Uma mulher de 37 anos, com história de alcoolismo
crónico e infecção por HIV-1, apresentou-se por lesões
cutâneas simétricas com hiper pigmentação de aparecimento progressivo nos últimos três meses, acompanhada por letargia nas últimas duas semanas. Estas
lesões apresentavam-se principalmente nos antebraços
e pescoço e foram previamente tratadas com corticoides, sem melhoria. Ao exame físico apresentava-se com
placas cutâneas eritematosas, hiperpigmentadas e
A 37-year-old woman with a history of chronic alcohol
abuse and HIV-1 infection was evaluated because of
the development of symmetric hyperpigmented skin
lesions appearing over the previous three months, and
progressive lethargy in the two previous weeks. The
lesions mainly affected the forearms and neck and had
been treated with steroids without showing improvement. Physical examination revealed symmetric welldemarcated erythematous, hyperpigmented
hiperqueratosicas que se assemelhavam a queimaduras
solares na região frontal, pescoço e antebraço (colar
e luvas de Casal, Figura 1). Contagem de CD4 era de
4/µL, viral load 300.000 HIV-RNA copias/µL, com
cortisol plasmático e ACTH dentro de valores normais;
sem doenças oportunistas. Iniciou terapêutica com
nicotinamide (200 mg/dia), tiamina, piridoxine, antiretrovirals, com abstinência alcoólica e apoio nutricional.
As lesões cutâneas e os sintomas mentais regrediram
no mês seguinte (Figura 2). Gaspar Casal, Gerona
1679 - Madrid 1759, humanista e médico, iluminou a
atmosfera cultural espanhola com ideias inovadoras e
promoveu acentuadamente neste pais a introdução da
Ilustração. Um dos seus maiores legados científicos foi
precisamente a primeira descrição precisa e elegante da
pelagra, que designou por “mal da rosa”.

CD4 count was 4/µL, viral load 300.000 HIV-RNA
copies/µL, and plasma cortisol and ACTH were
within normal ranges; no opportunistic diseases were
detected. Treatment with nicotinamide (200 mg/day),
thiamine, pyridoxine, antiretrovirals, along with alcohol
abstinence and nutritional support, were initiated. Skin
lesions and mental symptoms resolved in the following
month (Panel B). Gaspar Casal (Gerona 1679-Madrid
1759), humanist and physician, enlightened the Spanish
cultural atmosphere with his innovative ideas, and he
significantly promoted the introduction of Illustration
in this country. One of the most important scientific
legacies he left us in inheritance was the accurate and
elegant first description of pellagra (what he called
“mal de la rosa”).
