Prolaris® Patient Guide for Medicare Beneficiaries Introduction: This guide describes the safe use conditions of the Prolaris test for Medicare Beneficiaries with localized prostate cancer. Your doctor ordered Prolaris to obtain more information about your prostate cancer tumor so he/she can more precisely tailor and recommend a treatment strategy based on your personal risk factors. Purpose: You may know your prostate-specific antigen (PSA) level and your Gleason score. These tests tell your doctor how far your cancer has progressed, but not necessarily how fast it is growing. Prolaris is a measure of how fast the cells in your tumor are dividing, or its “aggressiveness”. Since you have had a biopsy, that tissue sample can be used to perform the Prolaris test, which measures the expression of certain genes that control cell growth and determine how fast your cancer is growing. Test Results: Since cancer is the uncontrolled growth of abnormal cells, an aggressive prostate tumor carries the potential risk of growing and spreading rapidly to other parts of the body. The Prolaris test result will help you and your doctor determine if your prostate cancer tumor is more aggressive, less aggressive, or similar to the cancers of other men who have similar PSA and Gleason scores. Treatment Plan: Your doctor will recommend a treatment plan based on several factors. These factors include PSA score, Gleason score, tumor stage, Prolaris ScoreTM, age, symptoms, general health, and patient goals for managing prostate cancer. There are a variety of treatment options your doctor will share with you. Some of the treatment options have a risk of complications and could cause side-effects. Prolaris may help you and your doctor to make more informed choices about how you manage your prostate cancer. Together you and your doctor should develop a treatment plan that balances your goals, the expected results, and the risks of complications or side-effects. Regardless of the treatment options you choose, it will be important for you to maintain regular contact with your doctor to ensure proper follow-up and control of your cancer. Safe Use Conditions: The Prolaris test result is not intended to replace the existing clinical and pathologic information about your prostate cancer or your doctor’s judgment. After receiving your Prolaris result and regardless of which treatment is chosen, it is important to follow your doctor’s recommendation for regular follow-up appointments and monitoring.
