Dear Faculty and Staff Advisors:
All of us know of AMC students who demonstrate strong leadership on this campus and in the community. Whether it be
through their involvement in student organizations, in the community or individual contributions to the University, it is
important that these students and student organizations be recognized for their outstanding efforts on behalf of our
campus. It is in this spirit that the Campus Student Services Office and the AMC Student Senate is pleased to announce
that we will be holding the sixth annual University of Colorado Anschutz Medical Campus
Excellence Awards for Student Organizations, Leadership, and Service
for the 2013-2014 academic year
The award categories include the following:
Outstanding Student Organization Leader
Outstanding Student Organization
Outstanding Community Service by an Individual Student
Outstanding Community Service by a Student Organization
Outstanding Service to the University by an Individual Student
The AMC Student Senate is asking that you please embrace this opportunity to assist in recognizing outstanding student
leaders on campus by nominating outstanding student leaders and student organizations, and individual student
contributors of service to the University. Attached please find the selection criteria as well as the nomination forms. If you

Please return the nomination form/s no later than 12:00noon on Friday, April 4th to the Campus Student Services
Office and the AMC Student Senate located in Ed II North, 3123 (Student Services suite), or via email to [email protected] .
Because the awards committee will be made up of representatives of AMC Student Senate as well as each of the
school/program/college student councils, these governance/leadership groups are ineligible for these awards.
Awardees, as well as their nominators, will be notified by Wednesday, April 23rd. They will be invited to as well as
recognized at the May 12th AMC Student Senate end of the year dinner.

Using the award descriptions on the following pages, please provide your nomination of this student or student
organization. In addition, please make sure you answer all questions related to the award and provide any supporting
materials necessary. All degree-seeking students and recognized, registered AMC student organizations are eligible for
awards listed below. Additionally, we have included an award for an individual degree-seeking student’s service to the

OUTSTANDING STUDENT ORGANIZATION LEADER
Eligibility:
Any student enrolled as a degree seeking student in a degree program at the Anschutz Medical Campus who has provided
outstanding leadership to a recognized and currently registered student organization at AMC during the 2013-2014
academic year.
Criteria:
Recognizes a student leader who has provided exemplary service, commitment, and strong leadership to a student
organization.





Made a meaningful contribution to members of the organization. Motivated the group.
Made him/herself available to student organization members and utilized the strengths and talents of individuals
in the group.
Exemplified integrity in their words and actions.
Demonstrated initiative by seeking ways to challenge the present and enrich the future.
Led the organization in its efforts to participate in interdisciplinary activities, presentations, etc.
An in-depth explanation about why you are nominating this student for an award, including:
1) Question(s) to be answered by nominator:
Please describe one particular example/situation reflective of this individual’s leadership qualities.
What particular quality distinguishes this individual from other outstanding student leaders?
What contribution(s) has this student made to the Anschutz Medical Campus that deserves recognition? In

OUTSTANDING STUDENT ORGANIZATION
Eligibility:
Any recognized student organization (affiliated or associated) at the Anschutz Medical Campus. The student organization
registration must be current with the Campus Student Services Office and the AMC Student Senate.
Registration/Affiliation Agreement forms are available at www.ucdenver.edu/studentsenate on the News and Forms page.
The Registration/Affiliation Agreement form must be complete with all signatures.
Criteria:
This award recognizes a student organization that demonstrated consistent flexibility, initiative, creativity, and
perseverance and made a meaningful contribution to its own student membership and the AMC campus community.
The student organization must have organized activities, events, and programs that have had a significant impact on the
quality of campus life.
An in-depth explanation about why you are nominating this organization for an award, including
1) Question(s) to be answered by nominator:
What has the student organization accomplished during the past year to deserve this recognition?
Describe how the student organization has worked with other student organizations, campus departments,
student academic communities, and/or community agencies.
2) List the Organization’s activities/events and describe how these impact the Anschutz Medical Campus

OUTSTANDING COMMUNITY SERVICE BY AN INDIVIDUAL
Embracing both the University community as well as the local Denver/Metro community through involvement and service
is a priority to many students at the Anschutz Medical Campus. Through commitment to enhancing our community via
acts of service, the individual student meets the needs of the community, engages in hands-on learning, and finds personal
fulfillment.
Eligibility:
Any student enrolled as a degree seeking student in a degree program at the Anschutz Medical Campus who has provided
outstanding community service through their own efforts as an individual, outside of any contributions they may have
made as a member of a student organization during the 2013-2014 academic year.
Criteria:
The award recipient must have a continuing commitment to community service as reflected in one or more programs and
initiatives, which are responsive to community needs, and exhibit a new or continuing partnership with the community.
(Note: Students whose service is limited to, or was initiated by participation in a course for academic credit are not
eligible for this award.)
An in-depth explanation about why you are nominating this student for an award, including:
1) Question(s) to be answered by nominator:
Please describe how this individual is participating in serving the community.
How is the outside community benefitting from this student’s efforts?
How does the campus community also benefit from these efforts?
2) Letter of recommendation: Please submit one letter of recommendation from another individual who can

OUTSTANDING COMMUNITY SERVICE BY A STUDENT ORGANIZATION
Embracing both the University community as well as the local Denver/Metro community through involvement and service
is a priority to many student organizations. Through commitment to enhancing life experiences of the members of our
community, student organization members meet needs, engage in hands-on learning, and find personal fulfillment via acts
of community service. Many student organizations find by providing service to the community, they further engage in
pursuing more sustainable commitments to outside organizations needing assistance.
Eligibility:
Any recognized student organization (affiliated or associated) at the Anschutz Medical Campus. The student organization
registration must be current with the Campus Student Services Office and the AMC Student Senate.

OUTSTANDING SERVICE TO THE UNIVERSITY BY AN INDIVIDUAL
Eligibility:
Any student enrolled as a degree seeking student in a degree program at the Anschutz Medical Campus who has
participated as a member of an AMC recognized student organization, or contributed their efforts as an individual (not
necessarily as part of a student organization) during the 2013-2014 academic year.
Criteria:
This award recognizes an AMC student who provided exemplary service to the University, and demonstrated commitment
to bettering the University through their efforts. The nominee must have been involved in University/campus activities
(through their academic program, and/or interdisciplinary efforts) and will have assumed responsibility in her/his area of
participation, while demonstrating strong service initiative.
An in-depth explanation about why you are nominating this student for an award, including:
1) Question(s) to be answered by nominator:
What contribution(s) has this student made to the University that deserves recognition?
In what way is the University and/or the Anschutz Medical Campus benefitting from this student’s
contribution(s)?
