*In order to register your team a deposit of $100.00 is required. This form must be filled out
completely and sent to 365 Sports Complex along with your deposit. The remaining balance
must be paid before your first game. NO EXCEPTIONS. Coaches are responsible for their
players, parents, assistant coaches, paper work and fees. All teams must follow the age
groups that are set. No refunds will be given once the session officially begins NO
EXCEPTIONS.
