Title:
Director of Health System Pharmacy Services
Job Summary
Supervise and coordinate the delivery of pharmacy related services in the clinic, longterm care environment and hospital. Identification and provision of drug related needs for
patients served within the health system. Serve as the pharmacy department’s
representative to administration and medical staff.
Qualifications
1. Completion of Doctor of Pharmacy Program from an accredited university
2. Licensure as a pharmacist with the Minnesota Board of Pharmacy
3. Completion of an ASHP accredited residency program
Responsibilities
1. Director of Health System Pharmacy Services
 Responsible for all operations related to medication delivery or
distribution to patients and departments within the health system.
 Responsible for all employees within the pharmacy department and
assurance of completion of their job related duties.
 Direct contact with administration in order to allow for safe and
accurate delivery of pharmacy services within the system
 Coordination of medication access with other departments within the
system.
 Hiring and evaluation of department employees
 Evaluation and change of department as necessary
2. Clinic
Consult with patients/families/providers regarding drug therapy management
or monitoring. The clinic visit with patients includes the following
components:
 Obtain necessary patient history in order to provide safe and effective
medication management
 Perform focused physical examination
 Analyze patient information and laboratory data for all aspects of
medical therapy
 Analyze factors that interfere with safe/effective medication use
 Formulate plan of care obtaining additional lab studies, adjust
medication dosages, arrange follow-up visits, provide appropriate
patient education and provide intervention or referral for newly
identified problems within current scope of practice
 Discuss treatment plans with other health care providers, as necessary
 Provide education for patients and families about medication use

Provide self-management education and training in chronic disease
states
Provide appropriate phone follow-up, when necessary, on clinical
issues
Fully document all clinical services in patient chart
On-call for pharmacy department services as needed
Clinic Administrative
Supervise all PharmD providers, residents and students
Assure appropriate staffing of clinics and services
Work with clinic management to assure appropriate reimbursement for
clinical services
Evaluate current clinical services through QA/I
Investigate and implement additional services
Keep Scope of Practice up-to-date with current practice
Work with clinic nurse manager on medication stock and
nursing/pharmacy issues
3. Hospital- Clinical
Provide consultation in the area of drug therapy issues including:
 Review diagnoses, pertinent labs, vital signs, progress notes, ancillary
tests and medication regimen of each hospitalized patient
 Screen for drug related problems in all hospitalized patients
 Make specific recommendations to the admitting physician to resolve
drug related problems when identified
 Provide inpatient education on medication related issues as needed
 Complete “Pharm.D. Consults” as ordered by the physician. This may
include a more in-depth medical review of the patient, as well as
assisting the physician in deciding the therapy course for the patient.
Clinical areas of focus may include diabetes, anticoagulation,
hypertension, asthma, antibiotic selection, medication pharmacokinetic
monitoring by following blood levels, and parenteral nutrition
initiation and modification
 Develop and/or review drug protocols and perform periodic drug
utilization reviews
 Provide nursing in-services as needed
 On-call for pharmacy related services as needed
4. Committees Attendance
 Chair Hospital Pharmacy and Therapeutics Committee
 Lead therapeutic discussions of medication classes and medication
delivery policies
 Evaluate medication formulary on a yearly basis and make changes
to formulary as needed
 Evaluate standing orders and medication supplies in hospital and
long-term care environments
 Research topics requested for review by the Committee

5. Hospital- Dispensing
 Screen orders for indication, safety, effectiveness and dose.
 Supervise or perform accurate dispensing of the medication.
 Prepare chemotherapy and other intravenous and compounded
medications
 Supervise technical staff in their duties
 Assist with medication ordering and inventory
 Provide drug information to hospital staff
 Assure compliance with hospital formulary
 On-call for pharmacy related services as needed
6. Hospital- Administrative
 Act as Pharmacist-in-Charge per Minnesota Board of Pharmacy
requirements
 Assure licensure through the State of Minnesota and compliance with
all State and Federal laws, rules and statutes.
 Direct all aspects pharmacy dispensing as Director of Hospital
Pharmacy Dispensing and develop pharmacy policies and procedures
as such
 Assure compliance with DEA and other federal regulatory issues
 Evaluate pharmacy to evaluate pharmacy billing and reimbursement
issues compiled by administration and the hospital billing department
 Work with Medication Delivery Coordinator on stock, billing and
medication delivery issues.

Implement and keep systems up-to-date
Participate in planning of implementation of new systems
Be in contact with computer system administrators to assure
continuous access.
Work with administration to keep staffing levels appropriate to work
needs
7. Long Term Care- Dispensing
 Supervise technical staff in day-to-day operations when staffing
 Screen orders for indication, safety, effectiveness and dose.
 Prepare compounded and intravenous prescriptions when necessary
 Supervise or perform accurate dispensing of the medication.
 Supervise technical staff in their duties
 On-call for pharmacy related services as needed
 Consult with providers on therapy decisions on all nursing home
residents
 Complete “Pharm.D. Consults” as orders by nursing home providers.
Areas of consult may include diabetes, anticoagulation, hypertension,
polypharmacy, pharmacokinetic services, and psychotropic drug use
8. Patient and Provider Medication Education
 Provide education for patients, families, and providers regarding
disease state pathophysiology and treatment, medication risks,
benefits, and precautions.
 Provide medication information to medical staff as needed
9. University of MN College of Pharmacy Clinical Faculty
 Assist in all aspects of Pharm.D. resident education through the
University of Minnesota Pharmaceutical Care Residency Program
 Assure elements required for continued accreditation are done in
accordance with ASHP residency standards
 Recruit pharmacy residents as needed per administrative request
 Work with administrative staff of Colleges of Pharmacy on logistical
issues related to resident education and experiences
 Serve as preceptor to Pharm.D. residents and students. Supervise
residents and student activities and provide on-site education for
residents and students. Support student professional and patient
projects.
 Serve as guest lector at University of MN College of Pharmacy as
needed.
 Sit on University of Minnesota Committees and Panels, as necessary
 Assist with precepting Pharm.D. resident(s) through the University of
Minnesota Pharmaceutical Care Residency Program
10. Diabetes Self Management Team
 Educate patients with diabetes and their families in accordance with
national ADA standards.

Educate medical staff on ADA Standards of Medical Care and work to
increase compliance with those standards.
Attend the Diabetes Team Advisory Committee meetings four times
per year.
Lead Diabetes Team meetings as required
Assure compliance with the ADA Accreditation Standards and
perform periodic reapplication processes
11. Other duties
 Consult with home health and hospice nursing in drug therapy
monitoring as needed
 Participate with community outreach activities and represent PAHCS
at fairs, talks, and meetings
 Prepare formal educational materials and documents as they relate to
job aspects listed herein
 Review medical texts and literature as it relates to job aspects listed
herein
Physical Involvement














Must be able to sit, bend, turn, and maintain other related body postures frequently to
perform a majority of the job duties.
Must be able to sit and/or stand continuously for periods of 8 hours without a break.
Must be able to visually utilize computers and other office equipment, to properly
enter information, as well as read required documentation.
Must be able to visually inspect IV solution and unit dose cards.
Must be able to make precise visual inspections, as well as read detailed
specifications, instructions, and medication orders for extended periods of time
Must be able to lift/carry up to 20 pounds, occasionally, an average distance of from 5
to 20 feet,
Must be able to tolerate the frequent grasping and fine finger motions.
Must be able to perform repetitive hand and arm motions.
Must be able to read required documentation.
Must be able to prepare concise, accurate written documents.
Must be able to communicate clearly with all levels of management and employees.

Excellent communication skills are required for interactions with management, staff,
patients, residents, clients, etc..
Must be able to perform complex pharmaceutical and patient care calculations (ie
renal dosing of medications, TPN orders and initial diabetes insulin orders)
Must be able to analyze situations and select a course of action.
Must be capable of multi-step and sequential problem solving activities; that include
comparing, analyzing and calculating data relevant to the facility.
Must be flexible and willing to undertake a variety of tasks.
Must possess the ability to accept change and respond appropriately.

WORKING CONDITIONS





Work is primarily performed within a clinic and hospital setting; occasionally the
employee may be required to perform job duties in a long-term care, University, and
conference setting.
May occasionally travel to other locations.
Temperature and humidity are regulated while indoors.
Noise levels may vary; however, there is continuous exposure to laminar flow hood
and ventilation fans
Proper lifting techniques and caution in handling materials must be used to ensure the safety of the
employee.
