2. JOB PURPOSE
The post holder will work as part of a team providing nursing services to meet the needs of the practice population,
including treatment room work, chronic disease management, health promotion and sexual health services.
They will also be responsible for raising awareness of health and social wellbeing with knowledge of issues
influencing the broader context of health. This will involve the understanding of occupational health risks and
lifestyle; gaining knowledge of epidemiology and local demographic data that affect health; taking a holistic
approach and undertaking opportunistic health promotion during routine consultations, with the aim of enabling
people to improve their own health.
The practice nurse will be required to work collaboratively with a variety of healthcare agencies and practitioners
to tackle health inequalities. These include specialist nurse practitioners, referrals to community outreach projects,
awareness of self help groups, local amenities and opportunities for adopting a healthier lifestyle.

Working within a General Practice setting, providing nursing care for all patients, children and adults who attend
the practice. Practice population approx. 7,100 patients.

5. ROLE OF DEPARTMENT
Providing general nursing services to a practice population as part of a multidisciplinary team. Planning,
implementation and delivery of exisitng and new nursing services to meet with national strategies and local
arrangements, within a robust clinical governance framework.
6. KEY RESULT AREAS
To have or develop skills in the following:

Clinical assessment and management of patients presenting with various conditions including chronic
disease management, cervical screening and travel health. Initiation and performance of diagnostic testing
provided by the practice, with the ability to act on results within practice guidelines. Patients may be
referred by GPs or self-refer.

Undertake when appropriate any treatment room duties – Venepuncture, dressings, complex wound
management, and aural care.

Actively engage patients and carers in the promotion of a healthy lifestyle, using current national and
local health promotional strategies including opportunistic screening and intervention.

Provision of a family planning and sexual health service to patients including emergency contraception as
determined by the practice. Supply or administration of medication/vaccinations using Patient Group
Directions (PGDs) or using registered qualification of Extended/Supplementary Prescribing.

Ordering and safe storage and handling of medicines, supplies, dressings and other practice equipment.
The maintenance of the cold chain for vaccine storage and the checking of fridge temperatures daily.
Implementation of Health and Safety policies as laid down by the practice, ensuring a safe environment
for patients and staff. Managing the disposal of clinical waste and spillage.

Responsible for the safe use of all diagnostic and treatment ranging from basic, syringes,
sphygmomanometers to complex equipment requiring additional training or use i.e. Spirometry,
Defibrillator and other emergency equipment.

Documentation and accurate recording of nursing intervention in patient’s computer record or medical
notes and templates for computer data entry, in accordance with all NMC documentation and working
regulations.

Maintain high standards of infection control.

Recognise and work within own competence and professional code of conduct as regulated by the
Nursing and Midwifery Council(NMC),

Work within established practice protocols and guidelines and contribute to their development and
evaluation.

Communicate effectively with patients and carers, recognising the need for alternative methods of
communication to overcome different levels of understanding, cultural background and preferred methods
of communicating.

Communicate effectively with other members of the primary care team and other agencies, referring
patients to other members of the health care team in line with agreed protocols.

Undertake statutory training and any other training as appropriate for the post.

Maintain IT skills necessary to perform role of Practice Nurse.

Summarising new patients notes after amalgamation with previous records.
Follow local policies regarding Confidentiality, Data Protection, Health and Safety, Equality and Diversity.
8. ASSIGNMENT AND REVIEW OF WORK
The post holder will be expected to prioritise their workload, anticipating and resolving problems.
Treatment room activity is generated by the demands of the practice population and other health care professionals.
The practice team will review work with planned meetings to address practice requirements and patient demands.
Primary Care Nurse Advisor is available for professional nursing issues arising from ongoing and future
professional development.
Professional development plans and annual appraisal will be undertaken by the employers.

9.KNOWLEDGE, TRAINING AND EXPERIENCE
Quality
Registered first level nurse with minimum 3 years post registration experience.
Community Health Experience, minimal 2 years
SVQ Practice Nursing
Evidence of ongoing continuous professional development
Evidence of Chronic Disease Management experience, supported qualifications
at diploma level and local experiential and theoretical training.
Basic IT Skills
Evidence of Cervical Cytology education and updated practice.
Family Planning Qualification
Recognised Education in Travel Health
Evidence of experience in developing and teaching junior staff.
Experience of multidisciplinary team working.
Good interpersonal skills
