Ulcer Characteristics • Venous stasis ulcers – Typically shallow, irregular borders, variable size – Associated with large amount drainage – Often associated with increased pigmentation of skin • responsibility of the physician, nurse practitioner, or physician assistant to correctly diagnose wounds • It is the responsibility of the licensed nurse to correctly describe the wound and risk staged • Burns not staged, described as full/partial thickness • If eschar on wound, cannot determine stage until eschar removed- for MDS code as Stage IV • If there is eschar or thick slough, Stage
