St. Joseph’s Hospital Pharmacy Preceptors Jennifer Christofferson, PharmD, BCPP Clinical Pharmacist; Preceptor for Mental Health and Addiction Medicine rotation Jennifer received her degree from the University of Minnesota College of Pharmacy in 2008. She completed a PGY1 psychiatric focused residency at the University of Minnesota - Fairview Riverside campus in Minneapolis. She is an active member of the College of Psychiatric and Neurologic Pharmacists (CPNP). Jennifer led the initiative to advance pharmacy services into the behavioral health and chemical dependency areas at St. Joseph’s Hospital. She is involved in provider and nursing work groups for the Behavior Health and Chemical Dependency departments. Jennifer also teaches medication education classes for patients on the behavioral health and chemical dependency units, during which she concentrates on educating patients about their psychiatric medications. Christina Glasgow, PharmD, BCPS Clinical Pharmacy Coordinator Christina received her degree from the University of Minnesota. She has been board certified in pharmacotherapy since 2011. Her responsibilities include overseeing clinical pharmacy services for cardiology, critical care and bariatric patients. She serves on various committees including Heart Care Council, Cardiac Surgery Committee, Critical Care Council, Pharmacy and Therapeutics and Medication Safety. Her interests include cardiology, infectious disease, critical care, bariatric care and patient education. She is a member of the Minnesota Society of Health-System Pharmacists (MSHP) and has served on multiple program planning committees for MSHP. Christina is also a member of American Society of Health System Pharmacists (ASHP) and the American College of Clinical Pharmacy (ACCP). Leah Frantzen, PharmD, BCPS Residency Program Director; Clinical Pharmacy Manager; Preceptor for Orientation rotation Leah received her Doctor of Pharmacy degree from the University of Minnesota in 2004. She has been board certified in pharmacotherapy since 2010 and is a member of the American College of Clinical Pharmacists (ACCP), Minnesota College of Clinical Pharmacists (MCCP), Minnesota Society of Health-System Pharmacists (MSHP), and the American Society of Health System Pharmacists (ASHP). She has a clinical faculty appointment with the University of Minnesota, College of Pharmacy and is a preceptor for advanced pharmacy practice and introductory pharmacy practice experiences at St. Joseph’s Hospital. She currently oversees the decentralized pharmacists at St. Joseph’s Hospital and represents clinical pharmacy services on several committees. Sherri Jobin, PharmD, BCPS Clinical Pharmacy Coordinator Sherri received her PharmD degree from the University of Minnesota in 1988 and has been Board Certified in Pharmacotherapy since 1994. Her interests include infectious disease and anticoagulation. Sherri is a member of the HealthEast Pharmacy and Therapeutics Committee, Surgery Clinical Council, Bariatric Surgery Council, Infection Control Committee, and Anticoagulation Management Committee. She oversees the Med/Surg pharmacy team and HealthEast Antimicrobial Stewardship program and is co-chair for the Antibiotic Subcommittee. Sherri has represented HealthEast on the Institute for Clinical Systems Improvement (ICSI) committee since 2003 to develop guidelines and order sets for VTE prophylaxis for community members. Stacy Livingston, PharmD Pharmacy Operations Manager, Preceptor for Medication Safety rotation Stacy received her degree from the University of Iowa College of Pharmacy in Iowa City, Iowa. She completed a PGY1/PGY2 residency at the University of Wisconsin Hospital and Clinics in Madison, Wisconsin where she focused her training in both clinical and operational practices. Stacy joined the St. Joseph’s Hospital team in July 2014 and her focus is on drug distribution, sterile product preparation, quality improvement, pharmacy automation/technology and medication safety. She currently oversees the central pharmacist team, the pharmacy technicians and the pharmacy internship program. Stacy is a member of the Minnesota Society of Health-System Pharmacists (MSHP), the American College of Clinical Pharmacy (ACCP) and the American Society of Health-System Pharmacists (ASHP). Adrienne Lunde, PharmD Clinical Pharmacist; Preceptor for Staffing rotation Adrienne graduated from the University of Minnesota in 2013. She completed a PGY1 Pharmacy Practice Residency at Elliot Hospital in Manchester, New Hampshire. As a resident her areas of focus were cardiology and intensive care, which remain her clinical areas of interest. In addition, she is passionate about infectious diseases. Adrienne currently staffs in the Emergency Department and Cardiology units. In addition to acting as primary preceptor for the staffing rotation, she often leads topic discussions for the ED, cardiology and infectious diseases rotations. She is a member of the American Society of Health-System Pharmacists (ASHP) and American College of Clinical Pharmacists (ACCP). Rose Mueller, PharmD, BCPS Clinical Pharmacist; Preceptor for Critical Care rotation Rose received her degree from the University of Minnesota. She has been a pharmacist since 2002. Her current responsibilities are to provide pharmacy services to patients in the neurovascular ICU, surgery admit unit, orthopaedic and oncology units. She previously served as a co-preceptor for the neurology rotation and accepted the primary preceptor role for the neurology rotation in 2011. She became BCPS certified in 2011 and her continuing education is focused on neurology, ICU and hospital patient care. She is a member of American College of Clinical Pharmacists (ACCP), Society of Critical Care Medicine (SCCM), and the Neurocritical Care Society. She is a member the St Joseph’s Hospital scheduling team and the RAC committee. Brandon Ordway, PharmD, MS Director of Pharmacy for HealthEast Acute Care Hospitals; Preceptor for Practice Management and Leadership rotation, Residency Research Project Advisor Brandon received both his PharmD and MS degrees from the University of Wisconsin. He completed a two-year residency in Health System Pharmacy Administration and Leadership at the University of Wisconsin Hospital and Clinics. He is an active member of both the MSHP and ASHP. Currently, he is the immediate past chair of ASHP’s Section of Pharmacy Informatics and Technology. He is also a preceptor for the University of Minnesota College of Pharmacy, with an APPE rotation on pharmacy informatics, operations, and leadership. His practice interests include pharmacy automation and technology, quality improvement of complex systems, regulatory compliance, medication reconciliation, and personnel training and development. Matt Peterson, PharmD, BCPS Clinical Pharmacist; Preceptor for Medical Surgical rotation, Residency Research Project Advisor Matt received his PharmD and BS in Pharmaceutical Sciences degrees from South Dakota State University. Matt practices on the medical/surgical and central pharmacy teams. He has been board certified in pharmacotherapy since 2011 and is a member of American College of Clinical Pharmacists (ACCP). Matt assisted in the implementation and expansion of decentralized pharmacy services in the surgery admit unit. His practice interests include internal medicine and pain management. Megan Sandgren, PharmD, MS, BCPS Clinical Pharmacy Manager: Preceptor for Practice Related Research Project rotation Megan received her PharmD degree from North Dakota State University in 2013. She completed a PGY1&2 Health System Pharmacy Administration residency at the University of Minnesota Medical Center/Fairview Pharmacy Services and received her Master of Science in Social and Administrative Pharmacy from the University of Minnesota in 2015. At HealthEast, Megan assists in the clinical oversight of pharmacy services in the medicine, surgery, and OB areas and serves on various committees including the P&T Committee, OB Council, Spine Council, and Ortho Quality Council. She is also an active member of the Minnesota Pharmacists Association (MPhA), MSHP, and ASHP. Megan’s practice interests include internal medicine, transitions of care, metric and outcome reporting, and medication safety. Adam Servi, PharmD Clinical Pharmacist; Preceptor for Emergency Medicine rotation Adam received his PharmD from the University of Wisconsin-Madison. He completed a PGY1 residency at Ministry St. Joseph’s Hospital in Marshfield, Wisconsin, where he staffed as an emergency department pharmacist, became Healthcare Provider BLS, ACLS, and PALS certified and completed his year-long residency project in the area of patient satisfaction with pain management. In July 2012, he joined the St. Joseph’s Hospital pharmacy team as a clinical staff pharmacist working primarily in emergency and intensive care departments. He became a member of ASHP and MSHP, has interests in his current areas of practice, as well as infectious disease, and has helped to develop an emergency department antibiotic order set for prescribers at St. Joseph’s Hospital. Linsey Thompson, PharmD Clinical Pharmacist; Preceptor for Infectious Disease rotation, Residency Research Project Advisor Linsey received her PharmD degree from the University of Colorado. She completed a PGY1 residency at Saint Luke’s Hospital in Duluth, Minnesota. Linsey joined the St. Joseph’s Hospital pharmacy team in 2013 and practices in Medical-Surgical and Emergency departments. She became board certified in pharmacotherapy in 2016. Her interests include infectious disease, anticoagulation and emergency medicine. Linsey is a member of the American Society of Health-System Pharmacists (ASHP). Katie Zabel, PharmD, BCPS Clinical Pharmacist, Preceptor for Cardiology/Heart Failure rotation Katie received her degree from Drake University College of Pharmacy in Des Moines, Iowa. She completed a PGY1 residency at HealthEast Care System and joined St. Joseph's Pharmacy Department in 2010. She has been Board Certified in Pharmacotherapy since 2012 and is a member of the American College of Clinical Pharmacists (ACCP). Katie practices within the Cardiology and Medical-Surgical care teams. Her interests include cardiology, heart failure, critical care medicine and patient education. Within HealthEast, Katie serves on various work groups including Medication Reconciliation, Heart Failure and Pharmacy Operations & Engagement. She is actively involved with performance improvement initiatives related to heart failure and staff education of Joint Commission Core Measures and Best Practices.
