Kent State University’s dental insurance has available a multi-tiered options Delta Dental plan. All of the options are part
of the Delta Dental PPO (Point-of-Service) program which gives you access to the Delta Dental PPO and Delta Dental
Premier networks. Each plan varies in benefits, Low, Basic, High, and AAUP High Plans. We have included information
below that will be helpful for making your plan selection.

Will I be receiving a dental ID card from Delta Dental?
New enrollees into the Delta Dental PPO plan will receive an ID card which will include your Delta Dental assigned ID
number. Your social security number may also be used. Delta Dental Web site at www.deltadentaloh.com.
What is my member group number?
Your group number is 1010, and will be shown on your ID card (however, a Delta Dental PPO or Delta Dental Premier
dentist does not need a group number for you).
Who should I call if I have a question about my coverage?
Should you have questions about your current claims or coverage, please contact Delta Dental Customer Service at (800)
524-0149. You can also access this information through Delta Dental’s Consumer Toolkit at www.deltadentaloh.com.

What are some of the differences in the plan options?
Low Plan covers Diagnostic & Preventive and Basic restorative treatment only. There are no Major services or
Orthodontic services covered under this option. Annual maximum $750.00.
Basic Plan covers Diagnostic & Preventive, Basic, and Major services. Annual maximum $1,000.00. In addition,
Orthodontic services are covered in Network Only*. Annual Orthodontic maximum $1,000.00.
High Plan covers Diagnostic & Preventive, Basic, and Major services. Annual maximum $1,250.00. In addition
Orthodontic Services are covered in Network Only*. Annual Orthodontic maximum $1,000.00.
AAUP High plan covers Diagnostic & Preventive, Basic, and Major services. Annual maximum $1,250.00. In addition,
Orthodontic services are covered up to age 19, for all providers (in or out of network). Annual Orthodontic maximum
$1,000.00.
*In the Basic and High Option, there is no Orthodontic age limit if utilizing a Delta Dental PPO network provider. There is
an age limit of 19 if utilizing a Delta Dental Premier network provider. No Orthodontic benefit if utilizing a non-network
Delta Dental provider.
How do I find a network dentist?
1. Go to www.deltadentaloh.com.
2. On the left-hand side of the page under “Enrollees,” click “Find a Provider.” Then, choose “Delta Dental”
3. Follow the prompts to find a dentist in your area.
You can also receive a list of participating dentists through the Consumer Toolkit or by calling Delta Dental’s Customer
Service department at (800) 524-0149.
Can I have more than one dentist?
Our Delta Dental PPO (Point-of-Service) plan give you full control in choosing your dentist. You can see any licensed
dentist you wish, and you and your family members are also free to see different dentists, if you prefer.
What if I want to change my dentist?
You are free to change dentists at any time. You do not need to notify Delta Dental of the change.
