote confidence and skills development to support lifestyle change, independent living skills and personal growth. Participate in planning weekly groups in response to client needs Respond appropriately to group needs. Respond to Group Evaluation and make changes according to Good Practice. Administration and Supervision Fill in incident and injury forms Prepare letters, complete statistics and other client related administrative duties Advocate on behalf of clients Collect and maintain data, enter all client information in the Client Information Management System (CIMS) and the NADA online data base. Answer phone and reply to messages, document calls in line with policy Assist in supervising students on placement and volunteer workers as required Undertake other administrative and office management tasks – eg petty cash, rent monies where applicable Adhere to Detour House Inc EEO and WH&S Standards Arrange medical appointments for random urinalysis Contact housing providers as per joint agreement during the tenancy of transitional housing clients Prepare for and actively participate in regular supervision sessions. Teamwork and Agency Development Provide peer support to and share expertise with other staff members and placement students Undertake additional appropriate tasks as required 2 Admin/Staff/JobDescription/Detour House/Drug & Alcohol Case Manager-Updated April 2016 Actively contribute to agency staff meetings, Quality Improvement Committee meetings, staff appraisals and training sessions Contribute to the development, implementation and evaluation of the agency service model and service delivery plan, policies and procedures Participate with peers in facilitated staff group support/supervision and debriefing sessions Participate in agency planning workshops and quality improvement meetings Maintain appropriate levels of skills and knowledge of drug and alcohol and homelessness issues and trends through regular training Liaise with other agencies as directed by the General Manager Represent the agency and advocate on the behalf of clients to the community Prepare reports for various agencies as necessary to support clients progress Inform visitors and students about the residential and transitional program Ensure smooth running of the daily program as guided by the General Manager Participate as on call when required.
