Communicable Disease Title Prevention and Control The Communicable Disease Prevention and Control Unit works to prevent and control the spread of over 70 infectious diseases within SCC. This responsibility is achieved through collaboration with those involved in the identification, diagnosis, treatment, and legal, ethical and social management of communicable diseases. ProgramsServices General Communicable Disease Monitors trends using surveillance data; detects and investigates outbreaks; identifies infectious agent(s) responsible for disease transmission; disseminates control recommendations to prevent additional cases of disease Enhanced Surveillance Conducts seasonal illness surveillance (e.g., West Nile Virus, influenza, etc.); monitors emergency department data in near real-time; examines trends in disease syndrome groups; provides situational awareness; collaborates with partners to react quickly to public health events HIV Surveillance and Prevention Monitors trends in incidence and prevalence of HIV; provides counseling and testing services to clients living with HIV Infection Prevention Detects and prevents outbreaks in out-of-hospital settings, including, but not limited to nursing homes, long-term care facilities and child care providers Sexually Transmitted Infections Monitors trends in incidence and prevalence of chlamydia gonorrhea and syphilis cases; performs contact tracing for STI/ STDs; ensures appropriate treatment for STI/STDs Tuberculosis Surveillance and Prevention Monitors trends in incidence and prevalence of tuberculosis cases; conducts contact investigations; maintains logs of persons with latent tuberculosis infections Vaccine Preventable Diseases Surveillance and Prevention Monitors trends in VPDs using surveillance data; detects and investigates outbreaks; disseminates control recommendations to prevent additional cases of disease; manages Vaccines for Children Program and collaborates with CCDPH IHSS on Perinatal Hepatitis B Prevention Program The Communicable Disease Prevention and Control Unit can assist schools and child care providers with the following issues: • active disease surveillance • outbreak prevention and management • immunization requirements For more information Phone 708-633-8030 Cook County Department of Public Health 2011 20
