I am writing to request copies of my child’s educational file (CUM File) pursuant to the Cal. Educ. Code
Section 56504.
My name is [parent/guardian's name], and I am requesting a copy of my child’s CUM file [child’s
name] who is in [include the grade level] and attends [name of school].
Please include all records or provide a complete copy, of any files, documents, and other materials that
are maintained by the school system and contain information relating to my child. This includes all
records referring to my child in any personally identifiable manner – that is, records containing my child’s
name, Social Security number, student ID number, or other data making them traceable to my child such
as assessment reports, assessment protocols, assessment plans, progress reports, report cards,
Individualized Education Program (IEP) documentation and notes, medical records, e-mails, and any
other records that personally identify my child.
A copy of my child’s CUM file must be submitted to me within 5 school days of the date of this letter

The parent shall have the right and opportunity to examine all school records of his or her child
and to receive copies within five business days after the request is made by the parent, either
orally or in writing.
A public agency may charge no more than the actual cost of reproducing the records, but if this
cost effectively prevents the parent from exercising the right to receive the copy or copies, the
copy or copies shall be reproduced at no cost.
