Child and Adolescent Behavior and Development Evaluations
Appointments



Initial evaluations are booked for 60 minutes. All initial evaluations will be assessed a
copay and will be subject to any deductibles as required by your insurance.
Follow-up evaluations are usually booked for 30 minutes. If you feel that your child has
a more complex problem or multiple problems that will need to be discussed, please ask
the receptionist to schedule a longer appointment.
Multiple follow-up visits may be needed until all issues have been addressed and goals
have been met.
How often do I need to be seen?

For all patients who are on medication for behavioral, mood, and/or developmental
conditions, there must be 2 medication checks and 1 complete review of general health
that includes a complete physical examination regardless of whether we are acting as
primary care physicians or consultants). Medications cannot be refilled unless you are
current with appointments. These visits can be completed in one of two ways:
o Option 1: A complete general health review without medication management
plus 2 separate medication management visits for a total of 3 visits
o Option 2: A complete general health review combined with medication
management plus one additional medication management visit for a total of 2
visits
NOTE: Copayment will be collected for all medication management visits,
whether or not they are stand-alone appointments or whether they are combined
with well visits/general health review per established billing practices and your
insurance guidelines. Thus, copayment amounts for Options 1 and 2 will be
identical over the course of 1 year. Deductibles will apply for all visits that
include medication management.


For Autism, Mood Disorders, and Eating Disorders, additional appointments may be
required depending on the circumstances.
Additional appointments may be scheduled as needed for any change of status.
NOTE: Anyone taking the medications Risperdal, Zyprexa, Seroquel, Abilify, Geodon, or
Invega will need blood work yearly.
How do I cancel an appointment?


Please call 303-655-1685 during regular business hours to cancel an appointment.
If you must cancel an appointment, please do so with at least 24 hours notice to avoid a
$100 service fee. This fee will be billed to you directly and not to your insurance carrier.
What if I am late or don't show up for an appointment?


If you show up more than 15 minutes late for an appointment you may be asked to
reschedule. As a courtesy to our other patients and in order to get the most out of your
visit, please show up on time.
All 60 minute appointments and new patient consultations must be confirmed with a
credit card or a cash deposit. Failure to show for one of these appointments will
generate a $200 charge.

When can I get my question answered by telephone or by patient
portal and when should I schedule an appointment?


Telephone consultation and messaging by way of the patient portal is appropriate for
brief questions about treatment, side effects of medication, or other minor educational
interventions. Most phone calls will be returned by one of our staff members trained in
the assessment of your child's needs. Most portal messages will be returned by the
provider. Portal use is secure and confidential and is strongly encouraged. To access

Office appointments should be scheduled for any questions that are not simple or brief,
issues that might prompt a change to a different medication, or complex educational
interventions.
How do I refill medication?

Medications that are classed as Schedule II by the DEA cannot be refilled, phoned in, or
sent electronically to a pharmacy. These medications require a new written prescription
every time the medication is refilled. These prescriptions cannot be post-dated,
however, once on a consistent dose of medication, we can write 3 months of
prescriptions at a time.
As an example: Let us say that today is 15 May 2014. You would receive 3
prescriptions dated 15 May 2014. The prescription that is to be used first will have no
further instructions. The prescription that is to be used second will say: “ Do not fill
before 11 June 2014.” The prescription that is to be used third will say: “Do not fill
before 7 July 2014.”
Please keep track of these prescriptions carefully. Original prescriptions will continue to
be written for free. Lost prescriptions that have to be rewritten will carry a $5.00
replacement fee.

Schedule II medications include the following: Concerta, Adderall, Adderall XR,
Focalin, Focalin XR, Vyvanse, Metadate CD, Ritalin LA, Daytrana, Dexedrine
spansules, Quillivant XR.
Medications that are not Schedule II and can be refilled, phoned in, or sent
electronically to a pharmacy include: Strattera, Intuniv, Kapvay, Tenex, Zyprexa,
Seroquel, Tegretol, Abilify, any antidepressant, Trileptal, Buspar, Risperdal, Geodon,
Invega, Depakote, and Lamictal.
How much time do I need to give Premier Pediatrics to have a Schedule
II medication ready?




If you would like to personally pick-up the medication, please call the office at least 3
business days in advance. Due to the large volume of prescriptions that are written,
same day refills are not possible.
If you live greater than 15 miles from either office and would like us to mail the
prescription to you, please notify us at least 1 week before you need the prescription.
If you would like to send a prescription to a mail-order pharmacy, please notify us at
least 3 weeks before you need the prescription.
Certain local area pharmacies have agreed to pick up prescriptions from us for your
convenience. These pharmacies are: Platte Valley Pharmacy and Dale Professional
Pharmacy. This service is subject to change as needed.
