Outreach Activity/ Event Tobacco Cessation Support Groups Ed Shultis Prostate Cancer Support Group Plan Do Check Act (Data collection, comparison, best practice, conclusions) (Interventions, guidelines, test sample before broad implementation, etc.) (Review the test, analyze the results and identify opportunities) (What we will do to continually improve) This is an evidence-based smoking cessation program offered to individuals who are thinking about or preparing to quit. The group will help with relapse prevention and provide continuous support. Will continue to support the staff members who volunteer to facilitate these classes. They meet regularly. Bassett facilities the provision of when possible. Eric N. Bravin, MD presented on the topic "Choosing Wisely" in May 2015. Bassett CTC’s community outreach program will continue to support the survivors who volunteer to facilitate these classes. Well received; no changes recommended On May 19, 34 gentlemen and their spouses attended. Between 2000–2001 and 2008–2009, adult smoking prevalence in New York State declined from 22.4% to 17.4%, and the percentage of adult smokers who made quit attempts increased from 54.5% to 65.6%. This program run by employees who volunteer their time recognizes the importance of assisting the public or staff with tobacco cessation and offering resources to help someone quit. Prostate cancer is the most common malignancy in men. With improvements in cancer-specific survival, treatment-related morbidity has become more relevant to the long-term health of prostate cancer survivors. On Sept 29, 17 were in attendance including 5 non-members. Dr. Hellenthal presented on prostate cancer signs and symptoms on September 2015. 6/6/15 National Cancer Survivor’s Day Cancer Survivors are described as individuals who have received the diagnosis of cancer, from the time of diagnosis throughout the rest of their lives Bassett Cancer Institute hosts a luncheon to celebrate Cancer Survivor’s Day. This event is a celebration of life for the cancer survivor, their family member or caregiver. Initiatives include Promote tobacco use cessation, especially among low SES populations and those with poor mental health. with poor mental health Participate in project team meetings, as appropriate; review meeting minutes and progress in meeting DSRIP goals Melanoma, the most deadly form of skin cancer, is the eighth most common type of cancer among both men and women in New York State; each year more than 3,700 NYS residents are diagnosed with melanoma and about 500 New Yorkers die from melanoma; 90% of skin cancers can be prevented by taking basic precautions. A majority of melanoma cases, the deadliest form of skin cancer, are attributed to excessive, unprotected ultraviolet (UV) exposure. Cancer screening programs apply screening guidelines to detect cancer at an early stage, which improves the likelihood of increased survival and decreased morbidity. A process is in place to follow up on all positive findings. In New York during 2010: -77 percent of women 40 and older were screened for breast cancer. -84 percent of women 18 and older were screened for cervical cancer. -70 percent of adults ages 50 and older were screened for colorectal cancer. -Lack of health insurance coverage is a barrier preventing adults from obtaining recommended cancer screening exams. Bassett’s mobile screening coach is utilized for outreach screening events for uninsured and underinsured men (colorectal FIT testing) and women (breast, pap, FIT test) in three-county region. Multiple events are held monthly throughout tri-county region, including special events at Relay for Life, county fairs, etc. Reports are submitted to the NYSDOH monthly to assess; process in place for follow up for positive results Well received; no changes recommended Clients Screened YTD 238 Breast F/U - 73 Cervical F/U - 11 Colorectal F/U - 31 Breast Cancer - 1 Cervical Cancer - 2 Colon Cancer - 1 Annual Health Fair educating the local community about health and safety, as well as promotion of cancer screening. The Bassett Cancer Center had a strong presence at the 2015 event. The Cancer Services Program was in attendance as well as six staff members who volunteered their time to promote cancer screening. 3rd Annual Bassett Cupola 5k race is being held in October 2015. Sponsored by local businesses, the 5k is run by Bassett’s Wellness Committee. Cancer Services Program will continue to attend event. No changes recommended, the event is being planned for next year. 40 adults and 15 children attended the 2015 event. This event is held annually at the Bassett Cancer Institute – mainly geared towards women – with local sponsors, food, and educational events. Recently, it was opened to the public for mammography screening. A process is in place to follow up on all positive findings. Well received; no changes recommended The Cancer Services Program recruited one person. Obesity rates continue to rise, and almost one-quarter of New York adults were obese. In 2011, only 27 percent of adults in New York consumed five or more fruits and vegetables a day; nearly 80 percent of New York adults did not meet recommended physical activity guidelines, and over one quarter did not exercise at all. Cancer is the second leading cause of death in New York State. Approximately 288 New Yorkers are diagnosed with cancer each day and more than 96 New Yorkers die each day from cancer. 39 women received screening mammograms at the event. Ongoing School Based Health Center HPV Screening Virtually all cervical cancers are caused by Human papillomavirus (HPV). In 2011, only 34 percent of females aged 13 to 17 in New York had received all three recommended doses of the HPV vaccination. HPV can affect both men and woman Some types of HPV can cause cervical cancer and oropharyngeal cancer (throat) No way of knowing which people with HPV will develop cancer HPV education provided to appropriate students through School Based Health Centers Bassett Healthcare’s educational campaign is provided throughout Bassett’s SBHC in an effort to assist students and their parents in better understanding the benefits associated with HPV vaccination. Well received; no changes recommended 19 SBHC’s in 14 school districts within a 4 county region. 10/3/15 Fly Creek Cider Mill Big Squeeze Event Fly Creek, NY- The Fly Creek Cider Mill & Orchard will feature its 16th annual "Big Squeeze Weekend" in October. The event is designed to increase awareness and early detection of breast cancer Bassett Cancer Institute's mobile medical screening coach will be in attendance. Cancer prevention and screening information will be offered along with tours of the coach. The Delaware, Otsego and Schoharie Counties' Cancer Services Program will provide free health screenings for uninsured women over 40 and uninsured men over 50. Local business sponsorship; wellness recognition and support for Bassett Healthcare Network and region. Well received community event. No changes recommended About 2000 people attended the event, around $2200 was raised. The Cancer Services Program was in attendance. 11/19/15 Rural Health Fair In Celebration of Rural Health Day, a Rural Health Fair will be sponsored by the Rural Health Network of Schoharie, Otsego and Montgomery counties. The Cancer Services Program will be in attendance promoting screening services for the underand un-insured rural populations. Community event, no changes recommended. 75 people attended the event. The Cancer Services Program was in attendance and signed up two attendees for screening care. To decrease the number of smokers nationwide, Bassett Healthcare Network, in coordination with the nation, will be participating in the Great American Smokeout. Bassett employees who are former smokers volunteer their time to share their quitting success stories. Community organizations will be sponsoring, offering a wide-range of services and education including nutrition information, dental health awareness, diabetes management, flu prevention, and many others.
