Continuing Annual Recheck Requirements
(Second and Third Year Students)
Please use this form as a guide for remaining in compliance with all clinical guidelines throughout your time in the program.
Instructions for the renewal process will be provided by your Compliance Coordinator by June of each academic year.

current while in clinical course work
Safety and Health Online Training
ACLS (Advanced Cardiovascular Life Support) Certification
(ACLS course will be provided by the PA program.)
CPR Certification
(Must be the American Heart Association – BLS for the Healthcare Provider OR the American Red Cross –
CPR/AED for the Professional Rescuer.)

(Check if information is current – only needs to be updated if information has changed.)
